Job summary
An excellent opportunity has arisen for a Long-term condition nurse or ACP.
This role will play a key role in delivering anticipatory, personalised care for patients living with long-term conditions and frailty across community and care home settings. With a clinical background in managing conditions such as diabetes, COPD, heart failure, and multimorbidity, the postholder will reduce avoidable deterioration, support independent living, and improve quality of life through holistic assessment, care coordination, and early intervention. Working within an integrated multidisciplinary team, the nurse will champion continuity, equity, and proactive planning particularly for vulnerable and housebound cohorts.
One of the main aims of the proactive care team is to help reduce unplanned hospital admissions and A&E attendances.
As an autonomous practitioner, the successful candidate will ideally be a non-medical prescriber, able to demonstrate critical thinking and decision-making relating to long term conditions.
We welcome applications from individuals who enjoy providing outstanding patient care, have a keen interest in being part of a team, striving to improve patient experience and outcomes, with excellent communication and IT skills.
We are also happy to consider applications from individuals who are already employed by other NHS organisations and explore secondment opportunities for the right candidate with that organisation.
Main duties of the job
Clinical Practice
- Identify and stratify patients with moderate to severe frailty using validated tools (e.g. eFI, Rockwood)
- Conduct comprehensive biopsychosocial assessments, including cognition, mobility, nutrition, and social support
- Apply specialist knowledge in managing long-term conditions, such as Diabetes, Respiratory Disease and/or cardiovascular conditions.
- Develop and review personalised care and support plans in collaboration with patients, families, and MDTs
- Monitor for early signs of deterioration and intervene to prevent crisis or hospital admission
- Facilitate advance care planning, DNACPR discussions, and end-of-life care coordination
Coordination & Collaboration
- Work closely with care home staff, community matrons, geriatricians, ARRS roles, and social care teams
- Participate in MDT meetings and contribute to integrated neighbourhood team (INT) working
- Liaise with voluntary sector partners and social prescribing link workers to address non-clinical needs
Quality & Improvement
- Maintain accurate, timely documentation in shared care records and contribute to PCN dashboards
- Use data to identify equity gaps and target interventions for low-uptake or high-risk cohorts
- Contribute to service evaluation, patient feedback, and continuous improvement initiatives
- Support education and training for care home staff and carers around frailty and LTC management

The Eating Disorders Service in Gloucestershire is growing and as part of Community Mental Health Transformation a review of all posts has been completed. The service provides specialist assessment, treatment and support for individuals who are experiencing an eating disorder and their families within Gloucestershire.
The GHC Eating Disorders Service is recruiting an experienced and enthusiastic senior clinician to work within a specialist mental health service. The role will predominately focus on the treatment of Children an Adults with a complex presentation. The service is based in Cheltenham and Gloucester holding a caseload of 700 clients.
The post holder will assess, clinically manage, care co-ordinate and treat children, adolescents and adults with an eating disorder and their families. Working alongside other health care professionals including RMN's, RGN's GP's, Dieticians, AHP's, Psychologists and Consultant Psychiatrists in order to support service users within their recovery.
Main duties of the job
The candidate will hold excellent communication skills, be enthusiastic in their approach and demonstrate an understanding of the complexities involved in this specialist area of mental health
- To work with the supervision of the Highly Specialist Clinician and Band 7 Clinicians with patients with clinically significant eating disorders
- To assess patient and family needs and devise an appropriate care plan, communicating the plan in writing to the patient with copies to their GP, referrer and other involved parties
- To participate in the measurement of outcome, timely recording of clinical notes and communication of any significant clinical changes including ending of treatment in writing to the patient and their GP
- To provide evidence based treatments for children, adolescents and adults and their families as appropriate (currently Family Based Treatment [FBT], and Cognitive Behaviour Therapy-Enhanced [CBT-E]
We have a team of highly dedicated and skilled clinical staff and due to a surge in demand following the Covid 19 pandemic we are looking to expand our service.
We will provide the successful candidate with a high level of psychosocial training including CBTE and family based therapy.

Job summary
The Royal Wolverhampton NHS Trust are looking for a highly motivated, dynamic and enthusiastic nurse to work within the Specialist Palliative Care team, 7-day service including bank holidays. The successful candidate will play a key role in shaping and delivering an innovative service model that aims to provide compassionate, high-quality care to patients with life-limiting illnesses and their families. Main duties of the job
As a band 6 clinical nurse specialist you will deliver expert clinical practice to patients with complex specialist palliative and end of life care needs. Your role will encompass elements of clinical practice, education, and service development whilst maintaining a strong focus on patient centered holistic care.
The successful candidate will work as part of the Hospital based Specialist Palliative Care Team. You will have experience in delivering end of life care and have the competence and confidence to assist in the management of patients with specialist palliative care needs. This will involve giving advice on symptom management, advance care planning , and delivering education to wider teams. You will work closely with members of the multidisciplinary team and will have excellent communication skills.

Job summary
We have an exciting opportunity for an Infrastructure Manager to join the Infrastructure team within the Unified Communications Division at Cambridge University Hospitals NHS Foundation Trust. The Unified Communications Infrastructure team has a broad role in enabling communication across the hospitals. This includes telephony, mobile and unified communications services as well as infrastructure supporting IT services for these highly regarded hospitals.
The Infrastructure Manager role is key in delivering and managing the primary Data Centre and over 100 cab room environments, all layer one infrastructure as well as telephony services across the Biomedical Campus. The candidate will have good project management and presentation skills to deliver the necessary programmes of work.
This is a Linked Grade role, with the salary dependent on experience. It is a requirement of the role for the candidate to hold a Certified Data Centre Management Professional (CDCMP) certification, or equivalent cabling or networking accreditation. If the successful candidate does not meet the criteria for qualifications the starting grade will be band 6, progressing to band 7 following completion of the certification within the first year in post.
Main duties of the job
To deliver and manage the tier 2/3 standard Data Centre and cab room infrastructure to meet high availability targets.
To be responsible for drafting standards, protocols, prioritising and managing an uplift programme to deliver all aspects of the warranted cab room environment for the above (including cooling, uninterrupted power, environmental monitoring and reporting) across the CUH estate.
To be responsible for cabling standards for voice and data services ensuring all performance and fire safety requirements are met.
To support the service desk engineers in managing VoIP, hybrid and mobile telephony services across the Trust.
To work alongside a number of other Trust departments, third party service providers and contractors to meet these requirements.

Main duties of the job

You will work collaboratively with our GPs and wider multidisciplinary team, independently prescribing within your scope of practice, interpreting investigations and referring appropriately where further specialist care is required.

Ensure robust systems are in place to meet statutory safeguarding responsibilities for children and vulnerable groups.
Work in partnership with midwifery, medical, AHP and corporate teams to ensure integrated pathways across maternity, neonatal and paediatric care.
Support delivery of key performance indicators including waiting times, flow, harm reduction, and patient outcomes.

Job summary
The Senior Engagement Officer helps the Health Research Authority (HRA) to make change happen. This is a key role within the Policy and Partnerships Directorate, supporting projects and engagement activities that form part of the Policy and Partnerships Directorate's busy work plan.
Main duties of the job
- Work with colleagues develop and implement communications and engagement plans for different HRA projects, reporting on these where relevant.
- Plan and deliver events with external stakeholders and members of the HRA community, including workshop sessions and webinars, that support the HRA's policy development and increase our influence to deliver our mission.
- Monitor issues affecting our stakeholders, and developments in the wider health and social care research environment. Share this information, showing how this impacts the HRA.
- Support collaboration with stakeholders including patients, the public and research professionals through projects and forums.
- Represent the HRA at events and meetings with stakeholders.

Job summary
To provide high-quality, evidence-based nursing care to critically ill and high-dependency children and young people within the Paediatric HDU/Level 2 Critical Care environment.
The post holder will work as an experienced clinical practitioner and provide effective leadership and supervision to the nursing team, ensuring safe, effective and child- and family-centred care. They will act as a role model for junior staff and support the Band 7/ward leadership team in maintaining high standards of clinical practice, patient safety and service delivery.
The post holder will demonstrate an ability to recognise and respond promptly to the deteriorating child, prioritise workload, coordinate the clinical area and escalate concerns appropriately.

The successful applicant will after training be expected to hold the Paediatric bleep supporting the unit, attending medical emergencies as well as supporting with Bed Management responsibilities.

Job summary
The Specialty Doctors will be expected to exercise an intermediate level of clinical responsibility but ultimate responsibility for the patients they treat will rest with the relevant consultant. At least one session per week will be under consultant supervision.
This is a 10 PAs post with up to 12 PAs negotiable. The core hours are 08:00 to 18:00 and on call shifts are 08:00 to 20:30 and 20:00 to 08:30. The rota is currently 1:8 and includes normal days, long days and nights. Full details will be discussed at interview.
Main duties of the job
Responsibility will be to the Consultant Lead for Specialty Doctors and the Clinical Director. The successful candidates will have the opportunity to review the job plan with the Clinical Director or representative at least once a year.
The post holder will form one member of a full shift rota which may be in theatres, ICU or obstetrics. In addition to daytime ICU shifts, work allocated may vary according to requirements and may include daytime PAs in main theatre, day surgery and obstetrics according to experience.

Why you should join our TeamAs part of Leeds Cancer Centre, we provide specialist cancer care for patients in a variety of inpatient, day case and outpatient wards and departments. Our specialist teams include medical oncology (chemotherapy), clinical oncology (radiotherapy and brachytherapy), clinical haematology, surgical oncology (thoracic, breast and gynaecology) and specialist palliative care.
What you will bring to the roleYou will play an essential role supporting Matrons and Senior Sisters/Charge Nurses by facilitating education, training and practice development within Oncology, Clinical Service Unit (CSU), establishing a high standard of education for all registered and unregistered nursing staff. You will be responsible for implementing evidence-based, patient-focused practice and will work within the clinical area, working with staff to facilitate the development of skills in order to integrate education and practice development, thus enhancing quality care delivery from both the registered and unregistered workforce.
You will help support our exciting and existing development sessions tailored to all nursing staff grades, which will require you to be organised and motivated.
The job holder will have significant experience in a junior leadership role and demonstrate working with a wide range of professionals, including medical, nursing and management colleagues. The job holder will have experience in audit and standard setting and be able to demonstrate lateral thinking, a positive approach to change and service improvement.
