We have a rare and exciting opportunity to join our Tobacco Dependency in Pregnancy team. We are seeking an enthusiastic and confident advisor to support pregnant people and their families to stop smoking; contributing to important work to reduce smoking and tackle health inequalities for the population of Tameside and Glossop.
The successful candidate will work closely with the Lead Midwife for Tobacco Dependence and other team members to promote and support Smoke Free Pregnancy in line with regional and national best practice.
Using a flexible and innovative approach you will provide smoking cessation advice and support to service users in both hospital and community settings via face to face and telephone contacts, and should have excellent communication and interpersonal skills to fulfil this role. You must be able to demonstrate the ability to engage with people and communities to motivate and support change in behaviour.
In this role you will be expected to work well within a team but also independently and without direct supervision. You should be well-organised and able to manage and prioritise your work to meet the needs of the service and the people you support.
Main duties of the job
The post holder will work with the Lead Midwife for Tobacco Dependence and other team members to provide individualised stop smoking support to women and families in a flexible and innovative way. This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking. It focuses on engaging with people in communities and offering them practical support to change their behaviour to achieve their own choices and goals.
The post holder will play a key role in the collecting, collating and reporting of Smokefree Pregnancy data. The post holder will be required to undertake other duties commensurate with the grade as directed.

Team Leader, Children's Community Nursing Team: Permanent (37.5 hours)
The Team Leader for the Children's Community Nursing Team (CCNT) is responsible for providing strategic, clinical, educational and operational leadership to the service, ensuring the delivery of safe, effective, high-quality, child and family-centred care. The post holder will lead and manage the Children's Community Nursing Team, making effective use of resources to meet service demands whilst maintaining the highest standards of patient care and professional practice.
Main duties of the job
As a key member of the Children's Services senior leadership team, the post holder will support the implementation of national policy, local priorities and Trust objectives relating to children's community health services. They will ensure that all staff work within the professional standards, codes of conduct and scope of practice defined by the Nursing and Midwifery Council (NMC).
Working collaboratively with colleagues across primary, secondary and tertiary care, the post holder will lead the delivery of community-based services for children and young people with acute, complex and chronic health care needs. They will be responsible for overseeing the coordination of complex discharge planning processes, ensuring seamless transitions from hospital to home and promoting integrated, multidisciplinary care pathways.
The Team Leader will provide expert clinical guidance, professional leadership and educational support to the nursing workforce, fostering a positive culture of learning, development and continuous improvement. They will champion evidence-based practice, quality improvement and innovation, identifying opportunities to develop the service in line with current research, best practice and the evolving needs of children, young people and their families.
Working autonomously within established Clinical Governance frameworks, the post holder will demonstrate advanced clinical expertise, sound decision-making and exceptional communication skills.

This is an exciting opportunity to join a developing GP-led model that supports people to receive treatment closer to home. Flexible bank opportunities are available across bed-based, community and virtual pathways.
The Stamford Unit is a 96-bed Intermediate Care facility providing rehabilitation, end-of-life care, step-up admissions and complex discharge support. You will provide medical care for patients, undertake ward rounds and work alongside ACPs, Nurses, Therapists and Social Workers to deliver coordinated care.
Our Hospital at Home service provides hospital-level care in patients' homes through remote monitoring, telemedicine and multidisciplinary support. Working with Urgent Community Response teams, advanced practitioners and specialist services, you will undertake clinical assessments, support admission avoidance and early discharge, and contribute to personalised care planning.
This varied role combines community-based acute care and digital healthcare, offering opportunities to develop interests in frailty, older people's medicine, palliative care and telemedicine while helping deliver innovative models of care across Tameside and Glossop.
Main duties of the job
- Provide clinical leadership across Intermediate Care, Hospital at Home and Virtual Ward services, ensuring safe, effective and high-quality care.
- Undertake assessment, diagnosis, treatment planning and ongoing management across bed-based, community and virtual pathways.
- Work with multidisciplinary teams to deliver coordinated, person-centred care and support treatment, escalation and discharge planning.
- Deliver clinical care through ward rounds, home visits and telemedicine consultations.
- Support admission avoidance, early discharge and care closer to home through Hospital at Home and Virtual Ward pathways.
- Manage patients with frailty, rehabilitation, chronic disease, palliative and acute care needs in line with best practice.
- Work closely with primary care, community services, care homes and system partners to ensure seamless care.
- Contribute to service development, pathway improvement, innovation and the promotion of best practice.

The £20m refurbishment of our Urgent and Emergency Care has been completed, so there has never been a more exciting time to join the Urgent and Emergency Care team, influencing and shaping the both the Emergency Care our patients receive and the environment our staff work in.
The new state of the art Emergency Department includes:
- 8 bedded resuscitation area
- 29 majors' cubicles
- 2 mental health assessment rooms
- Rapid assessment and treatment area
- 8 cubicles within the Children and Young Peoples Emergency Department
Main duties of the job
- To provide clinical expertise to the departmental team, acting as a role model in the deliveryof patient care.
- To support the Team leader in the effective management of the clinical area
- To always act in a manner consistent with the NMC Code of Professional Conduct, carrying out his/her duties in accordance with Trust policies and procedures.

Quality Governance Lead - 12 months fixed term
An exciting opportunity has arisen within the Corporate Nursing and Integrated Governance Unit for an innovative and skilled Quality Governance Lead.
You will require commitment, passion and motivation in order to fulfil the requirements of this role, in enabling corporate and divisional compliancewith the national requirements and standards set by commissioners, Care quality commission, NHS England and other regulators. The post holder will provide Quality Governance support across the organisation, with a quality improvement, safety, compliance and assurance focus.
You will need excellent organisational skills, as well as communication and interpersonal skills to enable you to work with varying levels of managers, clinicians and administrative colleagues. Main duties of the job
The post holder will be one of the team of senior staff and leaders in the Nursing and Integrated Governance Team who will collectively have theresponsibility for enabling corporate and divisional compliance with the national requirements and standards set by the Commissioners, CareQuality Commission, NHS England/Improvement and other regulators. The post holder will provide Quality Governance support across theorganisation. The role has a quality improvement, safety, compliance and assurance focus. The post holder will:
o Provide leadership and professional advice to managers and staff within the Trust on governance and assurance processes.o Support the delivery of the Trust and Divisional assurance, quality, safety, and governance strategies and agendas which underpin the delivery of the corporate objectives and Trust response to the NHS Long Term Plan.o Be responsible for leading the development, and maintenance of the Divisional Quality Assurance Framework with the senior management team and ensure alignment and compliance with the Trust's overarching policies relating to Integrated Governance Quality Assurance Framework.o With Divisional Directors and Directorate Managers, Senior Nurses and Clinical Leads and the Associate Director of Quality Governance ensure that systems are in place to provide assurances, insight foresight and oversight in respect of quality and safety for staff, patients and service users across spheres of responsibility.
