Job summary
North Somerset Wellbeing College provides free psycho-educational workshops, courses and group activities which are available to adults living in North Somerset. These are run online and in person at community venues across the county. The sessions are designed to explore, improve, and maintain mental health and wellbeing. In addition, brief 1:1 support can be offered to those requiring signposting to other appropriate services.
Main duties of the job
We have an exciting opportunity for someone to lead our small, friendly team in North Somerset. As a Team Manager, you will provide support and guidance to tutors to work in a psychologically and trauma informed way, be values led and be looking to grow both personally and professionally. You will have experience of managing and directing a community-based service working within a psycho-educational/recovery college approach utilising co-production methods. As well as proven experience of budget/financial management, you will have excellent IT skills, working on MS Office programmes, systems/databases and the ability to understand and use performance information to drive ongoing service improvement. The successful candidate will have the ability to develop and manage relationships with key partners, and experience of leading a dispersed team, providing effective team supervision, support and guidance.

The Directorate of Trauma and Orthopaedics is committed to delivering high-quality, patient-centred care through innovation, collaboration and continuous service improvement. We are seeking a motivated and compassionate registered Nurse or Allied Health Professional to join our multidisciplinary team and support the development of patient pathways and experience.
The post provides an opportunity to work across two hospital sites, delivering specialist care to elective and trauma orthopaedic patients alongside Orthopaedic Practitioners, nurses, therapists, consultants and the wider MDT.
Our service includes three inpatient wards caring for patients with traumatic musculoskeletal injuries, elective surgery, post-operative recovery and rehabilitation. Fracture clinics provide trauma and elective services for adults and children, supported by plaster room and MDT services.
We are looking for an enthusiastic, proactive practitioner with excellent communication, organisational and interpersonal skills, who can work independently and collaboratively. A commitment to evidence-based care, professional development, service improvement and excellent patient experience is essential.
This development opportunity offers support to achieve relevant competencies and qualifications, including clinical skills and Non-Medical Prescribing. Where required, the successful applicant will initially be appointed at Band 6, with progression following successful completion of the programme.
Main duties of the job
The Band 6/7 Ward Practitioner will work autonomously within the Orthopaedic Directorate, providing daily clinical review of inpatients across both sites.
The post holder will assess patients, monitor and act upon clinical results and diagnostic investigations, identify and escalate clinical concerns, support discharge planning and coordinate multidisciplinary care.
They will work closely with consultants, junior doctors, nursing and therapy teams to optimise patient flow, improve continuity of care, enhance patient safety and support the delivery of high-quality patient-centred care.

Woodbank Surgery in Brandlesholme, Bury are looking for a highly motivated and hardworking Advanced Nurse Practitioner to join their clinical team. The post holder will work collaboratively with the GPs, PNs and HCA to provide assessment, treatment and care to patients ranging from infants to elderly people.
The role is for 37.5 hours per week, to be worked Monday Friday. The practice is open until 8pm on Tuesdays and cover for this would be required. Salary is dependent on qualifications and experience. Annual leave allowance is 5 weeks + bank holidays. Enrolment to the NHS pension scheme is automatic.

Main duties of the job
The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.

Job summary
The Critical Care Outreach team (CCOT) covers 2 hospital sites 24/7 at Eastbourne District General Hospital and Conquest Hospital.
We are looking for an enthusiastic, highly motivated nurse with extensive critical care experience to join the Outreach team. This is an ideal opportunity for a critical care who wishes to expand their clinical skills, to work autonomously within a supportive and progressive team. The post holder will be expected to work flexibly across both hospital sites (12 hour shifts with internal rotation of both nights and weekends).
Main duties of the job
At EDGH and CQ, CCOT is led by a Senior Matron for Critical Care. The service provides expert assistance and support to acutely and critically ill patients outside the critical care unit. This includes patients recovering from critical illness and those at risk of deterioration (in ward and emergency care environments). You will work in collaboration with ward based teams and take part in education programmes for all groups of staff.
The post holder will complete an orientation period in accordance with the Trust's induction programme. You will also be supported completing the National CCO Professional Framework.

Mayfield Surgery is looking for one or two Salaried GPs to join our friendly and supportive GP Training Practice in Roehampton, South West London.
We have flexibility to offer 37 sessions per week across one or two posts and welcome applications from GPs at different stages of their careers. No particular special interest is required.
We operate an Accurx Total Triage model, with patient requests clinically reviewed according to need and appropriate timeframe. This is supported by a Duty GP who provides oversight of on-the-day demand and works closely with our wider clinical and administrative team.

Our multidisciplinary team includes GPs, nursing staff, a Clinical Pharmacist, Social Prescriber and First Contact Physiotherapist, supported by an experienced administrative team.

We are seeking a motivated, compassionate and values-driven practitioner to join Leeds Community Healthcare NHS Trust's new Proactive Support Service.

The service promotes stability, prevents escalation into care and improves long-term outcomes for vulnerable young people. The team also supports the Children with No Onward Destination (CNOD) pathway, working with young people who are medically fit for discharge but unable to leave hospital due to placement instability, accommodation issues or complex social circumstances.

Main duties of the job
This role would particularly suit a Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD). However, we also welcome applications from Registered Social Workers and other suitably qualified and registered practitioners who can demonstrate relevant experience and skills in supporting children and young people with complex needs, trauma, behavioural challenges, and placement instability.

This year CATAT have been given the opportunity to expand the team with several different posts, some of which have already been filled. But we do have a post that has now become vacant for a CNS - this CNS needs to have their qualification of Non-Medical Prescriber as well as a Mental Health Nursing qualification.
CATAT who after our first year of being live (2023) was highly recommended for the Working Together for Better Mental Health at the staff excellence awards. Are looking for professionals that have a keen interest of working with CYP and their families, wanting to help them achieve better outcomes for their future and strive to keep them in the community.
The aim of our team is to prevent CYP from having inpatient admissions wherever possible, and to support them and their family in the community in a proactive way. The team will serve as a gateway in and out of Tier 4 (T4) provision. CATAT work alongside the core community teams as well as partner agencies to offer CYP increased/additional support.
CATAT have 2 main pathways:
Admission Avoidance - Least restrictive option, CYP who are already open to a core team but need a more intensive intervention to support their mental health needs. To prevent things from worsening and/or needing a hospital admission.
In-reach/Outreach - support to young people leaving inpatient settings (T4) to help them adjust back into life in the community.

An exciting opportunity has arisen for a dynamic staff nurse to join First Community Health and Care working with the Responsive Services Team.
Responsive Services consists of the Urgent Community Response Team, Intermediate Care Team, Virtual Ward and Referral Hub.
This role with work in in collaboration with Primary Care and Secondary Care Colleagues, Urgent Community Response, Intermediate Care (Responsive Services), Community Nursing and other Specialist Health Care Teams across First Community, statutory and voluntary agencies to deliver excellent standards of patient care for patients who are acutely unwell requiring support to avoid admission to the acute hospital. Providing practical care, education and training for patients and carers admitted to responsive services.
Main duties of the job
You will complete assessments in the patients home and undertake treatments including IV medications, wound assessment, and management. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own home.
The candidate will be able to adapt to change and work in a busy environment. Excellent communication skills are essential to enable liaison with all our health and social care partners in the management and assessment in the community.
Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.

We are seeking to recruit a Community Nurse Practitioner to provide and support the provision of high-quality patient care. This is an exciting opportunity for an experienced, dynamic, enthusiastic, and motivated practitioner to develop new ways of working. They will be integral in delivering community services in different ways across the localities. As a Community Nurse Practitioner, you will be responsible for the assessment of the health and clinical needs of the patient, to undertake consultations with patients as an autonomous practitioner using advanced clinical practice skills and own clinical judgement to diagnose, treat, to prescribe where required, refer to other services and discharge patients.
Main duties of the job
A key part of the role will include making rapid clinical assessments of patients with undifferentiated conditions, making evidenced based treatment plans whilst aiming to ensure patients are cared for in the most appropriate place. As part of the Community Urgent Care ResponseTeam, the practitioner will be expected to respond to patients who are experiencing crisis within a 2 hour national target. You will be required to ensure delivery of high quality patient centered care measured by clinical effectiveness, patient safety and patient experience as well as meeting the key performance measures as required by the Trust.
This is an exciting role for the right candidate. You will be joining a developing and progressive service, giving you the opportunity to participate in the ongoing development of the team and the Clinical Hub. The successful candidates must have experience of service development and clinical leadership to ensure patients and their families receive timely, effective and supportive care. You will be managed and supported daily by the Advanced Clinical Practitioners, Hub Lead and Medical Lead as well as giving support to junior staff within the Dudley Clinical Hub.

Job summary
The Service Desk Officer will provide a proficient NHS GM Digital & IT support service across the customer base. Provide in-depth technical remote support for all end-user equipment, systems and software supported by the IT Service. We are looking for an enthusiastic and dedicated nurse to join our integrated, multidisciplinary team to work collaboratively, deliver care meeting best practice in heart failure. The post holder will be expected to contribute to the service as a whole under the direction of the lead specialist nurse for heart failure. The Heart Failure Specialist Nurse provides expertise in all aspects of cardiac care provision with a specific interest & experience in Heart Failure. The post holder will be an experienced practitioner with extensive experience of cardiology & the ability to work autonomously and as an autonomous practitioner will be required to manage their own caseload including nurse-led clinics, virtual clinics, and telephone consultations. Will also provide support & education to cardiac inpatients' within the hospitals. The post holder will provide a leadership role and forge links with those involved in the development of the cardiology services both internally and externally. They will analyse service performance by undertaking regular audits, quality improvement activities and participate in the strategic development of the service. Main duties of the job
As an autonomous practitioner, manage a caseload of patient's both acute and chronic heart failure across the Trust providing specialist assessment, planning, implementation and evaluation of care. Undertake nurse-led heart failure clinics to monitor, review and optimise patient management. Work cross-site under the direction of the Trust's Lead Heart Failure Nurse and collaborate with the multidisciplinary cardiology team to provide specialist inpatient heart failure reviews, management plans and patient education. Ensure patients understand their diagnosis and evidenced based heart failure medications that improve symptoms and prognosis whilst supporting effective discharge planning, arrange follow-up care. Maintain the heart failure telephone advice service for patients and healthcare professionals. Incorporate information and communication technologies (ICTs) into patient pathways and support the development of digital personal health records and digital care pathways. Work within agreed policies, procedures and protocols using professional judgement. Utilise multidisciplinary and multi-agency resources to ensure patients receive appropriate services and referrals.
