- Receive and make telephone calls as required, answering the call promptly in a polite and helpful manner, using agreed practice scripts as appropriate. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.
- Ensure patient-related tasks are completed in a timely and efficient manner, e.g. pathlinks admin
- Follow procedures for the recording and handling of home visits (house calls), ensuring careful recording of all relevant details.
- Action repeat prescription requests and ensure that they are ready for collection by the patient within 72 hours, either on the computer or manually.
- When directed, prepare prescriptions for dosette boxes, liaise with pharmacies as appropriate

Job summary
Burnham, Berrow and Highbridge Medical Centres (known as Symphony North) are looking to recruit a Treatment Room Nurse or Practice Nurse with chronic disease qualifications,to join their dedicated Nursing Team on a 12-month fixed term basis, working 24 hours per week across all three sites.
The pay rate for this role ranges from £19.28 to £22.73 per hour,. depending on experience and qualifications.
You will be part of a nursing team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.
You will focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives and vision of Symphony Healthcare Services. They will work collaboratively with the whole general practice team to meet the needs of patients and supporting the delivery of policy and procedures.
You will also demonstrate safe, clinical decision-making, assessment and expert care.
Main duties of the job
- Assist the whole practice team to help deliver effective implementation of management of patients and their needs.
- Managing the nursing care of patients with chronic disease, employing recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract purposes is also required
- Contribute to the development of policies / protocols / guidelines within the practice.
- Aid the practice team with implementation of new initiatives in the delivery of chronic disease management services.
- Encourage the development of integrated services within Primary Care to include integrated practice based teams and multidisciplinary working.
- Recognise the opportunities for working in partnership with the Primary Health care team.
- Implement and evaluate individual treatment plans for patients with a known long-term condition
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Job summary
The Advanced Nurse Practitioner (ANP) for Frailty and Proactive Care will provide expert clinical leadership and advanced practice to provide more joined-up care for high-priority cohorts through integrated neighbourhood teams (INTs), with a focus on delivering proactive support for people with frailty and long-term conditions.
Main duties of the job

To support the delivery and development of high-quality memory assessment and dementia care services within the Central (Conwy/Denbighshire) Memory Assessment Service. To act as an autonomous practitioner, managing a caseload of individuals undergoing assessment, diagnosis and ongoing support for cognitive impairment and dementia. To work collaboratively with multidisciplinary teams, service users, carers and partner agencies to ensure person-centred care is delivered in accordance with Health Board policies, professional standards and the NMC Code. To contribute to service improvement and the development of colleagues through clinical supervision, mentorship, education and the application of evidence-based practice, research and emerging developments in dementia care.
Main duties of the job
The successful candidate will be a part of the Memory Assessment Service where they will work with those who are experiencing cognitive changes and have concerns regarding Dementia. The successful candidate will undertake responsibility of the Memory Assessment Service in the absence of the manager. They will be required to divide their time equally between a clinic and community care setting due to the large rural geographical area with limited transport links and problematic mobile phone reception. It is necessary to work autonomously and conduct patient home visits. To have the ability to make complex decisions unsupervised due to working in isolated areas. The successful candidate is expected to participate as a full member of the multi-disciplinary team, working with people who present with possible organic mental health problems. Taking responsibility for a case load of patients and initial comprehensive assessments, delivering diagnosis and formulating treatment plans and initial review.

Job summary
The 0-18 Children's Bladder and Bowel Service is a small team of specialist children's nurses, who provide information, assessment, support and treatment recommendations to families, children and young people with a bladder and/or bowel difficulty. We help support parents, children, and young people to make decisions that affect their future health and wellbeing and support them to achieve their continence potential.
The Children's Bladder and Bowel service is an innovative and progressive service, and this role requires a flexible, warm and reflective practitioner committed to ongoing development of their skills and knowledge to maintain the excellent standards of the service.
You will require specialist knowledge, attitude and skills to deliver a service using evidence based practice to ensure quality care, advice and support to locality team members and the wider community. You will use advanced assessment skills to work in partnership with children, young people and families to understand their bladder and bowel needs and to agree appropriate support required to contribute to their long-term outcomes.
You'll work closely with the primary health care team, education services, social care and the independent sector. Above all your compassion will mean that together we can make a difference.
Main duties of the job
- Complete comprehensive bladder and bowel health needs assessments to plan, implement and evaluate programmes of care, including analysis, interpretation and comparison of options available to meet identified need, in partnership with service users, parents, children and young people.
- Develop and deliver programmes of care for complex needs and provide highly specialised bladder and bowels advice.
- Assess for provision of appropriate continence containment products including ordering and annual reviews.
- Offer evidence based advice, and refer to Safeguarding Children and other agencies where appropriate, in line with Safeguarding Children Policies and Procedures including court reports/legal documents etc.
- Actively participate in an advisory capacity on all aspects relevant to the promotion of continence in children and young adults.
- Establish and maintain a body of information so enquiries can be dealt with efficiently.
- Provide clinical expertise and advice on children's bladder and bowels.
- Support children and adolescents, their families and other carers to influence and partake in decisions concerning their care.
- Keep abreast of advances and current developments within the field of continence within Clinical Governance parameters.
- Liaise with other Paediatric Continence Advisors within the Association of Continence Advisors Regional and Nationally and other professional organisations concerned with the promotion of continence in children and adolescents

Band 4 - Blood Sciences Medical Laboratory Assistant (MLA) Supervisor Post
Permanent contract - PART TIME - either 22.5h (Mon - Wed) or 15h (Thu - Fri) per week.
2x part time posts available (as mentioned above). Main hours will be 1200 - 2030. Successful candidates will be expected to cover each others shifts where required by the department . Also flexibility to adapt to unexpected workload and staffing pressures will be required. E.g. recent implementation of new LIMS has resulted in temporary later working.
Must be available to cover weekend and night shifts when all other options have been exhausted.

The Pathology department at Stepping Hill Hospital (Stockport NHS Foundation Trust) is seeking a Medical Laboratory assistant to join the Blood Sciences team to assist with processing samples that arrive in the lab in the evenings. The successful applicant(s) would be joining a friendly and passionate staffing group providing diagnostic services to the hospital patients and wider community.
The role supports the delivery of Blood Sciences services. The ideal candidate will be expected to supervise the MLA team, carry out and assign day to day work in the Pathology Specimen Reception areas. Ensuring best practice and adherence to Trust and departmental policies and procedures. The post holder will be responsible for the training and competency assessment of the MLA team.
Main duties of the job
Supervise, carry out and assign MLA team activities and duties in all specimen reception areas to ensure workflow turnaround time (TAT) targets are achieved.
Manage the 24/7 MLA rota to ensure it is adequately staffed and shifts are covered. Including approving and declining annual leave and covering sickness absences to ensure service provision.
Conduct daily MLA huddles to cascade information and acquire team feedback for escalation to Technical Heads of Department and/or Senior BMS.
Deliver and coordinate training and competency assessment of the MLA team.
Ensure relevant departmental and Trust processes are followed in accordance with SOPs and policies such as Specimen Acceptance Policy and departmental H&S policies.
First line sickness management duties for MLA team i.e. logging sickness absences in accordance with Trust policy and escalating this to Senior BMS and THOD.
Support with Trust appraisal process for MLA Team along with Senior BMS and THODs.
Support with recruitment and selection processes for new MLA team members.
Ensuring equipment e.g. POCT devices and centrifuges are maintained appropriately.
Monitoring and ordering of relevant stock and inventory where appropriate.

This is an exciting opportunity to work within the UGI Clinical Nurse Specialist team. Under direction from the Lead UGI Cancer Nurse Specialist you will assess, plan, implement and evaluate expert nursing care and provide support to UGI and GIST cancer patients and their carers. The role calls for a professional who has vast experience of triaging mental health presentation ideally with children, young people and their families who are experiencing mental health and trauma difficulties.
Part of the job plan entails pre planned and on the day assessments, follow up assessments, collaboration with GP's within the Kirklees locality, multidisciplinary team working, and referral to suitable services within the area. The role also includes participating in the duty role for Kirklees CAMHS and providing clinical oversight to partner agencies.

Job summary
NHS England has established 17 Maternal Medicine Networks (MMN) around England. These networks are relatively new so this is an exciting post as the networks are evolving to meet the needs of the population in the East Midlands. The East Midlands is a regional network and works with Maternity Units in Leicestershire, Nottingham, Derbyshire, Lincolnshire and Northamptonshire. UHL is the lead center working closely with our co-leads Nottingham and Derby hospitals to optimize the care of women with pregnancy complicated by maternal conditions. Each hospital within the MMN network is required to monitor and submit their activity numbers
The key responsibility of this role is to provide administrative facilitation of team meetings for the regional MDT meetings as part of the network arrangement, ensuring external colleagues are able to join the meetings via electronic conferencing. This will include preparing, organising and circulating agendas, minutes, results and sending out minutes of the meeting to relevant clinicians.
The role will also be responsible for collecting, maintaining and submitting the data required for the network MDT and national submission and monitoring the dataset.
It is required that the post holder will provide administration support to the Network core team and will be a pivotal point of contact and support to the clinical leads across all network Trusts.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic and highly motivated individual to be the network MDT coordinator and data Navigator for the East Midlands Maternal medicine Network . As a member of the team you will be responsible for facilitation and coordination of the MDTs primarily providing support to the network core team members .
The role is varied and will develop to meet the needs of the service. The successful candidate will be required to possess excellent communication, relationship and administration skills. It will involve preparing the MDT, collecting, maintaining and the national submission and monitoring of the dataset.
The post holder must engage effectively with network hospitals and Trusts to ensure the data collection process is efficient, smooth and well-supported. This role also requires data collection, compilation of relevant reports and audits and the post holder must be able to work to tight deadlines and at times under pressure.

An exciting opportunity has arisen for a Band 7 Registered Nurse in Learning Disability (RNLD) to join the established CALDS (Child and Adolescent Learning Disability Service) which is a community based multi-disciplinary team within the Children, Young People (CYP) and Family Health Service. We are seeking a dynamic, compassionate, and experienced clinician to join our service in a Team Lead role for our tertiary response pathway (called Cyfuno). This is a key leadership role, providing clinical, operational, and strategic oversight to a specialist multidisciplinary team. You will play a pivotal role in ensuring that vulnerable children and young people receive timely, coordinated, and effective support during periods of crisis.
Main duties of the job
We are looking for a forward thinking RNLD who has in-depth knowledge of factors effecting the health and wellbeing of children with LD, reasonable adjustments that may be required, behaviours that are challenging and complex needs, along with an interest in supporting the development and improvement of the service. The post holder will have the ability to support CYP and their families; delivering good quality, individualised and evidenced based care. The post holder will have the skills to work in a team and as an individual, be confident to work within their competencies and be able to adapt to the changing needs of the service. Roles will include leading on risk assessment, decision-making and crisis planning, liaising with Safeguarding and Risk & Governance Leads, chairing and contributing to multi-agency meetings for complex and high-risk children, driving quality improvement, audit, and governance processes, working with professional leads to manage workload, co-developing service delivery and outcome measures, and fostering strong inter-agency relationships to support integrated care. You will be expected to complete, audits, rosters, reports, attend meetings and liaise with all members of the MDT as well as many other tasks.

The post holder is required to provide an efficient and effective support service to the residents in staff residential accommodation on the Ysbyty Gwynedd Hospital site, including accommodation booking, security of property, key holding, and record keeping.

Main duties of the job

Main duties of the job

- To lead the delivery of public health and universal and targeted interventions across the 0-19 years age range for a defined population and will be delivered in line with the National and Local guidelines and public health priorities.

Macmillan Renton Unit is an outpatient unit, providing services such as Haematology, Oncology. Gynaecology, Breast Clinics, Urology and Systemic Anti Cancer Treatment (SACT) to patients within Herefordshire and surrounding areas.
We are a satellite site for Cheltenham hospital, which means we are able to treat our patients closer to their homes.
The hours worked are flexible by negotiation, agreed to suit the service and the applicant to ensure a good work life balance.
We will invest in the successful candidate to complete their SACT administration competencies, this will be through further education either online or through Cheltenham (Gloucester University). We will support and honour set study days to complete this course.
