The key principles of Wye Valley Trust are to improve the health and well-being of the people we serve in Herefordshire and surrounding areas. We have an exciting opportunity for the right candidate to join our wonderful District Nursing Team.
The Ross-On-wye District Nursing Team are a very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes.
We work closely with Golden Valley DN Team and often work as a locality where needed.
We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. Ross DN Team also provide a wound clinic service based at the Community Hospital.
Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, lone working and the ability to meet the travel requirements of the post are essential.
Main duties of this role include, but are not limited to;
- Palliative care
- Wound care
- Managing end of life symptoms and providing support for relatives/carers- Managing long term conditions in a community environment- CVAD and drain management- Chemotherapy disconnect- Holistic assessments- Post operative care following discharge- Wound clinic service- Venous blood samples

The Alexandra Hospital provides a varied theatre environment with a range of surgical specialities, offering excellent opportunities to gain experience and develop clinical skills within a busy and supportive multidisciplinary team. Newly qualified practitioners will be supported through a structured preceptorship and development programme. Experienced practitioners will have opportunities to further develop their skills and contribute to maintaining high standards of patient care. Main duties of the job
As a Band 5 Anaesthetic and Recovery Practitioner, you will provide safe, effective, and compassionate perioperative care to patients undergoing surgical procedures. You will assist in the preparation and delivery of anaesthesia, working under the supervision of senior practitioners and anaesthetists, ensuring equipment, medication, and the clinical environment are prepared and checked in line with Trust policies.
You will deliver high-quality post-anaesthetic care in the recovery area, monitoring patients physiological condition, recognising and responding to changes in their condition, and escalating concerns appropriately. You will support patients through recovery, providing reassurance and maintaining dignity and comfort at all times.
You will contribute to maintaining a safe theatre environment, adhering to infection prevention, health and safety, and safeguarding policies. Accurate documentation and effective communication with the multidisciplinary team will be essential to ensure continuity of care.
The post holder must demonstrate the ability to work flexibly across the countywide theatres service in accordance with service demands.

Job Purpose

Theatre Practice Prepare the theatre environment, equipment, and instruments for surgical procedures, ensuring compliance with infection control and safety standards. Assist the surgical and anaesthetic teams during operations by performing scrub, circulating, or anaesthetic practitioner duties as required. Monitor patients condition throughout the perioperative period, responding promptly to any changes or concerns. Maintain accurate and detailed patient records and documentation in line with legal, professional, and Trust requirements. Ensure safe handling and management of surgical instruments, theatre equipment, and consumables. Participate in the safe management and administration of medicines and controlled drugs as per Trust policy.
Management Support team leaders in managing staff performance and teamwork within the theatre team. Report incidents, accidents, and near misses promptly through the appropriate channels. Macmillan Renton Unit is an outpatient unit, providing services such as Haematology, Oncology. Gynaecology, Breast Clinics, Urology and Systemic Anti Cancer Treatment (SACT) to patients within Herefordshire and surrounding areas.
We are a satellite site for Cheltenham hospital, which means we are able to treat our patients closer to their homes.
The hours worked are flexible by negotiation, agreed to suit the service and the applicant to ensure a good work life balance.

The East District Nursing team supports patients registered within the East Primary Care Network. We have formed excellent working relationships with our GP colleagues and the wider multi-disciplinary areas.

The focus of District Nursing is to support patients towards self-management, enabling them to continue to live as full and productive a life as possible, through education, equipment and or lifestyle changes. We also have the privilege of supporting people and their families at the end of their lives, working in close liaison with the Hospice Teams and Specialist Palliative Care Nurses.
As community teams we enjoy working with many other health and social care professionals, Care Homes and Care Agencies. All staff are expected to record their work electronically on EMIS, our electronic patient record system which links into the GP services to allow a seamless service provision. Training and on-going support will be given to support staff with EMIS and other electronic systems they will need to use.

The Ross-On-wye District Nursing Team are a very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes.
We work closely with Golden Valley DN Team and often work as a locality where needed.
We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. Ross DN Team also provide a wound clinic service based at the Community Hospital.
Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, lone working and the ability to meet the travel requirements of the post are essential.

Job summary
Our School Nursing team is seeking a Registered Nurse to support the delivery of the Healthy Child Programme 5-19 years to the children, young people and families of Herefordshire.
You will work as a key member of the School Nursing team and you will gain the experience of working in the community, in partnership with children, young people, families, education and other partner agencies.
You must have a strong public health interest and be fully committed to improving the health and wellbeing of our children, young people and their families.
Within School Nursing there are lots of opportunities for development and there is a robust support package available, which includes clinical, restorative and safeguarding supervision.
Applicants must have a full driving license and access to a car for work purposes.
Main duties of the job
- To promote and facilitate the physical, mental and emotional health of children, young people (5-19 years school age children and young people) and families.
- To work in partnership with all service users, members of the multidisciplinary team both internal and external to the Trust and ensure continuity of care to children, young people and their families.
- Work within the 5-19 pathway to deliver planned programmes of care under the direction of the School Nurse. This will involve onward referrals as required.
- Act as a practice supervisor/ assessor for pre-registration student nurses
- To assist the Duty Clinician in the duty desk with the administration of incoming clinical correspondence and allocation of referrals on a rotational basis.
- Review the A+E attendances that come into the service and follow up with parents/carers any A+E attendances and give advice as per the School Nursing standard. Refer any children that are enhanced and attended A+E to the named School Nurse.
- To actively support the appraisals for staff in band 4 and below.
- To assist in the recruitment and selection of staff into the team and to participate in the induction and orientation programmes for new staff members, this may include preceptorship support for newly qualified staff nurses.
