An exciting opportunity has arisen for a highly motivated and dynamic Registered Nurse to join our Acute Stroke Response Team. This specialist role is ideal for an experienced nurse who is passionate about delivering rapid, high-quality care and improving outcomes for patients presenting with suspected stroke.
As an autonomous practitioner, you will play a pivotal role in the early identification, triage, and assessment of patients with suspected stroke, ensuring timely intervention and swift progression through the appropriate stroke pathway. Working closely with medical colleagues and the multidisciplinary team, you will coordinate, implement, and evaluate care in line with local and national stroke guidelines.
Based within the Emergency Department, you will be responsible for facilitating the assessment and safe clinical transfer of patients with suspected stroke, promoting a culture of urgency and excellence in stroke care. This is a rewarding opportunity to make a significant impact on patient outcomes while developing specialist skills within a highly regarded stroke service.

Main duties of the job
The successful candidate will:
- Take charge of the ward or department in the absence of the Ward/Department Leader when required.

- Triage and assess patients with suspected stroke, ensuring prompt intervention and adherence to agreed pathways.
- Support medical staff in the assessment and management of emergency patients, arranging and acting upon diagnostic investigations.
- Initiate appropriate treatments and investigations based on clinical assessment.
- Coordinate the safe and timely transfer, admission, or discharge of patients, ensuring a seamless patient journey.
- Collaborate with multidisciplinary teams to deliver high-quality, patient-centred care and improve patient outcomes.

Job summary
Are you an experienced, motivated nurse who thrives in a fast-paced environment and is passionate about delivering exceptional care to deteriorating patients? We are looking for a confident and dynamic Critical Care Outreach Nurse to join our team and make a real difference across the hospital.
This is an exciting opportunity to use your critical care expertise beyond the ICU, supporting the early recognition and management of acutely unwell ward-based patients. You will work with a high degree of autonomy, assessing patients, initiating timely interventions and supporting the multidisciplinary team to prevent deterioration and improve outcomes.
You will be at the heart of our 24/7 Critical Care Outreach Service, providing expert clinical advice, leadership and support across the hospital. Every day will bring new challenges, opportunities to develop your practice and the chance to influence patient care at a critical moment.
Alongside your clinical role, you will inspire and empower colleagues through education, training, coaching and bedside support, helping to build confidence in recognising and responding to the deteriorating patient.
