Job summary
This is an exciting opportunity to work within the UGI Clinical Nurse Specialist team. Under direction from the Lead UGI Cancer Nurse Specialist you will assess, plan, implement and evaluate expert nursing care and provide support to UGI and GIST cancer patients and their carers. The post holder will have continuing responsibility for the co-ordination and organisation of individual treatment programmes throughout the illness trajectory and work to develop communication across divisional and organisational boundaries.
Provide specialist nursing advice and psychological support to patients, carers and healthcare professionals.
Acts as a resource and contributes to the education and training of other healthcare professionals as required.
Contribute to service development, audit and research projects and help develop educational packages as needed.
Main duties of the job
Work as an integral part of the UGI cancer multidisciplinary team within both Plymouth Hospitals NHS Trust and across the tertiary centres. May be asked to support other cancer nursing specialist teams when necessary.
o Act as liaison with tertiary centres and UGI Cancer CNS's to ensure smooth transition of care between centres.
o Assess, plan and implement and evaluate expert nursing care and provide support to patients and their carers. The post holder will have continuing responsibility for the co-ordination and organisation of individual treatment programmes throughout the illness trajectory and work to develop communication across divisional and organisational boundaries.
o Provide specialist nursing advice and psychological support to patients, carers, and health care professionals.
o Acts as a resource and contributes to the education and training of other health care professionals as required.
o Contributes to service development, audit and research projects and helps develops educational packages according to need of care.

Job responsibilities
- Act as keyworker to patients and relatives whilst undergoing cancer treatment at Plymouth Hospitals NHS Trust.

- Act as patient advocate when relevant and be able to educate on all the treatment options to enable a patient to make an informed choice, negotiating with the medical team to agree a pathway of care.
- Provide continuity of care throughout the patients journey and liaising across specialized areas ensuring that care is seamless between tertiary centres.
- Contribute to the development of specialized programs of care and provide specialized advice, written and oral information to patients, relatives.
- Provide clinical advice and support to health care professionals within and across organizational boundaries.
- To formulate individualized patient treatment care programs in conjunction with the medical/nursing team considering

This role provides specialist urology care to patients with a variety of bladder dysfunction and bladder outlet obstruction from diagnosis through treatment and beyond. You will run nurse-led clinics such as One stop LUTS, TWOC, bladder treatments, recurrent UTI, Functional urology issues ,nephrostomy care and Intermittent self catheterisation teaching. The team also offer a 'rapid access' walk in service from the main hospital site for a variety of urological issues such as blocked /stuck indwelling catheters, acute retention and blocked nephrostomy.
You will participate in multidisciplinary team meetings, and arrange diagnostic tests and investigations to support timely, effective care.
You will be actively involved in Audit and service development
You will coordinate care across services, making referrals to appropriate support networks and managing treatment-related side effects and ongoing patient needs, ensuring patients receive holistic, patient-centred care at every stage of their pathway.

This is a Specialist nursing post which enables the individual to be responsible for aspects of the provision of high-quality Specialist Nursing care to both inpatient and outpatients within the speciality and to provide expert advice and support for patients diagnosed the condition.

Main duties of the job
You will have the right attitude, skills and knowledge to share our passion in delivering the best quality care experience for children and their families at point of initial contact with CAMHS services.
Your duties will be related to leading on the clinical delivery of care to children, young people and their families accessing CAMHS from first presentation to management of existing mental health presentations alongside providing high quality supervision to the team.

NHS England has established 17 Maternal Medicine Networks (MMN) around England. These networks are relatively new so this is an exciting post as the networks are evolving to meet the needs of the population in the East Midlands. The East Midlands is a regional network and works with Maternity Units in Leicestershire, Nottingham, Derbyshire, Lincolnshire and Northamptonshire. UHL is the lead center working closely with our co-leads Nottingham and Derby hospitals to optimize the care of women with pregnancy complicated by maternal conditions. Each hospital within the MMN network is required to monitor and submit their activity numbers
The key responsibility of this role is to provide administrative facilitation of team meetings for the regional MDT meetings as part of the network arrangement, ensuring external colleagues are able to join the meetings via electronic conferencing. This will include preparing, organising and circulating agendas, minutes, results and sending out minutes of the meeting to relevant clinicians.
The role will also be responsible for collecting, maintaining and submitting the data required for the network MDT and national submission and monitoring the dataset.
It is required that the post holder will provide administration support to the Network core team and will be a pivotal point of contact and support to the clinical leads across all network Trusts.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic and highly motivated individual to be the network MDT coordinator and data Navigator for the East Midlands Maternal medicine Network . As a member of the team you will be responsible for facilitation and coordination of the MDTs primarily providing support to the network core team members .
The role is varied and will develop to meet the needs of the service. The successful candidate will be required to possess excellent communication, relationship and administration skills. It will involve preparing the MDT, collecting, maintaining and the national submission and monitoring of the dataset.
The post holder must engage effectively with network hospitals and Trusts to ensure the data collection process is efficient, smooth and well-supported. This role also requires data collection, compilation of relevant reports and audits and the post holder must be able to work to tight deadlines and at times under pressure.
You will become a vital part of a talented team of people, committed to providing the best care and treatment to pregnant people in the East Midlands.

Main duties of the job
The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. An opportunity has arisen for a passionate, friendly and enthusiastic individual to work as the Facilities Co-ordinator - Residential Accommodation within Facilities.
The post holder is required to provide an efficient and effective support service to the residents in staff residential accommodation on the Ysbyty Gwynedd Hospital site, including accommodation booking, security of property, key holding, and record keeping.
They will be working without direct supervision, using own initiative and prioritising own work as necessary. Adhering to laid down standards, following set procedures and delivering services in a timely manner. The post holder will liaise between tenants and other parties as necessary.

We are a multi-professional progressive team providing a comprehensive, evidence based and patient focused service for Contraception and Genitourinary Medicine specialising in integrated sexual health and HIV services. They will be working at the main hub at the Lyng Centre and our outreach services across Sandwell.
The post holder will work alongside consultants and senior specialist nurses to gain appropriate training and skills in order to provide clinical care for presenting groups of patients including history taking, clinical assessment and examination, safeguarding, diagnosis, treatment and evaluation of their care.
The post holders must be motivated to provide all aspects of sexual health and contraception to vulnerable and hard to reach groups who would otherwise not engage in Sexual health services
Main duties of the job
-To assess, plan, implement and evaluate individual care based on consultation, assessment and individual needs
-Undertake some higher-level clinical duties under the direction/supervision of a senior nurse/practitioner
-Collect specimens as directed by senior clinical nurse or doctor, ensuring its safe storage/transport or disposal as appropriate. Ensure appropriate labelling and recording of specimens and request forms.
-Collect blood samples (peripheral or venous) ensuring its safe storage, transport or disposal, as appropriate.

Main duties of the job
The MHST Senior Practitioner role supports the delivery of evidence-based 1:1 and group interventions for children and young people presenting with mild to moderate mental health need.
The role requires joint working and close liaison with education colleagues to support the whole school approach to mental health and wellbeing within school settings, facilitating appropriate signposting to other services as required. The post holders will actively contribute to the delivery and development of a quality MHST service within Oxleas.

Job summary
Please note that this role will be part-time working 25-hours a week.
The Care Coordinator will be part of the Primary Care Network (PCN) Multi-Disciplinary Team (MDT) who are responsible for managing the care of people registered with practices within a particular PCN. This will involve coordinating the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN population.
Main duties of the job
The post holder will contribute to tackling inequalities in health and social care particularly regarding individuals with long-term conditions. An ethos of promotion of independence and partnership-working is integral to this post.
A key part of the role of a care coordinator role is in the care Homes MDT: improving the continuity of care by acting as a point of contact for residents, families and professionals who visit care homes, such as MDT members and in-reach specialists.
They will support the MDT with the weekly virtual home round through identification of people in need of review, or collation of information on people requiring an MDT review in addition to providing coordination, secretarial and administrative support to the MDTs within a single or multiple PCNs.
