Job summary
The Humbleyard Practice is seeking a capable and highly motivated practice nurse to join our team as Nurse Manager on a full-time basis. This is a leadership opportunity for a nurse with enthusiasm, commitment, and good communication skills to develop our loyal and well-established nursing team.
The role will involve applying your primary care nursing experience to help both experienced and new to primary care nurses develop their skills and confidence within a busy 3 surgery general practice setting.
The Nurse Manager will help the practice support our nurses to access development opportunities and we welcome discussions about general career progression. Generous and appropriate protected management time will be offered to achieve this important activity and other leadership tasks. This includes NHS leadership training for the successful candidate.
Main duties of the job
- Provide leadership and staff development, including producing personal development plans/appraisals for our nursing team.
- Coordinate the provision of care given by the nursing team.
- Liaise between the nursing team and the management team and partners.
- Help coordinate patients long-term conditions care management, including QOF and vaccinations programmes.
- Provide leadership and coordination for our infection control policies and practices

Job summary
Lincoln House Surgery is looking to welcome an experienced and fully qualified general practice nurse to assess, plan, develop, implement and evaluate treatment programmes for our patients. We have two branches, located in Apsley and Highfield, Hemel Hempstead, and you would be expected to work at both sites, on a rota basis.
This role is offered on a full or part time basis, from 30-37.5 hours per week, with one late night 10am to 6pm.
Main duties of the job
- Undertaking health assessments, administering medication and treatments and performing diagnostic tests;
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met;
- Health and well-being promotion plus individual person-centred plans for patients with/at risk of a long term condition;
- Managing and treating long-term conditions in line with national and local policies and practice needs;
- Reviewing medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments.

Harewood Surgery is looking to welcome an experienced advanced nurse practitioner to join our team, on a part-time basis, for 20-27 hours per week. We can be flexible with regard to hours/days so please apply to discuss.
Main duties of the job
- Assess patients presenting with undifferentiated and undiagnosed conditions across all age groups, using comprehensive history-taking, clinical examination, and clinical reasoning to formulate differential diagnoses
- Initiate evidence-based treatment plans, including medication prescribing, lifestyle advice, and onward referral where appropriate
- Manage same-day acute and urgent presentations, including minor illness, minor injury, infections, musculoskeletal complaints, and exacerbations of chronic conditions
- Conduct routine and follow-up consultations for patients with complex or multi-morbid long-term conditions
- Deliver consultations face-to-face, by telephone, and by video
- Manage clinical emergencies in the practice in accordance with the practice emergency protocol
- Prescribe medicines independently from the British National Formulary (BNF) within scope of competence, including some controlled drugs (Schedules 2-5, subject to regulatory restrictions)
- Conduct medication reviews, optimise medication regimes, and deprescribe where clinically appropriate
- Prescribe in accordance with NICE guidelines, local formulary, and evidence-based practice
- Monitor therapeutic outcomes and adjust prescribing in response to clinical findings
