Job summary
A staff nurse position is available to join our Hospital at Home/Virtual Ward team within Central Lancashire. This service provides patients with safe, hospital-level care in their own homes or place of residence, helping to prevent unnecessary hospital admissions and supporting earlier discharges where appropriate.
Working in collaboration with Lancashire Teaching Hospitals and Lancashire and South Cumbria Foundation Trust, the service takes a multidisciplinary approach. Patients are reviewed by the clinical team through both face-to-face visits and remote monitoring, including telephone consultations and technology-enabled care.
The service is based at Leyland House, with visits carried out in patients' homes, places of residence, or within the hospital to support onboarding to the service. It currently operates 7 days a week from 8am to 6pm, with plans to extend to 8am to 8pm.
A valid driving licence and access to a vehicle during working hours are essential, as the role involves visiting patients in their own homes.
Main duties of the job
We are seeking an enthusiastic, engaging, and caring individual to join the Central Lancashire team, with a passion for patient-focused care and a commitment to supporting and contributing to this developing service. The role requires someone who can work effectively both within a team and independently, with strong organisational skills to prioritise workload efficiently. Responsibilities will include carrying out patient assessments and treatments in their own homes, conducting remote monitoring and telephone assessments, and providing clear communication to patients, families, carers, and the wider multidisciplinary team. You will be expected to recognise changes in patients' conditions and escalate appropriately, while also delivering education and support to help patients stay healthy for longer. In addition, you will play an active role in supporting learners and less experienced staff, maintaining accurate records, and contributing to daily huddles, departmental meetings, and MDT discussions. A commitment to personal development is essential, ensuring that practice reflects individual needs while working within local and national guidelines, processes, and policies.

The post holder will be joining 12 other psychological professionals within the team, including qualified psychologists, CAPs, a Trainee CBT therapist and Assistant Psychologists. There is regular clinical supervision, therapy specific supervision including CFT, EMDR. and a commitment to training. Main duties of the job
Working in a skilled and motivated older adult community mental health team that values a range of interventions and support to help people in their recovery journey.
Delivering a high-quality service to service users and their families as part of a collaborative and supportive team.
Key-working of an identified caseload with access to regular and robust supervision.
Delivering assessment, formulation and psychologically informed interventions, in accordance with an agreed care plan, to service users experiencing a wide range of mental health difficulties. Interventions are primarily individual but may include groups in the future .
You will be responsible for the assessment of care needs, including risk, and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence-based interventions. You will, in conjunction with the multi-disciplinary team, provide interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.

We are looking for enthusiastic, innovative and engaging Band 5 NMC Registered Nurses who have a passion for the delivery of high quality patient care within the community to join our Acute Care at Home - Virtual Wards.
Our Virtual Ward is a new and innovative service which offers face to face acute care in the patient's own home. The Virtual Ward has an agreed admission criteria which works in partnership with supporting care services and the aim of the service is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', and their carers' lives. This means we can proactively manage people living with frailty at home, and for those that have a sub-acute episode we can improve a patient's experience and outcomes. In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.
You will be able to prioritise your own workload without supervision, have an excellent patient focused approach and use your initiative as well as work as part of a multidisciplinary team.
This role is delivered in-person and works on a 7 day per week shift basis. Fixed term for 12 months.
Main duties of the job
By joining the Virtual Ward Team you will be responsible for the clinical assessment of the needs, development of programmes of care, and for the implementation and evaluation of theseprogrammes for patients and their relatives.
As part of this role you will also be expected to:
- Provide a high standard of clinical nursing within the ward / department.
- Take charge of the ward when rostered and assessed as competent to take on the roles and responsibilities of leadership and supervision

The opportunity has arisen to recruit a band 6 nurse practitioner to join the psychiatric liaison older person team.
The post holder will provide comprehensive mental health assessments, referrals and follow-up arrangements.
Main duties of the job
The post holder will triage individuals over the age of 70yrs accessing the ED for health and mental health issues; risk assessment and prioritization of interventions and assessment to facilitate an appropriate timely response. This is of significant benefit to patients attending the emergency department by reducing waiting times, streamlining the transition through the department and improving follow-up appointments. To support this process, the post holder will have excellent knowledge of Primary and Secondary Mental Health and Substance Misuse Services as well as Community and Tertiary services.
The post requires a high level of ability to undertake psychosocial assessments, risk assessments and draw up safe and effective discharge and progression plans. This includes the use of assessment tools, crisis intervention, signposting to community resources, follow-up, short-term contact clinics, and education for individuals, families, staff and the public.
The post holder needs to be able to work well with-in the team but also be confident and competent to work independently, and will exercise an appropriate level of independent judgement when setting work priorities.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers will be given the same opportunity to apply.

We are looking for a Practice Nurse or a Nursing Associate to join our small, busy team in North Kensington.

Main duties of the job
This role is focussed on providing appropriate, effective interventions and treatments to people with a range of mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
The post holder will work autonomously to undertake emergency assessment in a wide range of locations, planning and delivering defined, intensive, specialist interventions in line with the CPA framework to people with severe and enduring mental health needs, who maybe distressed and in crisis. Where appropriate the post holder will work alongside other services to plan, deliver and review effective "wrap around" intensive interventions to manage risk and prevent hospital admission for those with the most complex needs. The post holder will act as Care/Recovery coordinator or associate Care/Recovery coordinator as required.

Job summary
This role is funded by the NIHR funded UK Dementia Trials Network (UKDTN), supported as part of the Alzheimer's Society UKDTN Research Nurse Programme, and hosted by Surrey and Borders Partnership NHS Foundation Trust.
Main duties of the job
The Alzheimer's Society UKDTN Lead Clinical Research Nurse is a senior clinical leadership role responsible for leading the delivery and development of early phase dementia and mild cognitive impairment (MCI) CTIMP and ATIMP clinical trials.
The post holder will lead the delivery of a high-quality early phase dementia clinical trials portfolio, ensuring compliance with ICH-Good Clinical Practice (GCP), UK Clinical Trial Regulations, the UK Policy Framework for Health and Social Care Research, and local Research & Development requirements. They will provide clinical and operational leadership to the research nursing team, support workforce development through education and mentorship, and act as a clinical expert and ambassador for the Alzheimer's Society UKDTN Research Nurse Programme, promoting research as an integral part of the dementia care pathway.
The post holder will contribute to regional and national research priorities, workforce development and UKDTN and Alzheimer's Society initiatives that may require travel within the local region and occasional UK-wide travel to support programme activities and network meetings.

Job summary
An exciting opportunity has arisen for experienced Practitioners (RMN, RNLD, RGN or Social Workers/ Allied Health Professionals) with a criminal justice interest, to work in the Black Country Health & Justice Outreach/Reconnect Service. The post holder would be expected to work in the community within Black Country in the Outreach element of the team but may also incorporate other areas of Health & Justice, such as the Reconnect Service, Police custody or court. This will include providing high quality assessments of people attending and being released from Police Custody/Prison and supporting people under the care of the Outreach/Reconnect Team. The service will be delivered to people of all ages including those under the age of 18yrs who present with any vulnerability including but not limited to; mental health, physical health, learning disability, head injury, unemployment, housing, educational needs etc.H&J is not itself a treatment service, but it is an identification, assessment and referral service. The role will involve assessing; accessing; and implementing a wide and, at times, complex range of care services for people over the age of 18 with increasingly complex needs.
The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.
Main duties of the job
1. Working in a Health and Justice (H&J) Outreach/Reconnect service within the Black Country, dependent on skills and service need.2. Delivering a high quality clinical service to patients and their families.3. Managing complex assessment in Custody, Court and the Outreach/Reconnect service. Responsible for clinical and caseload supervision of other team members where identified.4. Diverting and Liaising with multiple criminal justice, police and other partners.5. Identify, assess and refer people of all ages with a wide range of mental health, learning disability, substance misuse and/or social vulnerabilities when they first come into contact with the youth and adult criminal justice systems on suspicion of having committed a crime.6. When a person is assessed as having single or multiple vulnerabilities, they will be referred to the appropriate treatment or support service and an appropriate package of care and/or support will be instigated.7. Accurate, timely information on the person will be shared with police and the courts (with an individual's consent where necessary) to ensure that any charging, sentencing or disposal decisions are based upon an authoritative assessment of their mental health, any learning disability and whether they have a substance misuse issue.

The team in Lichfield is multidisciplinary and provides high-quality care for service users that experience mental health conditions. The team is part of the wider Integrated Mental Health Team covering Lichfield & Burntwood and links together for CPD events, supervision and partnership working.

Main duties of the job
The post holder will provide professional, managerial and robust clinical leadership to a
specified ward area. The post holder is responsible for promoting and assuring robust risk
assessment and management processes and will manage within the constraints of the
identified budget as directed by the Service Manager.
The post holder will actively support and participate in the integration of a whole system
approach with multidisciplinary working at all times and will perform delegated duties on
behalf of the Service Manager in their absence.

Job summary
The post holder will work as part of a Community Nursing Team to provide nursing care without direct supervision for patients registered with GPs within Midlands Partnership NHS Foundation Trust, covering Cannock Villages PCN. Main duties of the job
The postholder will hold responsibility for a defined caseload of patients drawn from a GP practice population. The team provides holistic nursing care for pupils with a wide range of learning disabilities, physical disabilities and neurological conditions. The successful applicant will work within the Stafford Hub supporting Marshlands and Walton Hall special schools and Greenhall Nursery.
Working as a member of a friendly, highly skilled team you will have the opportunity to make a difference to the lives of children and young people who have special needs and their families. You will be responsible for their healthcare needs during the school day and work in partnership with education colleagues in the school and with parents. You will also work collaboratively with the wider Multi Agency Teams to meet health, developmental and safeguarding needs of the children and young people.
The post holder will be expected to work flexibly and will need to be able to transport themselves and any equipment to the different locations within the hub.
There will a requirement for travel to other areas/hubs to support with cover and attendance at team meetings and training.
Access to a vehicle for work purposes is essential.

The Home First Team provides clinical support for patients in the community to either avoid potential hospital admissions or to support people to return home through the Discharge to Assess Pathway 1 following a period of acute care in hospital.

About us
The Whickham Practice was established in the 1870's and is situated in Newcastle upon Tyne. We moved into our modern, spacious premises in July 2002. We have consulting and examination rooms, a minor surgery suite and modern nurse and treatment rooms. We have a large patient reception area and we use an electronic system to call patients. We have spacious offices for our admin staff and a library where we hold most of our meetings.
We are a large practice but communicate well together, currently using a combination of Microsoft Teams and face to face meetings where appropriate.
