An exciting opportunity has arisen for a qualified registered Adult Nurse to join our Acute and Chronic Conditions research team, facilitating a range of studies across multiple specialties- all which contribute to improved outcomes and experience of patients health.
You will have a key working relationship with the Team Leader, other team members and Clinicians in developing a portfolio of studies. There will be opportunities to work on a variety of studies whilst helping to promote a research active culture within the acute and chronic conditions specialties at ELHT.
We are looking for someone who likes a challenge and can work autonomously when appropriate whilst contributing to the wider team. You should be a good communicator and be confident interfacing with patients & staff at all levels.
Main duties of the job

Main duties of the job
As a Band 5 Community Triage Nurse, you will have professional and clinical knowledge of community nursing to provide remote triage of calls and referrals made to the community nursing teams in East Staffs.

- Understand the scope of the community nursing service and the role of other MPFT and external services within each Primary Care Network. To ensure referrals are directed to the most appropriate service (including directing to emergency services where needed).
- Occasional delivery of care to patients in a community setting in order to maintain up to date clinical experience.
- Whilst not providing regular face to face patient contact the post holder will need to maintain an up-to-date knowledge of the clinical aspects of community nursing to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. This includes palliative care (particularly symptom management and support to families in the last few days of life); wound care; medication management; diabetes management; catheter and bowel care.
- Have supervisory responsibilities in the absence of the triage lead nurse; including coordinating the team; allocating work to team members; resolving and escalating issues; coordinating the daily huddle and group chat and representing the triage team on the community nursing capacity calls.

An exciting opportunity has arisen to join our Integrated Out of Hours Community Nursing Team as a Band 6 Senior Staff Nurse. This is a permanent role, with vacancies across Stoke-on-Trent, Stafford and Norton Canes. Successful candidates will be required to work across two of these bases.
The service operates between 16:00 and 08:15, seven days a week, covering evening and night shifts. This may particularly appeal to nurses seeking a flexible alternative to traditional working patterns.
We cover approximately 1,047 square miles, therefore a valid driving licence and access to a vehicle are essential. You must be willing and able to travel extensively across the locality to meet service and patient needs.

Not every patient who arrives at hospital needs Emergency Department care.
You'll help assess, prioritise and redirect patients where clinically appropriate, supporting people to access the right level of care while helping reduce unnecessary pressure elsewhere in the system.
Whether you're treating and discharging a patient, referring them to another service or escalating their care, the patient and their safety will be at the centre of your decision-making.
Maintaining excellent clinical standards
You'll work within IC24's clinical governance framework and prescribing formulary, maintaining accurate and timely clinical records and working in accordance with GMC guidance and relevant national and local clinical standards.
You'll maintain your CPD and essential learning and have opportunities to contribute to audit, learning events, education and quality improvement.

Not every patient who needs urgent care can come to us. Some are housebound, frail, approaching the end of life or have been clinically assessed as needing a face-to-face review in their own home.
What they need is an experienced doctor who can listen, assess, make confident clinical decisions and get them to the right care.
At IC24, we're looking for Urgent Primary Care Doctors to work within our Home Visiting Service across Kent.
You'll work alongside our drivers, dispatch teams and wider urgent care colleagues, providing safe, timely and effective care to patients in their own homes.

What you'll focus on
Assessing and treating patients in their own homes
You'll provide face-to-face assessment, diagnosis and treatment for patients who have been clinically triaged as requiring a home visit.
You'll see patients with a broad range of urgent primary care needs, considering not just the immediate clinical problem but their wider circumstances, environment and support needs.
You'll assess what's happening, decide what needs to happen next and ensure patients receive appropriate treatment, referral and safety-netting.
Making confident clinical decisions
Home visiting requires sound clinical judgement.
You'll often be working with patients you haven't met before, sometimes with limited background information and sometimes when they're anxious, unwell or experiencing difficult circumstances.
You'll need to be able to assess risk, manage uncertainty and make safe decisions independently, recognising when a patient can be treated and supported at home and when they need escalation or an alternative care setting.
Treating the whole patient
Seeing someone in their own home can give you a very different perspective on their health.
You'll care for patients across ages and backgrounds, including older and frail patients, people with complex health needs and those approaching the end of their lives.
You'll consider the patient's clinical presentation alongside their environment, support network and wider circumstances to determine what they need to remain safe.
Keeping patients safely in their community
Not every patient who needs urgent care needs to be in an Emergency Department.
Where clinically appropriate, you'll help patients remain safely at home by putting the right treatment, advice, referrals and safety-netting in place.
You'll work with community nursing, pharmacy, paramedic, primary care and wider healthcare teams to support continuity of care.
And when hospital or another service is the right place for a patient, you'll recognise that and escalate appropriately.
Working independently, but never in isolation
Home visiting is autonomous by nature.
You'll be making important clinical decisions in patients' homes without the immediate infrastructure of a traditional clinical setting around you.
But you'll remain part of the wider IC24 team.
You'll work closely with our drivers and dispatch colleagues and communicate with other health and care professionals to make sure patients receive timely, joined-up care.
Maintaining excellent clinical standards
You'll work within IC24's clinical governance framework and prescribing formulary, maintaining accurate and timely clinical records and working in accordance with GMC guidance and relevant national and local clinical standards.
You'll maintain your CPD and essential learning and have opportunities to contribute to audit, learning events, education and quality improvement.
This is urgent care beyond the consulting room
No two home visits will necessarily look the same.
You'll see patients with everything from relatively straightforward acute illness to complex physical, mental health and social circumstances.
Sometimes the answer will be clear.
Sometimes you'll need to piece together limited information, understand what's happening around the patient and make an important clinical decision without having every investigation immediately available.

Main duties of the job
The Department requires an enthusiastic, self motivated individual to assist in providing an efficient and cost effective materials management service to wards and departments at University Hospitals, Coventry.

The post holder will be a part of the busy Appointments Office working as a member of the team, helping to maintain and improve our waiting list targets and deal with the day to day queries from patients, General Practitioners, Consultants, wards and departments.
The post holder will be responsible for the operational management of outpatient waiting lists. They will be responsible for complying with Trust Waiting List Policy and National Waiting List targets by ensuring patients are given appointments on the appropriate clinic as soon as possible according to their clinical priority. They will also ensure clinics are filled in accordance with clinic rules and advanced booking processes.
The post holder will be required to rotate between all roles within the department and cover for other members of the team as appropriate for annual and sick leave.
Main duties of the job
Answer call's according to script and deal with any appointment changes/alterations in a professional and polite manner.
Make all new and some follow up appointments on the Patient Administration System from the General Practitioners' tertiary referral letters, A&E admissions etc.
Make sure that all comments made by Consultants on referrals are adhered to.
Understand current national waiting list targets and ensure these are adhered, identify and inform the relevant General Manager and Outpatients Department Service Manager immediately about potential breaches of the national targets.
Update PAS and ensure that patient demographics are correct using the national tracing service if necessary.
Manage the Trusts Electronic Booking Service, 'Choose & Book'.
Ensure that all referral letters are printed and presented to Consultants on a daily basis.
Operate the procedures of advanced booking systems such as partial and fully booking patients as required.
Ensure that all routine patients receive an acknowledgement letter within 14 days of the referral being received.
Check ASI queries / emails from 'Choose & Book' on a daily basis and determine whether it is a technical or capacity issue of slot unavailability.
Contact patient and arrange appointment as necessary.
Provide a quality service to internal and external customers at all times.
Answer call's according to script and deal with any appointment changes/alterations in a professional and polite manner.

Main duties of the job
The post holder will work with the Lead Midwife for Tobacco Dependence and other team members to provide individualised stop smoking support to women and families in a flexible and innovative way. This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking. It focuses on engaging with people in communities and offering them practical support to change their behaviour to achieve their own choices and goals.
The post holder will play a key role in the collecting, collating and reporting of Smokefree Pregnancy data. The post holder will be required to undertake other duties commensurate with the grade as directed.

Due to working from a monthly rolling rota, the ideal candidate will be open to flexible working within the core hours of the service, between 07:00-18:00, and be able to work from all three UHL sites. Joining us a B6 CPN you will be part of a large multi-disciplinary service 80+ staff supporting transition of patient care from secure settings into the community, which feeds into various sites including Reaside, Ardenleigh, Tamarind, St Andrews and Hillis Lodge (Low Secure) and Elliot House (Approved Premises). We offer extensive support to staff in their role as CC which includes supervision, reflective practice, art therapy, autism support and case-busting sessions. We also provide flexible working patterns which includes working from home when needed, pool cars are also available for community visits. FIRST is a standalone service providing our own home treatment and crisis community support to our service users; our CPN's and SW's are on a rota to jointly provide cover for on call shifts (approx. 4 times per month) with support from On Call RC during evenings, weekends and bank holidays so that we can respond to urgent situations by telephone and complete home visits in emergencies (standby on call payments and overtime are payable).
Main duties of the job
To undertake the duties of care coordinator in the assessment, planning, implementation and evaluation of the care and treatment needs for a defined group of services users accessing Forensic Intensive Recovery Support Team (FIRST). To provide professional advice and support to other stakeholders involved in service users care which may include families, carers and care agencies, accommodation and housing providers, probation and criminal justice colleagues. To undertake a multi-disciplinary approach to care and working collaboratively with members of the team which may include Responsible Clinician, Social Worker, Occupational Therapist, Psychologist, Support Time And Recovery Worker (STAR), Specialist Substance Misuse Workers, and Peer Mentors. To support colleagues when required with day-to-day tasks and act as a role model and resource to Band 5 CPN's in the team and student nurses. To advocate for the client group we work with whilst promoting a culture of recovery and optimism.

Main duties of the job
The senior PWP role involves supporting both qualified PWP's and those in their training as apprentice/trainee PWP's. You will offer both clinical skills and case management supervision to the PWP's as part of your role, as well as ad hoc support to your step 2 colleagues. You will continue to retain a small caseload of clients in order to keep your PWP's skills up to date, as well as maintaining your professional registration.
You will work together with the senior PWP's and management team to create a supportive environment for the step 2 team. You will have knowledge of the service targets and support PWP's to achieve the targets set, as well as providing knowledge to the wider team about the role of a PWP. You will be supported by your fellow senior PWPs, as well as a Step 2 lead and your team manager.
You will support with the organisation and facilitation of our community events calendar. As well as taking a lead in one of our champion roles, in which you will oversee and manage the projects happening within this area of expertise.
The role requires someone who its reliable, with a flexible approach; working closely with PWP's who have differing requirements to ensure they continue to offer evidence based low intensity CBT based interventions to the Birmingham population, while keeping in mind the low intensity, high volume nature of the PWP post.

Main duties of the job

The post holder will be a part of the busy Appointments Office working as a member of the team, helping to maintain and improve our waiting list targets and deal with the day to day queries from patients, General Practitioners, Consultants, wards and departments.
The post holder will be responsible for the operational management of outpatient waiting lists. They will be responsible for complying with Trust Waiting List Policy and National Waiting List targets by ensuring patients are given appointments on the appropriate clinic as soon as possible according to their clinical priority. They will also ensure clinics are filled in accordance with clinic rules and advanced booking processes.
The post holder will be required to rotate between all roles within the department and cover for other members of the team as appropriate for annual and sick leave.
Main duties of the job
Answer call's according to script and deal with any appointment changes/alterations in a professional and polite manner.
Make all new and some follow up appointments on the Patient Administration System from the General Practitioners' tertiary referral letters, A&E admissions etc.
Make sure that all comments made by Consultants on referrals are adhered to.
Understand current national waiting list targets and ensure these are adhered, identify and inform the relevant General Manager and Outpatients Department Service Manager immediately about potential breaches of the national targets.
Update PAS and ensure that patient demographics are correct using the national tracing service if necessary.
Manage the Trusts Electronic Booking Service, 'Choose & Book'.
Ensure that all referral letters are printed and presented to Consultants on a daily basis.
Operate the procedures of advanced booking systems such as partial and fully booking patients as required.
Ensure that all routine patients receive an acknowledgement letter within 14 days of the referral being received.
Check ASI queries / emails from 'Choose & Book' on a daily basis and determine whether it is a technical or capacity issue of slot unavailability.
Contact patient and arrange appointment as necessary.
Provide a quality service to internal and external customers at all times.
Answer call's according to script and deal with any appointment changes/alterations in a professional and polite manner.

Job summary
We are looking for a compassionate and experienced Advanced Nurse Practitioner to join our Home Visiting Service at Ashfield North PCN. This is a rewarding opportunity to make a real difference by providing timely, high-quality care to patients who are unable to attend GP practice settings across four practices in and around Sutton-in-Ashfield, serving a population of around 52,000.
In this role, you will assess, diagnose, and manage a wide range of acute and chronic conditions through home visits and same-day consultations, delivering safe, effective, and person-centred care to those who need it most.
You will play an important part in supporting GP practices by responding to triaged requests, helping to ease pressure on primary care services and improving access for housebound and care home patients. The role also offers the chance to contribute to reducing avoidable hospital admissions and supporting proactive care for patients within frailty pathways.
As part of the Additional Roles Reimbursement Scheme (ARRS), you will be joining a wider multidisciplinary team across the PCN, working collaboratively to support integrated care and improve outcomes for patients and communities.
You will work closely with GP practices, community services, and PCN colleagues, building strong working relationships while maintaining high standards of clinical practice, communication, and patient advocacy in line with organisational and professional requirements.

- Act as an autonomous advanced practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients.

Job responsibilities
- To act as an autonomous advanced practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients.
- To avoid inappropriate admissions and maintain patients in their home environments with a safe package of care and referral to other agencies as necessary, including directly admitting patients to hospital when necessary.
- To respond to GP triage for home visiting both in the patients home and in care homes.
- Provide evidence based professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence and a proactive approach to care.
- To develop and lead new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice population.
- To provide education and training to other staff and students.
- To undertake research and audit as part of the role.
Advanced Clinical Practice
- The Advanced clinical Practitioner works autonomously and is accountable for own professional actions.
- Undertakes clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
- Utilises the four pillars of Advanced practice: clinical practice, leadership, facilitating learning and evidence, research and development.
- Assesses diagnoses and treats patients in own home who require acute/none acute medical attention and set up a package of care and/or refer to appropriate agencies if appropriate
- Prescribes appropriate treatment in line with Local/National prescribing guidelines according to assessment.
- Assesses diagnoses and treats patients who require medical care
- Telephone Consultations
- Refers patients to other medical specialities for assessment if deemed appropriate.
- Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for further clinical investigations including but not limited to ECHO, ECG, Bloods, X-ray
- Ability to interpret a range of diagnostic tests and routine clinical procedures.

Job summary
Kennet and Avon Medical Partnership (KAMP) is a welcoming and innovative GP practice serving 17,000 patients across Marlborough, Pewsey and the surrounding villages in beautiful North East Wiltshire.
We have an exciting opportunity for a Dispenser or Trainee Dispenser to join our friendly and supportive dispensing team.
We are seeking an enthusiastic individual to work between 30 and 37.5 hours per week. We can offer either a four-day (30 hour) or full-time working pattern.
30 hours as follows: 09:00 - 18:30 Mondays, Thursdays and Fridays; 14:00 - 18:30 Wednesdays.

The post holder will be based primarily in the Marlborough surgery, covering Pewsey surgery as and when required.
Main duties of the job
The post-holder will join the Dispensing team to deliver the following:
- Manage repeat medication requests safely and efficiently.
- Respond to patient queries relating to prescriptions and medicines.
- Dispense and check medications in accordance with practice procedures.
- Support effective stock management and medicines governance processes.
- Undertake daily prescription and cash handling checks, maintaining accurate records.
- Maintain patient confidentiality and information security at all times.
- Contribute positively to the KAMP mission, values and strategic objectives.
