This role will be to cover Inpatient unit, Segregation and Outpatient clinics. Free parking, free gym, a great reputation for new starter inductions offering supportive, hands on and in-depth training! Easily commutable by bus.Hours
37.5 hours a week, working across 7 days working either 7am-5pm or 7am-7pm, with every other weekend either 7am-5pm or 7.30am-5.30pm. A minimum of 4 nights across a 4 week period, working from 7pm-7am.
Salary
This position is full time, youll receive an annual salary up to £54,397 per annum FTE - depending on experience.

Main duties of the job
As a Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
- Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Main duties of the job
As a Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening

The main patient groups are patients with minor injury and minor illness. Patients who present to the Urgent Treatment Centre are managed by our expert team of Practitioners/Clinicians with an undifferentiated diagnosis through a process of assessment, formulation of a clinical management plan and subsequently discharge or transfer for definitive care. Main duties of the job
As a Senior Primary Care Nurse your responsibilities will vary. You will

HMP Altcourse is the busiest remand prison in the country, so there's lots going on in this exciting, varied role, where you will gain valuable experience, and no 2 days are ever the same!
As a Senior Primary Care Nurse, you will be based across multiple areas of the prison, including the healthcare department, planned care services, prison wings, and emergency response and a background ideally in A&E and long term conditions.
Hours:
- Monday - Sunday service
- 37.5 hours per week
- Various shift patterns
- Day shift - 7am - 8:30pm / 8:30am - 9:30pm / 8am - 4pm / 1pm - 9pm / 7am - 3pm
- Night shift 7.45pm - 7:15am
- 1 /3 weekends
- 2 month rota's
This position is full time, youll receive an annual salary up to £43,609- £49,175 per annum FTE depending on experience.

Main duties of the job
As a Senior Primary Care Nurse, your responsibilities will vary. You will
- Lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.

Main duties of the job
As a Senior Substance Misuse Nurse, your responsibilities will vary. You will
- Work as part of a multi-disciplinary team, prioritising patient care to develop individualised packages of care through comprehensive assessment and recovery planning
- Deliver a range of treatment options including clinical and psychosocial interventions in both a group and 1-1 setting with recovery at the centre of our healthcare wellbeing model
- Be able to carry out comprehensive assessments, initiate and review individual recovery plans and confidently communicate the full range of treatment options available to service users.
- Monitor therapeutic responses to medication and potential side effects and liaise with prescribers. You will work with individuals, on a 1-1 basis, providing advice and information
- Support in group work delivery and liaise with prescribers and Recovery Practitioners on clinical interventions. You will occasionally be required to support dispensing
- Have an understanding and commitment to recovery and a person centred approach.

Main duties of the job
As a Mental Health Nurse, your responsibilities will vary. You will
- Work within a multi-disciplinary team carrying out comprehensive assessments, initiating and reviewing care plans, and developing treatment options
- Undertake medication rounds, and assess and treat patients who may have undergone traumatic experiences and may be distressed

Main duties of the job
To deliver direct patient care to a group of designated patients during their inpatient episode, delegating and supervising care delivery of HCAs ensuring optimal standards at all times. Ensuring care and practices meet all external requirements of regulatory bodies working to the NMC code of practice at all times and within the framework of Practice Plus Group policies and procedures
Effectively manage and lead staff, supplies and equipment. This will involve taking charge of the ward in the absence of more senior staff providing the appropriate level of competence has been achieved.
To work as a member of the multi-disciplinary team, practicing and maintaining a high standard of clinical care and participating in all activities of the department to enhance the patients experience and ensuring safety for patients, visitors and staff in the department.

We are looking to recruit an experienced Pre-Assessment Nurse for our Outpatients Department. This is an exciting time to join PPG Plymouth as we are increasing our clinical activity and growing our teams. The inpatient ward primarily cares for our post-operative arthroplasty patients with some general surgery and other orthopaedic surgery patients.

Main duties of the job
As a Pre-Assessment Nurse in our Outpatients department you deliver a high standard of patient care supported by evidence based practice.
The Pre-Assessment Nurse will ensure that nursing is practised within the constraints of and guided by:
- All relevant Department of Health Legislation
- NMC Code of professional conduct.
- Care quality commission
- NICE guidelines
- Practice Plus Group policies and procedures.
- OPD Unit specific policies and procedures.
The post holder will function as a clinical practitioner within the Outpatient and Pre-assessment Service and will be responsible for the delivery of patient care, managing and organising a work load on a daily basis.
The post holder will ensure effective communication in which high quality patient care is delivered.

- Providing dynamic and effective leadership of the Referral to Admissions Team.
- Ensuring all patients referred to PPG Plymouth, progress through the referral to admission pathway in a timely and effective manner to provide a high volume of fit for surgery patients.
- Managing the Referral to Admissions Team to ensure co-ordination and delivery of all work activities within area of responsibility during that time.
- Co-ordinating all aspects of referral to admissions pathway for individual patients to ensure collaboration and prevent fragmented care by various health professionals and departments.
