Main duties of the job
As a Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.

Job summary
The post holder will work as a member of the multi-disciplinary team throughout the Homerton supporting temporary staffing requests. They will use their professional skill set and calming influence to identify and support patients presenting with mental health needs, supporting the implementation and engagement of care plans.
The RMN will be part of the multi-disciplinary team providing care to patients who present to the Homerton University Hospital NHS Foundation trust.

Main duties of the job
- Be able to support patients presenting with mental health needs, on a shift-to-shift basis, supporting the implementation and engagement of care plans ensuring the delivery of high quality and safe care.
- Be able to use their specialist knowledge and skill set, to identify and detail deterioration / improvements and fluctuations in patient's mental health and escalate to the relevant teams where there are significant risks.
- To engage directly with patients in a holistic manner and determine any clinical, environmental, and social needs.

Job summary
An exiting opportunity has become available for a Band 6 Nurse or Midwife to work in the Pregnancy Advisory Service (PAS) at the Central Health Clinic and WISH clinic.
PAS is a Nurse delivered, Consultant covered, service based at the Central Health Clinic and WISH. The role involves patient consultations, ultrasound scanning, informal counselling, sexual health screening, giving contraception advice and consenting for abortions.
May include some evenings & travel to Weston General Hospital.

Main duties of the job
- Provide a full range of nursing skills within the Pregnancy Advisory Service. This may include evenings.
- Provide a confidential and comprehensive sexual and reproductive health services including use of Fraser Guidelines.
- Administer and / or issue prescribed medication as appropriate following NMC professional guidelines, working from PGDs or Non-Medical Prescribing as appropriate.
- Use professional judgement to seek advice or refer to medical colleagues.
- Be aware of and adhere to NMC, Trust and Directorate policies and procedures.
- Adhere to departmental protocols, procedures and guidelines for clinical care in order to ensure that a high standard of care is delivered to all service users.

As a Senior Substance Misuse Nurse, your responsibilities will vary. You will
- Work as part of a multi-disciplinary team, prioritising patient care to develop individualised packages of care through comprehensive assessment and recovery planning
- Deliver a range of treatment options including clinical and psychosocial interventions in both a group and 1-1 setting with recovery at the centre of our healthcare wellbeing model
- Be able to carry out comprehensive assessments, initiate and review individual recovery plans and confidently communicate the full range of treatment options available to service users.
- Monitor therapeutic responses to medication and potential side effects and liaise with prescribers. You will work with individuals, on a 1-1 basis, providing advice and information

Our team provides a service 8-8pm 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. In addition, the role involves deputising for the Team Managers in their absence
You should be able to demonstrate your passion towards working with this client group and have the ability to assess, co-ordinate treatment and support both clients and carers. Main duties of the job
You will be based in either the dementia or functional sub team however you will be expected to work across both sub-teams at times. The Older Adults Home Treatment Team provides a service 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia.

Main duties of the job
As a registered children's nurse, with post preceptorship experience in the UK, you will support the Fracture clinic team in all areas of the adult clinic - including the trauma clinics, elective clinics, plaster rooms and virtual fracture clinic. Providing a high standard of care to children, young persons and adults.
Support the plaster technicians in the adult & children's plaster rooms and in delivering the plaster room outreach service across the inpatient areas. Develop your skills in various plastering techniques and attend the British Orthopaedic Casting Course to achieve the British Casting Certificate qualification.
Act as point of contact for any concerns relating to paediatric patients in the adult environment e.g. medication, safeguarding, wound management and treatments.
Work with the team using UHCWi methodology to continuously improve the services provided.
Take responsibility for the co-ordination of shifts - communicating with patients, carers, staff and the wider multidisciplinary team on the day to ensure the smooth running of clinics and patient pathways.

We are seeking to appoint a dynamic and enthusiastic candidate to the role of Staff Nurse - Specialist School Nursing within a full time role, located at Cedar House, within our St. Michael's Community Hospital site within the Specialist School Nursing team. Main duties of the job
We are seeking an enthusiastic individual, with an interest in special school nursing. The post holder will work collaboratively with the multidisciplinary team. The post holder will work as part of the SSN Nursing Team with support from the SSN manager and band 6 Nursing Team. Job summary
The Clinical Nurse Specialist will respond to urgent referrals, carrying out rapid specialist palliative care assessments for patients in their own home, developing immediate care/ support plans in relation to patients urgent and unaddressed specialist palliative care needs. The Rapid Response Service responds to unplanned community palliative care referrals across the Staffordshire St Giles Hospice catchment area. The service currently operates seven days a week between 08:00 and 20:00, with phase two extending provision to 24 hours a day, seven days a week.The second phase of the Rapid Response Service will entail on a virtual ward comprising 20 virtual beds for patients across our St Giles Staffordshire catchment and in support of our neighbouring hospices.
Main duties of the job
The role will provide clinical assessment, supporting the management of complex care pathways, contributing to audit and service development, and providing staff supervision in line with organisational and professional standards. Referrals may be received from 111/999, GPs, allied health professionals, community, hospice, acute and virtual ward teams, as well as directly from patients, families and carers.
The post holder will work flexibly in the delivery of specialist palliative care and support to patients, families, and carers in the place they consider to be home across the St Giles catchment area, with sensitivity to their changing physical, psychological, social, and spiritual needs, including action within two hours of urgent referral acceptance where required.
To provide rapid access to specialist assessment, advice, and care, working collaboratively with existing care providers and initiating referrals to additional services where these will enhance patient and family support. To communicate, coordinate, and liaise with the patients usual care team, including GPs, District Nurses, and other palliative care services, ensuring timely updates on patient condition, identified needs, and any significant changes, while maintaining accurate and contemporaneous clinical records.
Working autonomously and in collaboration with internal colleagues, along with other community, primary care, acute and virtual ward colleagues, the post holder will support.
