Job summary
Furness Rapid Response are looking for a Band 5 Nurse to join our team in the community.
This is a temporary post to cover Maternity Leave / secondment opportunity.
We are seeking a hardworking, dynamic, proactive, individual to join our well established multi disciplinary team.
You will support the delivery of the urgent community response standard which aims to provide a 2-hour crisis response to patients who are acutely unwell within our local community to avoid unnecessary hospital admissions. As well as working within the newly established Virtual Wards.
This is can be a challenging role , every day is different but exciting and rewarding.
Main duties of the job
The role requires fantastic communication skills, creative thinking and working proactively and collaboratively with others to ensure a high standard of care is delivered.
The role requires undertaking holistic assessments sometimes in complex and challenging situations to develop and deliver robust programmes of care for patients and carers. This is an exciting time to work in the community. The community is advancing and developing the urgent care response to prevent un-necessary hospital admissions.
You must be able to use your own initiative, work under pressure, keep to deadlines and have excellent nursing, organisational, interpersonal and communication skills. You should be flexible and able to work as part of a team.
We will provide a programme of induction and you will be appraised on a yearly basis with a view to developing knowledge and skills where appropriate.
With patient centred care as your main focus, you will deliver our core values, eg respect, empathy, consideration, compassion and dignity.
You will be joining an enthusiastic and motivated team who are friendly and approachable. The team works flexibly together to maintain support for each other and to meet our service delivery.
About us
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Job summary
Our JAG accredited Endoscopy service, based at Royal Lancaster Infirmary, is looking to recruit a Band 5 Nurse to join our established, motivated and friendly team. Our Endoscopy services are predominantly nurse led, from admission to discharge. It is therefore essential that our Nursing team are able to demonstrate a high degree of autonomy.
We are a 7 day service and are open from 08:00am - 18:30pm.
Our nurses are trained to assist Surgeons, Gastroenterologists, and Nurse Endoscopists, in a range of specialised procedures. This is supported by a dedicated competency framework, complimented by online learning and guidance from our Endoscopy specific, practice educator team.
There are 4 roles on the department, in which you will be trained in each area as part of your role.
Main duties of the job
Endoscopy is a great environment to learn new skills, and further your career. Many of our Nurses stay within Gastroenterology and take up specialist roles in Inflammatory Bowel disease, as Nurses Endoscopists and Bowel Cancer Screening Nurses.
The successful applicants will possess excellent communication and documentation skills, should be able to work well within a team, and demonstrate a keen interest in Endoscopy. They will work in close partnership with colleagues to ensure that the unit is compliant with JAG requirements. They will be actively involved in audit and governance to maintain standards.

Job summary
As a Medical Staffing eRostering / Rota Co-ordinator you will be responsible for supporting the Operational teams to provide a comprehensive, high quality and effective service in the delivery of junior doctor rota co-ordination ensuring minimum staffing is maintained at all times. You will also be responsible for securing and booking bank and agency doctors at the appropriate rates.
The post holder will support clinical team roll-outs, successfully implementing the full functionality of the eRostering system including self-service, roster creation, roster administration, roster analysis/ approval, time and attendance, management dashboards, bank module and validating rosters for payroll.
Educated to degree level or equivalent experience, knowledge of Rostering platforms providing an eRostering Service and Roster building experience is essential. Excellent interpersonal and communication skills, ability to work under pressure and work to project timescales is a must.
This role is to be worked over 5 days Monday to Friday
Main duties of the job
The key responsibilities of this role will include carrying out any tasks relating to the resourcing of Medical Staff for a designated specialty, ensuring an efficient process is followed to run an effective Medical Service 24/7, and minimise spend on locums whilst ensuring safe staffing..
The post holder will provide an efficient and high quality rostering data administration service supporting the use and maintenance of electronic rostering throughout the trust including integration with the temporary staffing system.
To deliver staff training and act as first level contact for enquiries from end-users.

Job summary
An exciting opportunity has arisen for a registered nurse to join the Cardiac Arrest Prevention (CAP) Team in a Sister/Charge Nurse capacity. The cardiac arrest prevention team provides a comprehensive service for the organisation including a significant training program for all clinical staff from basic through to advanced courses.
The teams main focus is to eliminate avoidable cardiac arrest through extensive audit programmes, rapid case reviews of any unexpected death or potential failure to rescue, teaching and providing professional support to clinical areas and individuals.

Main duties of the job

Some of your main duties will include (but not exclusive to)
a) In conjunction with other key professionals you will be accountable for a patient caseload and delegate responsibility to other professionals within the multidisciplinary team.
b) To apply advanced assessment of physical, mental and social care needs to provide clinical care and health promoting interventions that help avoidunnecessary hospital admission.
c) To interpret clinical signs and symptoms and diagnostic tests in order todevelop a treatment plan and /or treatment in conjunction with medicalpractitioners as appropriate. Provide advanced clinical advice to the widerhealthcare team to improve outcomes of patient care.

Main duties of the job
An opportunity has arisen for an enthusiastic Registered Nurse to join Gladstone ward based in Deeside Community Hospital.Deeside Community Hospital is a great place to work and feel part of a close community hospital team. The team enjoy a close working relationship and support from the hospital league of friends A range of outpatient's services also take place within the hospital site.Gladstone is a 20 bedded ward - escalated to 28. Both wards are supported by a Care of the Elderly medical team and have additional support from therapies to develop agreed rehabilitation plans with patients, with the focus on discharge to recover and assess.Patients admitted to Branwen and Gladstone wards are transferred either from acute hospital wards when medically fit or from the community to prevent the need for an acute hospital admission. Patients may require complex discharge planning, including the completion of long term assessments. Working as part of the nursing team, you will have the opportunity to develop excellent skills in patient assessment as part of their recovery as well as a range of nursing interventions. You will also work closely with community teams as part of the discharge planning process.
This post is fixed term until 31.03.2027.

Mersey Care NHS Foundation Trust is the health provider for the delivery of the Integrated Mental Health Team (IMHT) at HMP Altcourse. The service is designed to support the holistic mental health and learning disability needs of our client group, in partnership with physical healthcare providers and HMPPS colleagues.
HMP Altcourse are both Category B, male only, local prisons, in the Liverpool area.
The service provides individual and group-based interventions based on the clinical needs of the population. Interventions are individualised, and trauma informed, alongside comprehensive risk assessment and management.
Core aims of the service are to improve mental and psychological wellbeing via a range of interventions, build the development of independent core skills, reduce risk, and advocate for mental health and learning disabilities within the wider prison.
The post holder will be joining a dedicated team of clinical staff from a range of backgrounds and experience. Team members have high levels of skill, passion and enthusiasm (and humour!) and are committed to ensuring the on-going successful development of the service and the opportunities it provides for our population.
Main duties of the job
We are now recruiting x1 Registered Nurse Associates. The successful candidates will work under the supervision of a Registered Mental Health Nurse and will work alongside and be supported by the wider IMHT.
The post holder will have a key role in completing Primary Mental Health Screens for all new receptions in to the prison in order to identify mental health needs in a timely way. Other key roles and responsibilities will include mental health promotion, suicide prevention work, providing psychoeducation and information on coping strategies, supporting group activity and conducting some physical health checks.

Job responsibilities
- Practice autonomously with a high level of accountability at an advanced clinical level in the direct delivery of high quality care, making critical clinical decisions based on advanced clinical skills assessment.
- With the support from the Matron, Nurse Consultant, Advanced Nurse Practitioners (ANPs) and the Medical team, be responsible for a caseload of patients managing nurse led review clinics.
- Works in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. history-taking, holistic assessment, identifying risk factors, mental health assessments, requesting, undertaking and/or interpreting diagnostic tests and conducting health needs assessments).
- Collaborate with the multi-professional team to establish diagnosis and monitor disease progression in patients with ophthalmic conditions.
- Challenge practice so that the best and most appropriate treatment is given to the patient; maintaining and monitoring high standards of care, recognising and responding proactively to patients changing clinical needs.
- Under the supervision of the Matron, Advanced Nurse Practitioner (ACP), Nurse Consultant and medical team, work towards advanced ophthalmic practice competencies and non-medical prescribing as appropriate.
- Work autonomously, managing a group of patients to deliver high quality skilled, evidence based and patient centred care supported by departmental Standard Operating Procedures, policies and guidelines.
- Understand the workload and targets that has been set for clinical area and demonstrate commitment to their delivery.
- Practice as a professional nurse demonstrating leadership, clinical expertise and competence in the field of Ophthalmic nursing.
- Communicate and liaise effectively with internal departments and external agencies to arrange and confirm ongoing pathways of care, provide written communication and referral documentation as required.
- Receive patients in a calm, courteous and reassuring manner and ensure they are kept informed of progress and treatment, providing help and counselling when needed.
- Provides health education and advice to patients and their carers, teaching where appropriate.
- Maintain professional competence, authority and credibility by undertaking regular clinical sessions in accordance with your job plan and being highly visible to patients, carers and staff, actively seeking feedback about the quality of services.

- Be conversant with trust policies, procedure and systems for safeguarding including the assessment of Mental Capacity, the process for consideration requests for Deprivation of Liberty Safeguards (DOLS) and the application of the principles of Prevent (counter terrorism awareness).
- Report and raise concerns about Safeguarding, accessing advice and support at the point of need.
- Identify deteriorating patients and take the appropriate action to manage the situation, liaising with the multi-disciplinary team and external care providers as appropriate.
- Use professional judgement to intercede and act as advocate for patients to ensure a patient focused approach to the delivery of care.
- Support and enable patients and carers to make informed decision relating to their treatment and management, escalate concerns within professional structures as required.
