Main duties of the job

TheImmunisation Nurse has a responsibility to be an effective member of the Immunisation team working across Children and Family Health Surrey (CFHS) administering immunisations in a variety of community settings including schools.
The post holder will promote the mental, physical and emotional health of individuals, families and groups and be aware of the needs of children, families and carers including vulnerable children. You will also have knowledge or understanding of partnership working, such as GPs, shared community services and local education authority. The service will be offered within the framework of national and local targets and available resources.
This position will be 18 hours per week, Term Time only and based at Walton Health Centre
Main Responsibilities
- Assist in the delivery of the school and community immunisation programmes following local policies, PGDs, and SOPs.
- Administer vaccines safely, ensuring correct cold chain storage, handling, and accurate documentation.
- Prepare, transport, and maintain immunisation equipment and vaccine stocks for sessions.
- Act as Lead Nurse when fully competent, supervising and mentoring bank nurses, students, and other team members.
- Maintain up-to-date clinical knowledge and participate in professional development, training, and audits.
- Communicate effectively with children, young people, families, and multidisciplinary teams, adapting language and approach to individual needs.
- Support children and families, addressing anxieties, concerns, or complaints, and act as an advocate where appropriate.
- Participate in policy development, service improvement, and implementation of immunisation programmes.
- Organise workload independently, ensuring accurate record-keeping, data management, and reporting.
- Take appropriate action in safeguarding concerns and adhere to NMC Code of Professional Conduct.
- Provide evidence-based advice on immunisations, including side effects, to children, parents, and the public.
- Contribute to meetings, working groups, and professional discussions, supporting team communication and cohesion.
- Participate in audits, research, and mandatory training; supervise and teach students, junior staff, and carers.
- Undertake other duties as required consistent with the role and grade of the post.
Ideal Candidate
Essential
- Registered Nurse with experience working with children and young people.
- Knowledge of NHS issues, clinical governance, and risk management.
- Effective written and verbal communication skills.
- Able to assess needs and act appropriately, work independently, and manage own time.
- IT literate and able to work collaboratively within a team.
Desirable
- Mentor preparation, ENB 998, or Practice Supervisor/Assessor qualification.
- PHE core immunisation training and Skills for Health immunisation modules.
- Experience in school nursing, clinical supervision, appraisal, and immunisation delivery.
- Teaching skills and ability to be assertive under pressure.
Other requirements:Full UK Driving Licence and access to a car insured for business use.

We are currently looking for a part time nurse practitioner to join our friendly team.
The candidate will provide direct input in the care of practice patients to include, treatment, preventative care, screening and patient education.

The Lawns Medical Practice is one of 2 GP practices located in Diss. We have 3 GP Partners, 1 Managing Partner, and a fantastic nursing team comprising of 1 Advanced Nurse Practitioner, 3 Practice Nurses, 1 Outreach Nurse and 2 HCAs. We have a great team of admin and dispensary staff and we dispense to approximately one third of our 7100 practice patients. We share our building with Parish Fields practice and also with a large selection of community teams, including nursing and physio. We all have a good relationship and try to help and support each other. Working alongside the community teams and our resident Social Prescriber is extremely valuable in being able to ensure that our patients have the full package of care that they may need.
COVID-19 has changed the way we work but we are a strong team and we are coping well with all of the many changes we have had to implement. Our aim is to always ensure that our patients and staff feel comfortable and safe and we have achieved this.

Cornerstone Practice and Healthcare is looking for a Same Day Consultation Nurse to join our friendly, forward-thinking team, caring for a large and growing practice population across four sites in Blackburn and Darwen. You'll be the clinician patients see on the day they need us most - assessing, diagnosing and treating a wide range of undifferentiated presentations, by telephone and face-to-face. Main duties of the job
You'll see patients booked into your same day sessions, by telephone or face to face, using your clinical skills to assess undifferentiated presentations and decide, there and then, how best to manage each one.

Job summary
Continuous expansion in our Renal Home Therapies (RHT) programmes requires appropriate & skilled workforce. We aim to provide the best care and keep patients safe at home. We are looking for a dynamic, enthusiastic, hardworking and passionate renal nurse. This role will enhance your knowledge, skills and experience both in Peritoneal Dialysis (PD) & Home Haemodialysis (HHD) and gain management experience. You will have opportunities to work autonomously and collaboratively with the Multi-Disciplinary Team.
A skilled renal nurse with substantive renal experience (passionate for teaching and excellent communication skills), in possession of a relevant specialist and recognised renal course and able to travel/make use of public transport for home/community visits are the essentials.
You will have the opportunity to work both in PD and HHD - gaining the knowledge, skills and expertise - a truly enriching experience.
Main duties of the job
- Carry out planned care and training for a group of patients without direct supervision.
- To coordinate the PD and HHD training program based at Home Therapies training unit and/or satellite units and external companies.
- Plan, organize and deploy patients into the PD and HHD training program according to suitability.
- Prioritize and manage patient workload as per referral of patients in liaison with the Head nurse Use initiative where training targets are not being met, discuss with Head Nurse, Home therapies team at earliest opportunity and agree revised action plans.
- Assess the needs of patients, relatives and carers and provide them with information as requested. Liaise with the dietician, counsellor, technical services, transplant team and refer to patient seminars as appropriate.
- To keep up to date with current developments in nursing and ensure evidence based practice.
- Develop new skills and knowledge pertaining to the specialty as advised by the senior home therapies team.

We have an exciting opportunity for an exceptional and highly experienced senior nurse to join Acute & Specialist Medicine as Lead Nurse for Acute Medicine at St Mary's Hospital.This is a pivotal leadership role within the Directorate Management Team.
The post holder will provide strategic, professional, clinical and operational nursing leadership across a broad and complex portfolio, while working closely with Emergency Department colleagues to strengthen urgent and emergency care pathways.

Job summary
Thistlewayte Ward is a 20-bedded acute Medicine for the Elderly ward at St Mary's Hospital, caring for older adults with frailty, complex medical conditions and discharge needs. The ward offers a supportive learning environment in which to develop expertise in comprehensive assessment, frailty care, rehabilitation, medicines management, dementia and delirium care, multidisciplinary working, family communication and safe discharge planning.
As a Band 5 Staff Nurse, you will assess, plan, deliver and evaluate evidence-based, person-centred care for a group of patients; promote dignity, independence and wellbeing; recognise and escalate deterioration; administer medicines safely; maintain accurate documentation; and work closely with medical, therapy, pharmacy, discharge and community teams. Job summary
We are seeking a motivated and compassionate Band 5 Staff Nurse (37.5 hrs per week) to join our friendly and dynamic Endoscopy Team at Hammersmith Hospital, Imperial College [MA1] Healthcare NHS Trust.
You will play a key role in delivering safe, high-quality, and person-centred care to patients undergoing a range of diagnostic and therapeutic endoscopic procedures. This is an excellent opportunity to develop specialist skills in areas such as gastroscopy, colonoscopy, EUS, ERCP, and bronchoscopy, working within a supportive and highly skilled multidisciplinary team.
Previous Endoscopy experience is not essential, as full training and support will be provided. However, applicants with experience in post-anaesthetic care, recovery nursing, day surgery, theatres, critical care, acute medicine, or surgical wards are particularly encouraged to apply, as these skills are highly transferable to the Endoscopy environment.

Main duties of the job
- As a Band 5 Staff Nurse within the Endoscopy Unit, you will be responsible for delivering high-quality, safe, and compassionate care to patients throughout their endoscopy journey, from admission and preparation through to recovery and discharge. You will work closely with consultants, nurses, healthcare support workers, and the wider multidisciplinary team to ensure an excellent patient experience and positive clinical outcomes.
- Your duties will include assessing, planning, implementing, and evaluating patient care, assisting with diagnostic and therapeutic endoscopic procedures, monitoring patients receiving sedation, and providing post-procedure recovery care. You will be expected to recognise and respond appropriately to changes in patient condition, maintain accurate documentation, and contribute to a safe and effective clinical environment.
- Previous Endoscopy experience is not essential; however, experience in a recovery, theatre, day surgery, acute care, or critical care setting would be highly advantageous.
- Experience caring for patients following sedation or anaesthesia will be particularly valuable as recovery nursing is an integral part of the Endoscopy patient pathway.

Job summary
Our UTC operates a Digital Front Door, which is a clinically driven digital triage tool designed to improve safety and efficiency across urgent care and is changing the way patients reach and move through urgent care in Greenwich. Our patients use the DFD to check in and complete a short, guided triage, enabling faster prioritisation and smoother patient journeys.
Whats New?
Our clinical system supplier is developing a new appointment book, which for the first time will allow patients to be booked directly into an urgent care appointment rather than simply being given an arrival time or having to sit and wait.
We are redesigning our Directory of Services (DOS) so that appointment slots align to NHS Pathways disposition timescales. A patient with a see a GP within 48 hours disposition can be offered a later appointment, while presentations more likely to need an emergency department can be booked within two hours.
NHS Pathways sorts patients leaving the digital front door into a number of groups, including those who are not urgent and can safely be seen later. Booking to the right slot at the right time helps us manage surge, improve flow and demand, and reduce attributable breaches.
Greenwich Health will be launching a new system where NHS 111 can book patients into a named urgent care appointment.
This role is available on a bank basis. 12-hour shifts finishing no later than 22:00. 7 days/week availability will be required.
Main duties of the job
This is the perfect opportunity to be part of a diverse team at the very front of urgent care in Greenwich, and to help deliver a model that is the first of its kind in South East London.
As a Front Door Navigator you will be the first person a patient meets when they arrive at emergency and urgent care. Working from a tablet at the entrance, you will welcome patients, establish why they have attended, check whether they hold a booked appointment made through NHS 111 or the digital front door, and direct them to the right place without delay.
You will keep people moving away from the reception queue and make sure that patients who are unwell, distressed or vulnerable are brought to the attention of a clinician immediately. This is a non-clinical role, you will not assess, triage or advise on a patients condition, but your judgement about when to escalate is central to keeping patients safe.
You will also play an important part in helping us evaluate the new model, recording how patients arrive and where they are directed, and feeding back to the UTC Lead where the appointment book or the Directory of Services is not working as intended.

Furthermore we are a Trauma Unit receiving trauma patients from across Hertfordshire and North London. We are a busy unit receiving up to 90 ambulances per day and over 120,000 patients per year. - The post holder will assist the Endoscopist with procedures/investigations and manipulation of intricate equipment after appropriate training.
- The post holder will be responsible for ensuring nursing care is delivered safely and effectively.
- Liaise, report and document any relevant nursing/patient needs with other health care professionals with ongoing responsibility for patient care, communicating changes as they occur.
- Responsible for the timely entry of accurate, complete and valid data into the specialist Endoscopy reporting system.
- Promote health education within the Endoscopy Unit and be able to advise the patients and their relatives and others in health promotion issues.

We are looking for registered Health Visitors who are innovative; motivated; and enthusiastic, seeking to make a real difference improving outcomes for children and families in Devon!
Main duties of the job
Here in Devon, we strive to deliver an excellent service to the children, young people and families living in our Community. Health visiting in Devon affords amazing variation allowing you opportunity to make a difference to the lives of families and communities across both rural and urban locations. Devon is varied with health inequalities and affluency which cross paths across the varied landscape.
You will work within corporate teams and be rostered to work in our duty hub covering a defined geographical area. You'll be supported by Team Leaders and various dedicated health professionals including Health Visitors, School Nurses and Community Health Workers.
You'll lead and manage changing and challenging work load priorities. Working in close partnership with community, voluntary and statutory agencies; multi-agency working and safeguarding children will be an integral part of your role. We are working together with our local midwifery services to support the Better Births agenda.
Continuity is fast becoming priority for every family in Devon, with a focus on a strong public health approach. The opportunity to think innovatively is increasingly becoming part of everything we do in our new model of health visiting. Our aim is not just to influence local Health Visiting agendas but also to be innovative and a partner in influencing national Health Visiting agendas.
