Are you a GP passionate about high-quality, person-centred care? Do you enjoy multidisciplinary working and want to help shape innovative community services? We are seeking Bank GPs to support our Intermediate Care and Hospital at Home services.
This is an exciting opportunity to join a developing GP-led model that supports people to receive treatment closer to home. Flexible bank opportunities are available across bed-based, community and virtual pathways.
The Stamford Unit is a 96-bed Intermediate Care facility providing rehabilitation, end-of-life care, step-up admissions and complex discharge support. You will provide medical care for patients, undertake ward rounds and work alongside ACPs, Nurses, Therapists and Social Workers to deliver coordinated care.
Our Hospital at Home service provides hospital-level care in patients' homes through remote monitoring, telemedicine and multidisciplinary support. Working with Urgent Community Response teams, advanced practitioners and specialist services, you will undertake clinical assessments, support admission avoidance and early discharge, and contribute to personalised care planning.
This varied role combines community-based acute care and digital healthcare, offering opportunities to develop interests in frailty, older people's medicine, palliative care and telemedicine while helping deliver innovative models of care across Tameside and Glossop.
Main duties of the job
- Provide clinical leadership across Intermediate Care, Hospital at Home and Virtual Ward services, ensuring safe, effective and high-quality care.
- Undertake assessment, diagnosis, treatment planning and ongoing management across bed-based, community and virtual pathways.
- Work with multidisciplinary teams to deliver coordinated, person-centred care and support treatment, escalation and discharge planning.
- Deliver clinical care through ward rounds, home visits and telemedicine consultations.
- Support admission avoidance, early discharge and care closer to home through Hospital at Home and Virtual Ward pathways.
- Manage patients with frailty, rehabilitation, chronic disease, palliative and acute care needs in line with best practice.
- Work closely with primary care, community services, care homes and system partners to ensure seamless care.
- Contribute to service development, pathway improvement, innovation and the promotion of best practice.
- Participate in clinical governance activities including audit, incident reviews, complaints, patient safety investigations and quality improvement.
- Work flexibly across services to meet patient and operational needs.
- Support the development of colleagues and act as a role model for professionalism and clinical excellence.

Main duties of the job
Main Duties of job:o Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system.o Work within limitations and escalate to Practitioner as appropriate.o To support Practitioners, STR Workers and Lived Experience workers and Admin as necessary.o Provide an embedded service in police custody suites, courts and Outreach, working within legislation requirements, such as PACE (1984).o Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service.o Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support.o Liaise with other agencies as required dependant on the person's presentation, work closely with other agencies, such Custody and Health Care Professionals, and community agencies.o Actively promote the service to external agencies as a positive example of user led services.

Job summary
As a Practice Nurse within the E4 PCN Enhanced Access Service, you will play a key role in delivering safe, high-quality and responsive nursing care to our diverse patient population.
The E4 PCN is astrong and trusted provider of primary care services, working closely with our five member practices to deliver accessible, consistent and patient-centred care.
Enhanced Access provides evening and weekend appointments, improving access for patients who cannot attend routine daytime services. The successful candidate must be available to work flexibly across these hours, with the majority of nursing services delivered on Saturdays. The role requires a nurse who is confident working autonomously and without direct nursing supervision, using sound clinical judgement to assess, manage and treat patients within their scope of practice. This is an important role within our PCN and requires a clinically confident, accountable and adaptable nurse committed to delivering excellent patient care.
Main duties of the job
The role requires the nurse to work autonomously and be self-sufficient in clinical practice, without direct nursing supervision, while recognising and escalating any concerns outside their scope of practice.

As a Recovery Nurse Practitioner, you will provide high-quality, evidence-based nursing care to patients throughout the post-operative period. You will work closely with the multidisciplinary team to assess, monitor and manage patients following anaesthesia and surgery, ensuring their safe recovery and transfer to the appropriate clinical area.

We are seeking an experienced and highly motivated clinical leader looking for a new challenge to lead Mile End Hospital's - Early Diagnosis Centre (Endoscopy Unit).

Main duties of the job
1. To be a professional role model, leading and coordinating the ward/department team. Taking continuing responsibility and accountability for the 24-hour management and organization of theWard/Department. This involves managing a wide range of complex and sometimes conflicting priorities.
2. Provide specialist nursing advice Trust wide to support and facilitate high quality, individualised care to patients /clients and relatives/carers in collaboration with the multi professional team, in line with the local and national targets.
3. At all times ensure that the needs of the patient are placed at the centre of care delivery.
4. To be flexible in the approach to the clinical area and provide cover for any area specified.
5. To ensure that the unit is well led and that the teams are being proactive in the delivery of the:

Job summary
An opportunity has arisen for an experienced Band 6 nurse on Ward 7D, Paediatric Assessment and Short Stay Unit in the Royal London Children's Hospital, Barts Health NHS Trust.
The contract is for a permanent part-time role at 0.53 WTE = 6 shifts per month of 11.5 hours a shift.
PASSU is fast paced area with a varied patient profile, providing high-quality nursing care for Babies, Children and Young People under different specialities including, General Paediatrics, Paediatric Oncology, Orthopaedics, Acute mental health and General Paediatric Surgery. The ethos of the unit is working in partnership with the children and their families.
The post holder will have every opportunity to develop new skills and knowledge, working closely with all multi-disciplinary team members. Staff are also encouraged and given the opportunity to continue their professional development through attendance on relevant courses in Barts Education Academy, and externally, with City St George's and London Southbank Universities.
Barts Health has a busy and demanding Children's service, requiring staff to be flexible in their working. The post holder will be required to work within a challenging clinical environment and undertake shift work which will include nights and weekends.
