Main duties of the job
As a Mental Health or Learning Disability Nurse, you will work as an effective member of the Multi-Disciplinary team and promote the best interests of the clients and carers and ensure that prescribed nursing care is effectively implemented.
- Deliver prescribed care to a defined group within an agreed framework.
- Assess, plan and deliver, with the patient and their carer/family, nursing care which takes account of patient choice and wishes and acting as the patients named nurse.
- Accurately observe and engage with patients in order to assess their need and evaluate their progress.
- Monitor and review Nursing Care plans.

Are you a caring and experienced Practice Nurse looking for a new opportunity in a friendly and supportive environment?
Paston Surgery is a close-knit GP practice in North Walsham, and we have an exciting opening for a Practice Nurse to join our friendly team. This position is approx. 30 to 37.5hours per week, and we are flexible about how those hours are arranged.

Main duties of the job
About the Role
Were looking for an NMC registered GP Practice Nurse who can bring experience, enthusiasm, and compassion to our team. In this role, you will need to be a team player and:
Help with Chronic Disease Clinics: Support patients with long-term conditions. Ideally to lead on one or more Long Term Conditions.
Provide General Treatment Room Care: Carry out usual practice nurse duties.
You will work as part of a close team, collaborating closely with our GPs, nurse colleagues, healthcare assistants, and administrative staff to ensure the practice runs smoothly. We believe in a true team approach you'll always have friendly colleagues ready to support you on a busy day or when facing a challenge.
We understand that flexibility is important, so we are open to discussing how you'd like to spread working hours across the week.

- Be responsible for maintaining a high standard of hygiene and cleanliness in the nursing area and of the clinical equipment used
- Monitor and maintain adequate supplies of drugs, clinical items and equipment

- Appropriately recording all treatment undertaken in the patients record on the computer system
- Ensure templates and read codes are used effectively

- Discuss, implement and audit new nursing procedures
- Participate in audit and inspection as required by the GMS Contract, including cervical cytology
- Assist the doctors in the maintenance of relevant and accurate clinical histories
- Assist in overseeing/supervision of Health Care Assistants
- Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
Communication
- Team player and attention to detail skills are essential

The Open Access and Assessment (OAT) Team plays a vital role in supporting individuals entering treatment services. We provide advice, brief interventions, structured key working, comprehensive assessments, and a needle exchange service, helping people access the right support at the right time.
Main duties of the job
Undertake comprehensive assessments in order to assess the eligibility and suitability, strengths and needs of a service user in relation to the service.
To devise, monitor, review and update recovery care plans within the service to ensure that service users needs are met in a timely and appropriate manner.
Deliver care packages including, where appropriate, psychosocial and psychological interventions

Main duties of the job
As a Deputy Clinical Services Manager, your vital role will involve a wide range of clinical duties, deputising for the Clinical Services Manager and providing day to day leadership within the clinical team, including line management, staff leadership and mentorship.
You will take the lead on harm reduction across the service, supervise and support staff through clinical supervision, appraisals and training, and complete audits to ensure clinical pathways and protocols are high quality, adhered to and continually improved. You will also oversee safe medicines management, including medicines optimisation, clinical checks and controlled drug processes in line with service PGDs/PSDs, maintain high standards of clinical record keeping, support delivery of clinical KPIs and safeguarding, and escalate clinical risks appropriately.
This is a predominantly service-based role, so you will spend most of your time working from our Bi-Borough sites delivering and overseeing face-to-face clinical care, with a portion of your role providing clinical interventions reflecting your clinical skills and expertise.

We are looking for a highly skilled Practice Nurse will help carry out Treatment Room duties, chronic disease care, immunisations, health promotion and family planning. They will work as part of the primary healthcare team providing Personal Medical Services to the Practices patients, using Systmone.

We are offering an exciting opportunity for a NMC Registered Band 6 Senior Nurse Co-ordinator to join our Discharge to Access (D2A), Home First Service based in the surrounding areas of East Staffordshire.
The Home First Team provides clinical support for patients in the community to either avoid potential hospital admissions or to support people to return home through the Discharge to Assess Pathway following a period of acute care in hospital.

The ethos of the team is to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment so they can remain in their own home and avoid unnecessary hospital admissions.
As an efficient and effective team member you will work collaboratively in a multi-disciplinary team alongside the Operational Leadership to ensure high quality care delivery to service users and provide specialist assessment and care planning in conjunction with service users and carers.

Job summary
The Physician Associate or Physician Assistant (PA) will be accountable for ensuring that high quality patient care is always delivered, where they can be a source of continuity for service users by monitoring physical as well as mental health requirements.
The PA will support the effective functioning of the multidisciplinary team and wider service developments through developing knowledge of assessing and managing mental disorders, undertaking quality improvement (QI) work such as clinical audit, gaining knowledge about psychotropic medication and taking a lead role in the physical health of service users.
The PA will have regular supervision (for both clinical and professional supervision) from a designated Consultant, with regular meetings to support on-going development within the role, as well as attending regular continued professional development (CPD) sessions.

About us
In keeping with General Medical Council guidance, the post holder will be responsible for their own practice but working under the supervision of a Consultant Psychiatrist. There will also be regular supervision meetings to support the PA's ongoing development in the role; the level of supervision will be agreed between the post holder and supervising Consultant but will not be less than monthly in frequency.
