Job summary
The Lead Nursewill provide expert autonomous clinical practice whilst leading the strategic development, implementation and evaluation of an integrated Continence, Catheter and Intermittent Self-Catheterisation (ISC) Service across outpatient, community and interface services. The post holder will lead the redesign of bladder dysfunction pathways, ensuring equitable access to high-quality continence care for adults with complex lower urinary tract dysfunction, including patients requiring intermittent self-catheterisation following neurological injury, Cauda Equina Syndrome, spinal pathology, chronic urinary retention and other urological conditions. Working at an advanced level of clinical practice across the four pillars of Advanced Clinical Practice (Clinical Practice, Leadership & Management, Education and Research), the ACP will work autonomously to assess, diagnose, investigate, treat and manage patients whilst leading service transformation across the organisation. The role will provide expert leadership for catheter care, continence management, bladder rehabilitation and patient education, ensuring compliance with national guidance, NHS Long Term Plan priorities and GIRFT recommendations.

Job summary
An exciting opportunity has come up for an enthusiastic, competent and ambitious person to join the Thoracic Surgery Team as our Thoracic Cancer Pathway Co-ordinator and Schedular.
We are recruiting for a Cancer Patient Navigator who shares our ambition and vision to deliver the best patient focused care and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.
To support the implementation of the NHS Cancer plan by providing dedicated and effective support to multi-disciplinary teams, working with team members to facilitate the delivery of high quality care for cancer patients.

Main duties of the job
To navigate and support patients through the cancer pathway using own judgement and initiative, utilising specific departmental and patient administration systems, ensuring strict adherence and compliance to all Trust Policies and procedures.
To co-ordinate appropriate investigations required, signpost patients to appropriate support and assist with 'Making every Contact Count' to ensure the delivery of patient care is of the highest standard and those patients and their families are treated with dignity and respect.
To utilise iPM/iSOFT/CRIS and other systems/databases to track patient and upload relevant cancer data.

To help maintain existing databases of patients coming through the pathway.

Job summary
1.To act as a specialist in knowledge in MND (motor neurone disease) and MND patients research participation as well as advise on the complex nursing care of MND patients when participating in MND research. To proactively support MND patients and their carers and health colleagues across the South West region to promote inclusion of MND patients into research across the South West region.
2. Proactively lead and develop an MND person-centred research service as part of an MND research nurse network. Working alongside patients and carers as well as stakeholders across the South West region.
3. To ensure that everyone diagnosed with MND in the South West region has access to the support and opportunity to take part in research across the region, working across organisations and linking closely with complex care services and research delivery teams and research leads.
4. To support people with MND to navigate the research process, providing information about current MND research opportunities, identifying research that people living with MND are eligible for, explaining what is involved, and supporting people with their application and ongoing involvement in trials--while ensuring their complex care needs are recognised and supported throughout.
Main duties of the job
- Contribute to the progress of MND research and clinical trials, dedicating 100% of efforts to supporting, delivering, and leading initiatives that contribute to breakthroughs in MND care and understanding.
- Collaborate effectively with the MND Association to support the strategic development and growth of the Research Nurse Network in the South West region.
- Oversee and support clinical trials across the region and research efforts aimed at improving outcomes for people living with MND, playing a pivotal role in facilitating high-quality research and trial execution

This is an exciting opportunity to work within the UGI Clinical Nurse Specialist team. Under direction from the Lead UGI Cancer Nurse Specialist you will assess, plan, implement and evaluate expert nursing care and provide support to UGI and GIST cancer patients and their carers. Job responsibilities
-
Providing and delivering a high standard of nursing care by initiating and coordinating the assessment, planning, implementing and evaluating of individual patients needs based on evidence. This includes all aspects of care from admission to discharge.
Develop key skills in patient management which include: - the setting up and calibration and use of critical care equipment, including ventilators, dialysis machine, intra-aortic balloon pumps, pacing devices and advanced haemodynamic monitoring. Repatriation of level 1-3 patients within the trust and outside. Near patient testing e.g., blood analysis, TEG
(thrombelastograph analysing) and urine.
Reporting and documenting all relevant nursing/patient needs, communicating changes with other health care professionals in the multi-disciplinary team with ongoing responsibility for patient care as they occur.
Engaging and interacting with carers and family, encouraging them to be actively involved in patients care planning as outlined in ward / unit philosophy.
Promoting and delivering health education within the clinical area and advise
patients family and/or significant others in health promotion issues.
Demonstrating and undertaking evidence-based practice playing an active part in the introduction of this evidence to improve patient centred care.
Assisting other clinical areas within the trust if the clinical situation or staffing levels demand.
Supervising and directing Health Care Assistants with their daily workload.
Administer prescribed medications/fluids safely and effectively according to trust policy.
Communicate with other areas/disciplines ensuring the sensitive and complex information regarding the patient is given appropriately with the confines of the data protection act and patient confidentiality.
Communicating with patients, relatives and outside agencies as appropriate within the confines of the Data Protection Act and Patient Confidentiality.
