Job summary
The Learning Lead will provide expertise in the delivery, coordination and evaluation of mandatory learning, induction and colleague development across Provide Care Solutions through engaging and effective learning solutions.
They will play a key role in ensuring colleagues have access to high-quality training and development opportunities that support safe, effective and compassionate care. The post holder will lead on the delivery and continuous improvement of corporate induction, the Care Certificate programme and mandatory training provision, ensuring colleagues are equipped with the knowledge, skills and behaviours required to fulfil their roles.
Main duties of the job
Main duties of the Learning Lead are:
- Support the management and delivery of mandatory training across the division to ensure colleagues complete required learning within agreed timescales.
- Lead on the delivery and continuous improvement of Provide Care Solutions Induction programmes, ensuring it provides a positive, engaging and informative experience for all new colleagues.
- Support the development and maintenance of online learning resources within the Learning Management System.
- Maintain up-to-date knowledge of learning and development practice, mandatory training requirements, adult learning methodologies and developments within health and social care

Main duties of the job
We are looking for nurses with exceptional communication and interpersonal skills to join our team. As a Staff Nurse, you will be responsible for delivering high-quality, patient-centred nursing care. Working as part of a multi-disciplinary team, you will support patients with complex needs, ensuring they receive the highest standard of physical, psychological, and emotional support. You will also act as a role model and mentor to colleagues, junior staff and students, fostering a caring and professional environment.

Main duties of the job
You will be the professional clinical link between all areas of care for individuals with Parkinson's/Headache and provide care and support for patients from diagnosis through to ongoing treatment and care. An exciting opportunity has arisen for an registered nurse who is experienced or looking to develop skills in acute care at home nursing to join the Acute Care at Home Team based at St Barnabas Hospital, Saltash.

Main duties of the job
The post holder, as a registered nurse within the acute care at home team, is responsible for the delivery of high-quality evidence-based care for acutely ill patients within their homes, with the aim of preventing hospital admission or facilitating rapid discharge.
The post holder will liaise with the referring hospital or GP when arranging admission to the service and will take responsibility to effectively assess, develop, implement and evaluate patient care.
The Acute Care at Home Team work on a flexible rota shift system. The service covers a period of 0800 hours to 2030 hours 7 days a week. The geographical area when on duty is anywhere within North and East Cornwall areas. The post holder is predominantly a lone worker within this area.
As part of the role you will regularly deputise in the absence of Team Leader and assist in the management of the team, including the performance and development of staff and providing support to other team members.

Job summary
The post will involve working both day and night duties. This could predominantly be night duty depending on service cover. We are a dynamic and progressive community hospital setting and as a Band 5 Registered Nurse you work to support the Ward Sisters in the assessment, planning, implementation and evaluation of patient care. Langdale Ward is a 21-bed male acute admission ward based at Dorothy Pattison Hospital in Walsall. We support individuals experiencing an acute deterioration in their mental health, providing short-term, focused interventions aimed at helping them return home as soon as possible. As part of the role, you will also be required to work additional Place of Safety (Section 136) shifts within your 37.5-hour working week.
Main duties of the job

Assess patients' wellbeing and related needs, identifying and escalating physical or psychological health concerns to the appropriate team members.

An exciting secondment opportunity has arisen for a highly motivated, experienced ENT band 6 nurse to join our dynamic MDT at the Royal Free trust but will predominantly work at Barnet and Chase Farm Hospitals with occasional Royal Free hospital cover.
You will work independently and collaboratively, managing a clinical caseload, lead nurse-led clinics, and support patients throughout their journey
Key Responsibilities
Clinical Leadership: Support the management ENT patients, conducting clinical assessments, planning care, and initiating treatment plans for in and out patients .Nurse-Led Clinics: Run autonomous clinics (e.g,minor procedures such as micro suction, tracheostomy tube change clinics, wound care clinics, & allergy testing).MDT Collaboration: Work closely with ENT Consultants, Speech & Language Therapists and other MDT to coordinate patient journeys.Patient Education: Act as a key resource for patients and families, providing expert advice, support, and detailed information regarding conditions and treatment plans.Service Development: Support clinical audits, drive evidence-based practice, and contribute to the development of departmental guidelines and policies.
You will be supported by the band 7 CNS and ENT matron whilst working within a small supportive team of CNS's who take pride in providing compassionate care and maintaining excellent communication between them.

Job summary
The Therapies Complex Discharge Lead will provide advanced specialist Occupational Therapy or Physiotherapy expertise across inpatient services, supporting patients with complex therapy needs that are impacting discharge and patient flow. This is the second year of receiving winter funding for the complex discharge therapy team, this year we will be approaching this as a UHN Therapies model, adding resilience and expertise across both NGH and KGH. Whilst NGH will be your primary site there may be occasions where you are required to work at KGH.
The post holder will lead a roaming therapy function, providing expert assessment, clinical decision making and advice to the daily Discharge Hub, escalation areas and medically fit wards. They will act as the senior therapy representative within discharge planning processes, working collaboratively with multidisciplinary teams to facilitate timely, safe and effective discharge decisions whilst ensuring the patient remains at the centre of all decision making.
Main duties of the job
The post holder will undertake highly specialised assessments and interventions for patients with complex clinical, functional and social needs. Working autonomously within professional standards and scope of practice, they will demonstrate advanced clinical reasoning, positive risk management and expert decision making to support discharge planning and optimise patient outcomes.
As a senior therapist, the post holder will provide clinical leadership, advice and support to therapy colleagues and the wider multidisciplinary team, helping to resolve barriers to discharge, promote evidence-based practice and maintain high standards of care. They will contribute to service development, quality improvement, education and workforce development activities across Therapies.
The post holder will be responsible for team leadership of the Therapies complex discharge team including day to day operational management, as well as responsibility for data collection and reporting on the impact of the service to Therapies management and the wider trust when requested.
The role includes participation in seven-day services and, where appropriate, professional on-call rotas. Nursing Team
We are very lucky to have a fantastic nursing team.
We have 2 Advanced Nurse Practitioners with a wealth of experience, Linda (our clinical nurse lead, who works 4 days per week) and Karen, who works 3 days per week. They predominantly have same-day, face-to-face appointments. Additionally, we have Elaine, our Asthma and COPD nurse.
Our Treatment Room nurses include Marie, working 4 days per week, maintaining a very high standard of care for our 766 patients with diabetics. We are very proud that she was awarded Doctor Magazine Practice Nurse of the year award in 2012 and has continued to be amazing ever since! Bea works 5 days providing our main treatment room services, performs learning disability checks and home visits for long term condition assessments and frailty checks.
Alice works as our Healthcare Assistant and is passionate about promoting health and wellbeing. She completes multiple long-term condition reviews, including CHD, AF, stroke plus MMSEs, ECGs and dressings.
We have additional support for our COPD patients with remote clinics run by 2 specialist respiratory nurses.
Administrative Team
Our Business Manager, Chamila Perera joined us in January 2023, bringing a wealth of experience with 20 years managing in General Practice. Our Operations Manager, Andrew Poole joined in August 2022 and has modernised the way we work, bringing new innovative ideas. We have a fantastic team comprising 3 medical secretaries and 12 part-time administrative staff (including team-leaders, care navigators, coders and a care-coordinator). The usual front line pressure withstanding, the team is cohesive and effective. We embrace a very open and supportive atmosphere. We host social occasions through the year, with an annual Christmas party funded by the Partners.
Out of Hours & Home Visits
Our patients have access to Out of Hours through 12 Point Care, at the nearby Sollershot GP surgery. There is also an Urgent Care Centre at Lister Hospital, Stevenage, which includes Mental Health Urgent Care.
The Hertfordshire Acute In Hours Visiting service see our patients for any acute issues, which hugely reduced the number of home visits performed by our GPs, usually to 0 or 1 per week. This is generally to provide care to Palliative patients or routine matters for housebound patients on our own lists.
