Job summary
Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post holders will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
The post holders will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
Main duties of the job
Act at all times with compassion, to be committed to delivering high-quality care, use effective communication, be competent and have courage when needed.
Participate in the holistic assessment, implementation and evaluation of programmes of care and therapy for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs.
Provide safe medicines management in your area, according to professional and organisational guidelines to ensure safety and efficacy.
Develop and maintain constructive working relationships and liaise effectively with all members of the multidisciplinary team so that patients' needs are met.
Act as a role model in the promotion of person-centred practice, and challenge practice, which is not person-centred, so that a person-centred culture is maintained.
Keep updated with relevant clinical developments and use knowledge to enhance standards of care.
Practice and promote safe and effective skills in all aspects of clinical practice.
Practice and promote safe and effective record keeping in line with trust policies and professional standards.
Demonstrate competence and confidence in clinical practice: this includes all clinical procedures that are relevant/specialist to the area.
Recognises own limitations in the provision of clinical care and urgency of patient's needs, referring to other healthcare professionals accordingly and is accountable for his/her own action.

Main duties of the job
The Digital Clinical Safety Officer (CSO) is responsible for providing leadership and expert advice on clinical safety across the Trust's digital portfolio.
The post holder will ensure that all digital systems, software, clinical applications, medical device integrations and digital transformation programmes are developed, procured, implemented and maintained in accordance with recognised NHS Digital Clinical Safety Standards.
The CSO will work closely with clinical, operational, technical and supplier teams to identify, assess and manage clinical risks associated with digital technology and support the delivery of safe and effective digital healthcare services.
The post holder will act as the Trust's designated Clinical Safety Officer for digital systems and services and will champion a culture in which digital safety is recognised as an integral component of patient safety.
You must be a registered healthcare professional and hold a CSO qualification.

University Hospital Tees - Gastroenterology & Endoscopy Services
The ERCP Specialist Nurse will play a pivotal role in delivering a safe, high-quality, and patient-centred ERCP service across University Hospital Tees. This new post has been created to strengthen the Trust's HPB (Hepato-Pancreato-Biliary) pathway, improve compliance with national BSG ERCP Quality Improvement Programme (QIP) standards, and address key patient safety risks associated with inadequate pre-assessment, stent tracking, and post-procedure follow-up.
Working as an autonomous practitioner within the multidisciplinary HPB team, the post-holder will lead the ERCP pre-assessment service, coordinate inpatient and outpatient pathways, support the consent process, and provide structured post-procedure follow-up. The role will also include responsibility for maintaining the Trust's biliary stent registry, ensuring timely stent exchange or removal, and reducing avoidable complications.
Main duties of the job
The ERCP Specialist Nurse will act as the named lead nurse for ERCP within the Trust, supporting clinical governance, audit, and service improvement. The post-holder will contribute to MDT meetings, liaise with consultants, endoscopy teams, radiology, and surgical colleagues, and ensure that patients receive clear, personalised information throughout their ERCP journey.
This is an excellent opportunity for an experienced endoscopy or gastroenterology nurse to develop advanced skills in a specialist area, supported by a structured training and competency programme aligned to BSG standards. Job summary
Temporary 12-month maternity cover Band 3 administration and clerical post 30 hours per week.
The post holder will work as part of a team providing comprehensive Administration / Co-ordination support to the Paediatric Physiotherapy Hip Scanning Team.
The post holder's work will incorporate direct support for the hip scanning service that is delivered within Paediatric Physiotherapy Team. This will include at times sitting in the clinics completing the administrative tasks and onward correspondence.
This post will involve working across sites supporting clinic at Birmingham Heartlands Hospital, Good Hope Hospital and Solihull Hospital.
This will also involve working in conjunction with colleagues, service users and stakeholders to provide an efficient service.
Main duties of the job
To support the therapists with day to day admin tasks related to the infant hip screening service, this may include sitting in clinic to create letters.
To undertake typing which may include touch type from audio and/or copy type from written documentation which may include letters, emails, reports, and any other correspondence as required.
To exercise initiative and judgement based on acquired knowledge and experience when dealing with enquiries.
Receiving and dealing with telephone and email enquiries as appropriate, from both staff and patients.
To maintain diaries as required, and book/reschedule patients accordingly.
Input and look up data on Trust IT systems in accordance with Trust policy. Extract and input data from several different sources.
Arrange meetings, appointments and support with any internal audits, as necessary.
To assist colleagues as directed by the Supervisor/Line Manager in times of pressure of work.
Co-operate in the introduction of new technology and new working practices to ensure the smooth running of the department.
Photocopying, Scanning, and filing letters and documents as required.

This exciting opportunity will allow an experienced Allied Health Professional to oversee the delivery of recovery-focused interventions in an acute mental health setting.

You will lead an established team of Occupational Therapists, a Band 7 Physiotherapist, and support staff across two adult acute mental health wards, our older people's ward, and our Psychiatric Intensive Care Unit. The team delivers clinically effective, evidence-based treatments and interventions to groups and individuals. You will also work alongside psychologists and art therapists. You will balance team management responsibilities with maintaining a clinical caseload, particularly involving complex cases.

The AHP team works across all wards to support discharge planning, therapeutic activity, and purposeful admissions. There is an ADL kitchen and gym on site, as well as an outdoor games area. The team also facilitates community activity and engagement.
Main duties of the job
You will provide leadership to a therapy team within the multidisciplinary team context. You will have a high degree of autonomy, with a key role in supporting the development of skills of staff across the inpatient wards. You will also be contributing to service and policy development within the service and reporting on team governance.
Ensure a responsive, high-quality service is provided to patients, their families and carers within the clinical environment and maintain compliance with key performance indicators.
Provide line management supervision to the AHP team including recruitment, development and supervision.
Provide specialist clinical input across the service including support for complex cases. You will work with adult inpatients and older people who have organic (Dementia) frailty and functional presentations such as psychosis, anxiety and mood disorders. You will have the opportunity to use your physical health training alongside the ability to communicate and engage with service users living with complex mental and physical health needs including frailty and work to tackle health inequalities.
Role model and foster strong working relationships between the AHP and the MDT, to ensure that care is integrated and results in the best possible outcomes for service users.
Oversee the therapeutic timetables for the wards and coordinate the allocation of activity worker resource across the site.

We are a busy GP training practice with 7700 patients, 3 partners and 6 salaried GPs.

Box Surgery is an established and respected 3 Partner Practice which explores and delivers comprehensive integrated healthcare. The practice is considered progressive and encourages people to take responsibility for their own health and wellbeing, whilst treating those who become ill with care, compassion, empathy and skill.

All Age Eating Disorder Service- East (covering Nelson, Burnley, Blackburn, Ribble Valley and surrounding area).
Would you like to be part of a service which is innovative, forward-thinking and committed to enabling people who are experiencing eating disorders to recover and discover their potential?
We are seeking to appoint someone with a caring, compassionate nature, who is innovative, creative, flexible and highly motivated. Primarily you will undertake specialist assessment, therapeutic work and physical health care for people presenting with an eating disorder within the East locality. You should be an excellent communicator and team player, with the successful applicant joining a dynamic multi-disciplinary team of professionals to provide assessment and treatment to children and adult service users and their families/carers. The team includes a range of other psychological professionals, nurses, dietitians and a consultant clinical psychologist/clinical lead.
Main duties of the job
Existing experience in eating disorders across the age range is desirable but not essential. However, a keen interest to develop knowledge and skills in this field and a strong desire to help service users and families is vital. The successful candidate will benefit from regular clinical supervision as well as professional supervision, with encouragement to engage with CPD activities as appropriate.

Job summary
To provide an essential role in supporting individuals with neuromuscular conditions Pan BCUHB. The post holder is expected to successfully co-ordinate the health and social care needs, provide support and information to families and ensure a seamless transition from child to adult services, where applicable.

To support the neuromuscular network in the planning and provision of neuromuscular services, providing a regional planning role for these services
Main duties of the job
1. The post holder will be responsible for engaging in the development and implementation of care pathways, protocols and communication strategy with colleagues from the public, private and voluntary sectors across Wales
2. The post holder will co-ordinate the specialist advice and support in the stages immediately following the outcome from the Newborn Screening process and following the diagnosis of a neuromuscular condition.
3. The post holder will provide awareness raising sessions for staff groups involved in providing health and social care services and support to those living with a neuromuscular condition and their carers.
4. The post holder will attend multidisciplinary clinics with the lead clinicians to provide expert advice and support to families, carers and children or young people living with a neuromuscular condition.
5. The post holder will develop self and the advisory post in line with the recommendations from the Thomas Report (2010) and the recommendations from the Wales Neurosciences review.
6. The post holder will be responsible for developing and maintaining an up to date database of local resources that can meet the needs of people with neuromuscular conditions and their families.
7. The post holder is expected to build and maintain links with local professionals and voluntary services and develop expert knowledge of other resources that can provide help and support with e.g. benefits and bereavement services.

Main duties of the job

Main duties of the job
The successful applicants will work within Sefton CAMHS, Venus or Parenting 2000 as part of a multi-disciplinary teams delivering, under supervision, high-quality, outcome informed, focused and evidence-based interventions for children and young people in accordance with the competency framework for their therapeutic modality.
Applicants need to be able to start the Post Graduate training programme taught academic dates in January 2027 and commit to attendance at all university based taught and self-study days required by Manchester University.
You will work in the service for the remaining days of the week using your newly developed skills to ensure you gain the necessary clinical practice hours to fulfil the academic programme. This is 200 hours of modality and case specific practice for the full time courses.

Alder Hey Children's Hospital in partnership with Venus are excited to offer three CBT IAPT training places for 2027/2028. One post will be hosted by Sefton CAMHS, one will be with Venus and another will be with Parenting 2000. This will enable the successful applicants to work with children in a variety of community settings with support from their colleagues to identify young people with the level of needs suited to the requirements of the academic programme.

This is a rewarding but demanding post that requires committed and dedicated nurses to provide quality care to infants, children, young people, and their families/carers from a diverse population. We offer all of our Band 5 Staff Nurses the opportunity to undertake a nurse-in-charge programme, so if you are an experienced nurse looking for more responsibility then we will positively support your development.
The Paediatric A&E Department at Hillingdon Hospital is a separate unit based within the main A&E Department. On average, 25% percent of our emergency attendances are infants, children, and young people. We endeavour to provide the highest possible level of care to them at all times. The department has close links with the paediatric wards, specialist nurses, and our community nursing team.
Consideration will be given to newly-qualified nurses, as we offer a comprehensive preceptorship programme and an opportunity to undertake a rotation on the children's inpatient ward and neonatal unit, if desired.

Main duties of the job
- Under the supervision of senior colleagues be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care.
- Act as an associate mentor to student nurses in training and mentor once Mentorship in Practice course completed.
- Under the supervision of senior colleagues facilitate the professional and clinical development of paediatric practice through the application of research.
- Under the supervision of senior colleagues provide clinical support to the infants', children's and young people's Emergency Department.
- Ensure that the communication of issues relating to the child's care is communicated in a manner that is appropriate and understandable to both the child and family.
- To ensure that patient documentation is accurate and legible and is aware of the legal implication of these documents.
- To be able to assess child's individual needs and have the ability to recognise a sick child and initiate urgent when necessary.
- Support senior colleagues with the introduction and implementation of new ideas and methods of nursing care.
A passion for children's emergency care, with proven communication/assessment skills, enthusiasm, and the ability to be flexible are essential for this role. In exchange, we train all of our nurses in Manchester Triage, venepuncture, cannulation, and plaster of paris application. Those wishing to progress further will be able to apply for their APLS/EPLS certification.

We have an exciting career development opportunity for a Paediatric Junior Sister/Charge Nurse to help lead and manage our growing designated paediatric emergency service. We are a self-contained unit situated within our main A&E Department. We are looking for an experienced enthusiastic & dedicated emergency Nurse to ensure staff develop a patient-centered approach that provides clinically effective and safe care.
This is a great opportunity to lead the way training is delivered to new and existing staff, whilst working in partnership with the wider team and other practice educators to improve practice, clinical nursing and service delivery.

About us
The department is classified as a trauma unit and we are supported by specialty services such as; critical care, acute medicine, specialist surgery and paediatric specialists. We have a purpose built helipad onsite.
We have a new, state of the art staff facility for staff changing with access to a shower as well as a new staff room and education room.
The department has its own staff retreat for quiet / reflection and the department supports time out in here.
The department also runs wellbeing initiatives and supports staff wellbeing.
