We are looking for a qualified nurse to join our Staffa and Limes Core Community District Nursing Team, on a 12 month fixed term basis. The post could also involve working across other teams in North East Derbyshire and South Hardwick.

Main duties of the job
Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurse in the management of the health care environment.
Implement / refer on to the relevant services as appropriate.
Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.

We deliver professional internal audit, assurance and consulting service across the NHS in Wales, consisting of 12 Health Boards/Trusts, and a number of Special Health Authorities, operating with a budget in excess of £8bn.
A continuously changing and evolving risk landscape, particularly following the Covid pandemic, means that the NHS in Wales faces new and unfamiliar challenges. Main duties of the job
The Internal Audit Manager role within the Specialist Services Unit provides the opportunity to work across a range of NHS Wales organisations and be responsible for supporting the development and delivery of audits plans and managing a portfolio of project and estates assurance audit assignments, as well as delivering complex high-level reviews of risks, strategies, and governance structures, Quality & Safety Arrangements, financial management and service provision within directorates.

We are seeking a motivated, compassionate, and experienced Midwife to join our Antenatal Clinic team as a Band 6 Antenatal Clinic Midwife. Job summary
An exciting opportunity has arisen for a District Nurse to work within our St Neots/A1 Community Nursing Team.
You will be working as part of a team which includes Community Nurses (RN), Community Nursing Clinical Lead and Health Care Assistants. You will link closely with colleagues outside of CPFT including GPs and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.

The post holder will be supported and be committed to further development within role either with modular post graduate development or completion of the Specialist Practitioner DN Qualification.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. We are looking for an enthusiastic and patient focused Registered Nurse to join our growing Clinical Homecare Nursing Team as a Community IV Nurse.

The Grove Medical Centre was built in 1985 to provide facilities for a comprehensive range of primary health care. It provides care to approximately 15,500 patients.
There are three partners and a clinical team of salaried GPs, Practice Nurses, Health Care Assistants, as well as allied health professionals. There is an administrative and management team, including an Operations Manager and Reception Manager who support the Practice Manager and partners.

We are looking for Community Nurses to work in our Cambridge East and South City Nursing Service. You will be fully supported to gain new skills and experience needed to deliver high quality nursing care to patients in their own home or residential setting.
As a community nurse we will develop your wound care, palliative care, and long-term conditions, knowledge and skills; supporting you to become an expert in assessment and care-planning to deliver quality care that is caring and compassionate.

Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Have experience of working in the community.
Are educated to RGN level.

Can work flexibly and as part of a team.
Have the ability to assess and formulate care plans.
Are able to teach and support student nurses and integrated care workers within your team.
Have enhanced communication and interpersonal skills.
You will be responsible for:
Keeping the patient at the centre of care, ensuring patient and carer participation in decision making.
Providing a high standard of care.
Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required.
