Job summary
The Older Adult Community Mental Health Teams are multi-disciplinary teams that offer assessment, treatment and care to Older adults who experience complex mental health difficulties including Dementia
The Clinical Lead in collaboration with the CMHT Manager will develop and coordinate a multi-professional approach providing evidence based mental health care to all those who use the service
To support operational management, and deliver clinical leadership and supervision to designated teams of staff.
To support the on-going day-to-day management of a multi- disciplinary team. To liaise with external/internal services including primary care to ensure that services are coordinated and that pathways into and out of the service are managed effectively and efficiently.
Main duties of the job
This is a leadership role and the post holder will work closely with the Team Manager, Operational Manager and Locality Matron to achieve positive outcomes in the teams' clinical management to:
Providing day to day clinical leadership ensuring the highest standard of care is maintained
- Identify the skill mix required for staff recruitment and support this process
- Audit case notes and care plans against approved audit tool.
- Support the planning, delivery and evaluation of interventions and treatments where there are complex issues.
- Support the learning outcomes as part of the PSIRF process
- Provide specialist knowledge and advice to the team and partnership agencies relating to mental health services.
- Undertake Care Co-ordination/ Key Worker responsibilities for a designated reduced case load of individuals

Job summary
An exciting opportunity has arisen for an enthusiastic and highly motivated individual to join the Acute Oncology Team, within the Medical Division of Wirral University Teaching Hospital NHS Trust. The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.
The post-holder will take an active role in contributing towards the development andImplementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive i.e. coordinating health and well-being clinics to enable independence and support self-management as appropriate.
The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs.

We are seeking an enthusiastic and experienced nurse to join our Paediatric Infectious Diseases and HIV CNS team on a fixed-term basis to provide maternity leave cover.
This is an exciting opportunity to deliver specialist nursing care to children, young people and their families affected by infectious diseases and HIV. The successful candidate will work as part of a multidisciplinary team, providing expert clinical support, care coordination, patient education, and advocacy across inpatient and outpatient settings.
Applicants should be a registered children's nurse with significant paediatric experience. Previous experience in infectious diseases, HIV, or a specialist nursing role is desirable. Excellent communication, organisational, and leadership skills are essential.

Main duties of the job
- Provide specialist nursing care and advice on all matters relating to the prevention, diagnosis and management of Paediatric Infectious diseases across Children's Services, young adult HIV transition services, Maternity and Neonatal services and adult Hepatology services.
- Demonstrate expert knowledge as a visible, accessible resource for children, families and professional colleagues both within and outside the trust
- Work with colleagues to ensure a consistent approach to Paediatric ID nursing across all areas of responsibility, through teaching, audit, collaborative working and the development of shared procedures and guidelines
- Manage various patient caseloads within Paediatric ID, including organising and following-up on clinical investigations; developing individual care-plans; supporting treatment adherence; facilitating multi-disciplinary and multi-agency collaboration; co-ordinating admissions and outpatient appointments; supporting discharge and communicating information to young people and families
- Work on a one-to-one basis with individual children and young people, to obtain consent for investigations, provide age-appropriate information about diagnosis, treatment and prevention of onward transmission; to promote engagement with and adherence to treatment; to identify safeguarding concerns and to prepare young people for transition to adult care

We are looking to recruit a Practice Nurse, 24 hours per week over 3 days, to join our friendly and supportive team
To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. Main duties of the job
To assess, plan and implement and evaluate evidence-based nursing care for people with complex needs for their Diabetes management. The post holder is required to provide expert clinical care for patients and to support other staff to develop their competencies in diabetes management. The post holder will be involved in teaching and training both formally to clinical staff and to patients.The post holder will be required to work collaboratively across primary and community care, and link to other specialists within the acute setting to implement the integrated care pathway for people with diabetes. The post holder will lead on audit, Service Development and Qualityimprovement work.
