This is for a 37.5 hour permanent post
Main duties of the job
Whilst community experience would be an advantage we are keen to recruit those who have relevant transferable skills and experience with long term condition management, who are enthusiastic to develop their career in the community setting.
You will be required to work shifts across the 7 days. Access to a car for work purposes is essential.
The post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable. You will have a sound knowledge of recent changes within the NHS and current professional issues. You will have an understanding of frailty and long term condition management. You will be working in a lone capacity under the direction of the Clinical team leader/ Caseload manager and in conjunction with other team members, ensuring clinical standards are maintained using evidence based practice.

Main duties of the job
An opportunity has arisen for a part time Health Visitor to work in the 0-19+ Service working for Wirral Community Health and Care NHS Foundation Trust.
The post requires good communication and interpersonal skills and the ability to be adaptable, reliable, compassionate and approachable. You will need to have strong interpersonal and leadership skills, be flexible solution focused and support service development.

Main duties of the job
The successful candidate will have proven leadership, experience and knowledge of school nursing. We have an exciting opportunity for a Registered Nurse to join our Cheshire East North 0-19+ team as a part time Public Health Nurse, supporting the Health Visitor North team covering Macclesfield, Wilmslow, Knutsford, Poynton and Disley. Initially the position is based in Macclesfield.
The work involves working with families ,children and young people and delivering the Healthy Child Programme core offer. The role supports the Health Visiting service working with the team to carryout the Healthy Child Programme, via home visits, clinics and group work in the family Hubs. The role gives a great insight to our service and has supported employees to develop into other roles associated with public health and 0-19 services. We have great feedback from students and staff who enjoy working within a friendly and supportive team.
Main duties of the job
Supporting the 0-19+ service to deliver the 0-19+Public Health Nurse offer.
Managing your own caseload allocated by Health Visitor
Completing developmental clinics as per the Healthy Child Programme and drop in clinics. Facilitation of group work working closely with the family hubs in Macclesfield. Escalation of concerns to Health visitor. Providing supervision to CHPA and being assessor/supervisor for student nurses or trainee staff.
Completion of home contacts, working with SEND children and supporting families, working with CIN/CPP AND Cared for children with oversight from the Health Visitor.

The Cheshire East 0-19+ service is a friendly supportive team offering staff the opportunity to work flexibly if needed.
We work with the trust values of compassion openness and Trust ensuring that we demonstrate these values to children and families in Cheshire East North Team and to each other.
We have weekly safety huddles to support staff at the beginning of each day and monthly team meetings. Each member of staff has Clinical and Management supervision every 12 weeks or more often if required.
Supervision is available to all staff both planned and reactive.
We work closely with our specialist Safeguarding nurses to ensure staff feel supported and have access to appropriate training and induction

Job summary
We are recruiting to a team of dedicated community nursing staff to work with the Ageing Well and Frailty Service and our Primary Care Networks. This exciting opportunity means that you'll be working as part of a combined team of community care staff and PCN colleagues to proactively support people with moderate and severe frailty as part of WCHC's Population Health Management and NHS Proactive Care model.
You will be based within the Ageing Well and Frailty Service service working 22.5 hours per week over 3 days.
This is a permanent post
Main duties of the job
This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable.
You will be working with the patients, their family/carers and the wider team (including PCN colleagues) to create outcome goals for the patient to achieve during their time actively within the service, and lead on the development, implementation and monitoring of the patient specific care plan to ensure thesegoals are achieved.You will be working in both lone capacity and in conjunction with other team members, ensuring clinical standards are maintained using evidence based practice. You will have a good knowledge of the components of Clinical Governance, Best Practice, Risk Management, Clinical Audit and relevant Professional Codes. You will be able to evidence excellent clinical skills.
