Job summary

The role will combine onsite clinical sessions with planned and urgent community visits to housebound patients and residents in care homes. The ANP will manage acute presentations and long-term condition reviews and coordinate care pathways across multidisciplinary teams.
Main duties of the job
Independently assess, diagnose, plan, implement and evaluate care for patients presenting with both undifferentiated and chronic conditions.
Work within the total triage model reviewing online and telephone consultations, escalating appropriately and managing within scope.
Prescribe and review medications within scope of practice, aligned with local and national guidelines.
Undertake planned and urgent community visits to housebound patients and residents in care homes, providing acute and chronic condition management.
Manage patients with complex needs using evidence-based practice.
Interpret and act upon diagnostic results, correspondence, discharge summaries, and clinical alerts.
Initiate and monitor treatment plans, ensuring continuity of care.
Support urgent/emergency presentations, initiating appropriate response pathways.
Maintain accurate, contemporaneous patient records and documentation in accordance with practice protocols.
Maintain effective communication with triage GPs, practice staff, community teams, and external stakeholders.

Essential Criteria
Registered Nurse with NMC registration.
MSc/Postgraduate qualification in Advanced Clinical Practice or equivalent.
Independent Non-Medical Prescriber (V300).
Substantial experience in primary care or urgent care setting.
Proven ability to work autonomously in clinical decision-making.
Full UK driving licence and access to a vehicle for community visits.
Desirable Criteria
Experience using SystmOne or other primary care systems.
Experience in care home settings or community settings.

We are seeking a Band 6 Wound Clinic Team Leader to join our Tissue Viability and Wound Therapy Service, providing high-quality, evidence-based wound care to patients attending Medway Community Healthcare wound clinics.
The successful candidate will work as part of a specialised multidisciplinary team, assessing patient needs, developing and evaluating personalised care plans, and delivering appropriate wound treatment, prescribing, advice and support to promote healing and improve outcomes.
You will support the effective day-to-day management of wound clinics, assist the Senior Wound Therapist, and lead workload allocation during short periods of absence. The role includes providing leadership, supervision and development support to Healthcare Assistants, students and visitors, ensuring safe, compassionate and high-quality care aligned with current evidence and best practice.

This is a varied and rewarding role where no two days are the same. You will assist with personal care, mobility, nutrition and hydration, while encouraging patients to achieve their rehabilitation goals. As part of the MDT, you will undertake a range of clinical duties including observations, blood glucose monitoring, catheter and stoma care, venepuncture, ECG recording, bladder scanning, simple wound management, specimen collection and maintaining accurate patient records.
You will also contribute to risk assessments, infection prevention activities, and support patients' physical, emotional and social wellbeing. Working closely with nurses, therapists and other healthcare professionals, you will help deliver safe, effective care and promote the best possible outcomes for patients.

- Assess patients presenting with undifferentiated and undiagnosed conditions across all age groups, using comprehensive history-taking, clinical examination, and clinical reasoning to formulate differential diagnoses

Thorpe-Le-Soken Surgery is seeking an experienced advanced nurse practitioner/emergency care practitioner/paramedic for 37.5 hours a week to join our friendly, supportive and caring team. The role is working between Thorpe Surgery and our branch surgery in Kirby Cross.

We are excited to share an opportunity for a Band 7 Clinical Team Lead.
The role involves working in our hub located in Frimley and providing a crisis and home treatment service for all emergency, urgent and routine referrals in the Surrey Heath and Northeast Hants area. We are looking to recruit a Band 7 Clinical Team Lead to be part of and lead the service, providing senior clinical and managerial leadership. You will be accountable for the day-to-day management of the service, overseeing referrals, assessments, allocations and discharges to ensure a high standard of patient care.
Working closely with the Service Manager, you will drive service governance through audit, quality-improvement initiatives and performance-data audit, and ensure compliance with CQC standards.
You will support the management of operational functions such as incident investigations, complaints and escalations. You will provide supervision, appraisals and professional development for the team, and oversee recruitment, induction and training.
You will collaborate with inpatient wards, community mental health teams, primary care, social services and voluntary partners to ensure seamless care pathways and drive service improvement.
This is an integrated role to support the triage and/or rapid response as well as the Home Treatment elements of the service and the post holder will be responsible for creating an environment which is safe, supportive and an easily accessible with a no wrong door approach.

We are looking for an trainee psychotherapist with experience working with children under 5 years of age and their families to work in the CAMHS Inner London Under 5s Team.
The post holder will be embedded within our community services' multi-disciplinary teams responsible for the support of children and their families through initial assessment, consultation and intervention. Main duties of the job
The post involves seeing children and their families in family hubs and in the CAMHS clinic, as well as supporting the clinical work completed by the team.
You should be interested in offering focussed psychodynamic work to cases where there is a degree of complexity, as well as providing consultations to the networks supporting children and their families. You will receive supervision from experienced psychotherapists to support ou in your role.
Some of the responsibilities of the role include:
1. Kensington and Chelsea CAMHS is well established multi-disciplinary team. Job summary
An exciting opportunity has arisen for an experienced clinician looking to work at the front door of urgent mental health services in CNWL.
The SPA/OOHCR Senior Practitioner will play a crucial role in delivering urgent mental health support to people in mental health crisis.
The SPA is CNWL's 24/7 crisis line and urgent referral service where clinicians work primarily over the telephone to conduct clinical triages and support callers to access crisis support in a timely manner, and to remain in the community.
The service also provides face to face crisis assessments between 8pm and 8am across Brent, Harrow, Hillingdon, K&C and Westminster. Within these hours clinicians will travel to peoples' homes and conduct assessments in order to provide an alternative to attending A&E.
We believe that the best care is delivered by enthusiastic and skilled healthcare professionals who are passionate about crisis support and understanding of the social stressors that can contribute to people feeling distressed. The service values those with a wealth of experience of mental health conditions and their treatment.
You will also be prepared to work nights and be involved in the clinical supervision of the staff undertaking the OOHs crisis assessment role.
Main duties of the job
The key elements of this post are:
-Overseeing and conducting rapid engagement with service users over the telephone and in-personby conducting face to face assessment in their homes.
-Delivering care and training/supervising others to ensure a high level of triage, assessment andsignposting of service users to statutory and non-statutory support services.
-Working closely with our Bed management services, home treatment teams and psychiatric liaison teams and the Mental Health Crisis assessment services as well as having an excellent understanding of CNWL referral pathways in secondary care
-Using excellent clinical assessment and risk formulation skills and being able to confidently communicate to the team and to stakeholders both verbally and in writing.

We are seeking motivated Band 6 practitioners to join our Assertive Outreach Team (AOT), a new, high-impact service designed to support individuals with severe mental illness who are at risk of disengaging from services and require intensive, proactive, community-based care.
This is a dynamic, community-facing role, focused on engaging patients in their homes and local environments to stabilise their mental health and reduce crisis presentations.
The Assertive Outreach Team (AOT) role is fundamentally a patient-facing position, focused on engaging and managing individuals who are often reluctant to engage with services. Given the nature of this work, requiring proactive outreach, direct contact, and consistent face-to-face intervention, the role is not suitable for remote or hybrid working arrangements.
Main duties of the job

You will join a supportive multidisciplinary team delivering high-quality care across a 10-bedded PICU.
Main duties of the job

These activities promote social inclusion, recovery, and a sense of purpose, and are offered both on the wards and within the local community.

Main duties of the job
The Activity Co-ordinator role is a dynamic and rewarding patient-facing role, central to delivering holistic care and supporting the smooth operation of an inpatient acute mental health ward.
You will need to be creative and possess determination to continually improve / adapt and mould the offers of therapeutic, recovery and skill-based groups in order to promote recovery and independence amongst our service users.
We are looking for someone who has:
- Good communication skills: verbal and written, English to GCSE or equivalent

Job summary
Following recent investment to support the transformation of community mental health nursing, we are pleased to announce that we are looking to recruit additional Band 6 Community Psychiatric Nurses within the Community Recovery Services across the Sandwell locality.
This is an exciting opportunity to be a part of a supportive and diverse, multi-agency team who are currently receiving significant investment aiming to transform community mental health services across the Black Country. We will develop a new community mental health way of working and modernise community mental health services, taking into account the particular needs of our four places (Dudley, Sandwell, Walsall and Wolverhampton). Our new model will focus on supporting people living in their communities with mental health challenges.
Your role will include working as a Care Coordinator where you will have responsibility for managing a caseload of service users with mental health needs who have been referred to the team via a number of access points. As a further extension of this role you will also be involved in Crisis management as part of the duty system and you will undertake the role of the duty nurse on a rostered basis. You will work from our community base facility, via phone/agile working or within the service user's home.

As a Registered Mental Health Nurse, preferably with adult and community mental health experience, you will be responsible for assessing and formulating individual interventions, plans and outcome measures that promote recovery, in line with the recovery Model and relevant legislation for all people on your case load.
You will need to have an up to date knowledge of NHS policies and practices in mental health. The successful applicants will have previous experience of a range of therapeutic interventions and psychotropic medications. You will have proven ability to work with partner agencies, lead projects and motivate people to deliver them. You will have excellent communication skills with a flexible approach.The team is committed to provide safe, sound and supportive services. You will receive regular supervision, with opportunities to develop clinical and professional practice in line with service users' needs and organisational values.

Job summary
We are looking for an experienced, enthusiastic individual to join our Psychology Administration Team as a Medical Secretary / Assistant Team Leader.
This is a newly created role which provides great opportunity for collaboration and autonomy, as part of an established administration team. You will be responsible for the daily administrative tasks that are necessary in supporting a large Psychology service.

Main duties of the job
- You will need to demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.
- You will need to be a self-motivated individual with proven secretarial skills, excellent audio typing ability, diary management, and an excellent telephone manner as is the ability to communicate effectively at all levels.

- You will be responsible for supervision, staff rotas, raising of invoices, rostering data input for annual leave and sickness.

Job summary
The post holder will work within the Facial Lipoatrophy Service to provide assessment and non-surgical treatment for moderate to severe Facial Lipoatrophy in patients living with HIV. This service aims to restore facial appearance and improve patient experience and satisfaction.
Treatment of Facial Lipoatrophy is delivered by deep dermal injections of Poly-L-Lactic Acid PLLA (New-Fill) directly into the face. This helps to correct HIV related lipoatrophy and aims to restore facial appearance. Training in the administration of the New-Fill treatment is required to ensure efficacy and safety. The Facial Lipoatrophy Service is centralised and managed by the Chelsea and Westminster HIV services. All patients attending London HIV Clinics with moderate to severe Facial Lipoatrophy are eligible to be referred for New-Fill treatments.

Clinical Leadership and expertise.
Clinical responsibilities and independently assessing, treating and evaluating patients with Facial Lipoatrophy.

As a Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
