Job summary
We are looking for a compassionate and motivated Nurse Associate to join our Primary Care team within a GP practice environment. This is an exciting opportunity to work with a diverse patient population, supporting the management of long-term conditions, delivering preventative healthcare and carrying out a range of clinical interventions. You will work closely with GPs, Practice Nurses, Healthcare Assistants and other health and social care professionals, helping to provide high-quality, patient-centred care. The role offers excellent support and supervision, with a strong commitment to ongoing professional development and progression, including support for Nursing Associates who aspire to become Registered General Nurses (RGNs).
Main duties of the job
KEY RESPONSIBILITIES
Deliver a range of clinical interventions including health checks, chronic disease monitoring, venepuncture, ECGs, immunisations, wound care and patient assessments to support high-quality patient care.
Promote health and wellbeing by supporting patients to manage long-term conditions, encouraging participation in screening programmes and providing advice to improve health outcomes.
Work collaboratively within a multidisciplinary team, maintaining accurate clinical records, supporting care planning, contributing to service improvements and ensuring safe, effective and compassionate care.
KEY REQUIREMENTS
Registered Nurse Associate on the NMC register with a Nursing Associate Foundation Degree qualification.
Ability to deliver safe clinical care, including patient assessments, long-term condition monitoring and health promotion activities within scope of practice.
Excellent communication, organisational and teamwork skills, with the ability to use clinical systems, prioritise workload and maintain accurate patient records.

Job summary
ABOUT THE ROLE
As a Practice Nurse, you will be responsible for the delivery of practice nursing services care to the practice population. Supported by the Lead Nurse and General Practitioners within the practice, you will deliver care within the boundaries of your scope of practice, focusing upon supporting patients to be healthy, monitoring and management of long-term conditions, health prevention, wound care and screening activities that may include cervical screening, baby immunisations and travel health. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The working pattern for this role is Thursdays and Fridays, with a 7:30am start on one of those days.
The rate of pay is dependent on the following competencies
Including thorough understanding of QOF requirements / enhanced services / contribute to audits / governance structures / leaning from incidents / active participant in nursing and clinical meetings
Undertake assessment and interventions (ear irrigation)
Understanding of screening programmes, practice uptake and contribute to maximising uptake in practice population
Cervical screening: Undertake procedure, follow practice protocol, interpret results, audit results
Understand national schedules, administer from PGD or PSD as appropriate, ability to fully meet patients needs for information and guidance

We are looking for a compassionate and experienced Advanced Nurse Practitioner to join our Home Visiting Service at Ashfield North PCN. This is a rewarding opportunity to make a real difference by providing timely, high-quality care to patients who are unable to attend GP practice settings across four practices in and around Sutton-in-Ashfield, serving a population of around 52,000.
In this role, you will assess, diagnose, and manage a wide range of acute and chronic conditions through home visits and same-day consultations, delivering safe, effective, and person-centred care to those who need it most.
You will play an important part in supporting GP practices by responding to triaged requests, helping to ease pressure on primary care services and improving access for housebound and care home patients. The role also offers the chance to contribute to reducing avoidable hospital admissions and supporting proactive care for patients within frailty pathways.
As part of the Additional Roles Reimbursement Scheme (ARRS), you will be joining a wider multidisciplinary team across the PCN, working collaboratively to support integrated care and improve outcomes for patients and communities.
You will work closely with GP practices, community services, and PCN colleagues, building strong working relationships while maintaining high standards of clinical practice, communication, and patient advocacy in line with organisational and professional requirements.

KEY RESPONSIBILITIES
Clinical Practice:
Assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.

Assisting GPs/ANPs: Support with minor surgeries and clinical procedures.
Maintaining accurate clinical records on IT systems in accordance with NMC standards.

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
