Job summary
The Adult Community Nursing provides nursing care provision and support to Hounslow locality 24 hours, 7 days a week. The service covers different localities including Heart of Hounslow, Bedfont and Feltham, Brentford and Chiswick and the night service from 7pm-7am based at Clayponds Hospital.
Main duties of the job
To work with the locality nursing team leadership in the delivery of the locality nursing service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working.
To support the locality nursing team leadership in the management of the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.
To work in partnership with the primary health care team, social services and the wider multi-disciplinary locality team, providing holistic assessment of health needs and the provision of nursing care to housebound people with a range of medical needs and long-term conditions.

Job summary
The Hounslow Health Visiting Service is seeking a Band 5 Community Staff Nurse to support the delivery of the Healthy Child Programme. The successful applicant will work within a skill mix team to support and assist in the development, implementation and evaluation of activities aimed at promoting the health of the local population, as well as offering support to identified families as delegated by a Health Visitor. The post is suitable for those wanting to develop the skills and knowledge to prepare for a Health Visiting career.
Main duties of the job
The Community Staff Nurse will work in a skill mix Health Visiting team supporting the delivery of the Healthy Child Programme, including
- Health Review contacts
- New birth reviews
- Maternal mood assessment and listening visits.
- Transfer in Contacts
- Individual, group and local public health activities
- Follow up on DNA's
- Delegated Home visits
- Infant feeding support
- Accident prevention and follow after A/E attendances
o To support Duty Health Visitor role
o To hold a small caseload of families to deliver delegated packages of care e.g. LAC children, children with SEND needs, premature infants , children with long-term healthcare conditions, listening visits. To report back to named HV and seek regular caseload supervision with the named HV.

Job summary
An exciting opportunity for a fixed term contract ( 1 year initially with the potential to extend) has become available for a Community Mental Health Practitioner to work in Ealing Acton MINT. You will have the opportunity to work in a multidisciplinary Team as well as having an autonomy to manage a caseload of patients with varied mental health conditions.

The ideal candidate will have:
Prior experience of working across a range of working with a range of mental health diagnoses/need
Hold a registration for the correct governing body such as the Nursing and Midwifery Council, Social Work England or the Health & Care Professions Council
Knowledge of and experience in delivering a range of brief interventions to patients with mental health needs;
Good written and verbal communication skills and the ability to use these to strengthen local relationships with the local MINT Teams

Main duties of the job
The Hounslow Community Neuro Rehab Team (CNRT) is a client-centred, integrated multi-disciplinary team comprising of an Advanced Clinical Practitioner, Physiotherapists, Occupational Therapists, a Neuro Psychologist, a Pharmacist, a Specialist Nurse practitioner, an Occupational Therapy Assistant and Rehab assistants.
The Neuro-rehab team works across both intensive short-term rehab and longer-term rehab pathways. There are opportunities to develop skills in rapid assessment and facilitating discharge from hospital, long term condition management and specialist neuro-rehab.
This role provides the opportunity to work across both rehabilitation pathways within the service, with both Occupational Therapists and Physiotherapists. This role involves providing therapy for patients within their own home environments as well as the local community. There will be expectations for lone working and independent monitoring of patients progress with timely feedback to the MDT.

They will provide unit management support and coordinate patient flow (bed management) outside of regular hours. Additionally, the Unit Coordinator will offer senior nursing support, professional leadership, and guidance in all clinical areas within the unit, available 24 hours a day, 7 days a week.

Main duties of the job
To provide high quality, skilled nursing care and to assist the Community Nursing Team Manager in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.
Deliver high quality nursing care to housebound patients with complex needs to enable them to remain in their own homes. Working closely with out of hours service providers, specialist nurses, allied health and social care professionals to meet the needs of the patients without unnecessary attendance in A&E or be admitted to hospital.
This will involve working independently but as part of the Community Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care. Candidate will be expected to be flexible to support corporate approach to patient care as and when required.

Main duties of the job

The Band 6 ECT/rTMS Nurse is a senior post.
