Job summary
An exciting opportunity is available for an experienced Community Children's Nurse to work in the Continuing Care Team as a clinical lead. This is a clinical role to manage the continuing care caseload, it is separate and apart from the undertaking of continuing care assessments and care package set up, the focus of the nurse will be to lead on areas related to direct clinical care.
Main duties of the job
- Ensuring the Continuing Care Team (0-18 years) provides a high quality, evidence based locally appropriate service to families.
- Identifying new theories, policies and guidelines relating to complex health needs care in the community; disseminating research findings and developing local policies and standards accordingly.
- Providing professional and clinical leadership to all children's community nursing staff, ensuring that they are engaged with and adhering to local policies, guidelines and expectations.
- Assessing nursing care needs, developing, implementing and evaluating programmes of nursing care for children and young people on the caseload.

Working in offender healthcare
Main duties of the job
As a senior registered nurse you will provide high quality primary care services and a range of specialist interventions for offenders. At HMP Elmley, we deliver a nurse-led integrated primary care service and operate a specialist practice-based clinical model of care.
You will be delivering high quality clinical care within the Integrated Healthcare Service for the prison, demonstrating robust expert clinical skills and providing evidence-based nursing practice to those in prison.
Our senior registered nurses carry out a range of duties including; reception screening, emergency response, long term conditions management, planned care (e.g. running clinics), enhanced assessment and diagnostic intervention services and health promotion activities.
As a senior member of our team, you will hold a complex caseload and support more junior colleagues with any help or advice they may require. Job summary
Oxleas NHS Foundation Trust has been providing L&D services since 2012.
The L&D team currently provide an assessment service to Bromley and Bexley Police stations operating from 08.00-22.00 hrs Monday to Friday and 13.00-21.00hrs weekend.
The shift pattern consists of early shift 07:00-15:00 and late shift 14:00-22:00 hrs Monday to Friday. Weekend service operating hours are 13.00-21:00hrs.
The L&D also deliver a service to Bromley and Bexley Magistrates 09:00-17:00hrs Monday to Saturday including bank holidays. It will introduce community link workers as a third sector provider provided by Waythrough which will allow the L&D team greater scope to identify those vulnerable offenders who can be supported in the community once released from either police custody or the magistrates. Oxleas will continue to deliver the L&D service in close partnership with the South London Partnership (SLP).
Main duties of the job
- To provide specialist knowledge on forensic mental health within the criminal justice system, namely police stations and courts within Oxleas NHS Foundation Trust, offering advice and consultation to colleagues in the criminal justice system and mental health services.
- Maintain clinical direction and leadership, coordinating a team of clinical staff to ensure consistent delivery of high-quality mental health care for people with vulnerabilities in crisis within police custody.
- Hold responsibility for assessment of care needs and the development, implementation, and evaluation of therapeutic care.

We are seeking to appoint a committed, highly motivated, enthusiastic, and skilled Adult Physical Health , Senior Staff Nurse to join our dynamic, integrated multi- disciplinary Rapid Response Team.
In this exciting role you will be an integral part of the team developing new pathways to provide Same Day Emergency Care in the community alongside other Adult Community Services, Social Care, Acute Trusts and Primary Care colleagues.
The Rapid Response Team are part of the SE London Accelerator Programme. The Rapid Response Team are delivering the essential elements of the Urgent Community Response pathways. The Rapid Response Team has been operational since 2013 and is a multi- professional team consisting of Nurses, Occupational Therapists, Physiotherapists , Social Workers and support staff . The team provide holistic health and social care assessments for people over the age of 18 years, who are registered with a Bexley GP and live in the Borough of Bexley. The Team treats/ manages/ cares for patients in their usual place of residence with an urgent health or social care need.
The team are based at Queen Elizabeth Hospital, Woolwich and operates 7 days a week, on an 8am-8pm basis, focusing on rapid 2-hour assessments to patients with potential short-term intervention(s).

Main duties of the job
The post holder will work as a part of the multi-disciplinary Rapid Response Team including Consultant Geriatrician, Senior Nurses, Occupational Therapists and Social Services colleagues to meet the needs of patients referred to the team, ensuring the highest possible standards of practice with a strong emphasis on falls, frailty.
The post holder will conduct comprehensive nursing assessments for patients within acute and community settings. They will need to demonstrate sound clinical reasoning and risk assessment skills to guide appropriate nursing intervention and when appropriate avoid unnecessary ED presentation. The post holder will also be skilled in the management of breast drains, wound care and intravenous antibiotic therapy.
The post holder will be required to work across hospitals sites on a planned basis and in community when responding to urgent community referrals. Post holder will undertake delegated line management of junior staff, both informal and formal basis.
The team act as a liaison between acute and community settings, by identifying the most appropriate onward referrals and initiating integrated care pathways, ensuring seamless provision of care and on- going case management. Main duties of the job
The MHST Senior Practitioner role supports the delivery of evidence-based 1:1 and group interventions for children and young people presenting with mild to moderate mental health need.
The role requires joint working and close liaison with education colleagues to support the whole school approach to mental health and wellbeing within school settings, facilitating appropriate signposting to other services as required. The post holders will actively contribute to the delivery and development of a quality MHST service within Oxleas.
The MHST Senior Practitioners, together with MHST Lead practitioners, offer support and supervision to the MHST workforce and it is an expectation of the role that the post holders have completed, or are willing to complete, the required Education Mental Health Practitioner supervision course in order to appropriately support the EMHP workforce.

Main duties of the job
The post holder will provide leadership to the Mainstream School-age Service and coordinate external contracts in Bexley ensuring continued and co-ordinated service delivery and development of clinical provision and inter-agency, local partnership working with local partners.

Main duties of the job

Job summary
This position is located at HMP Swaleisde within the Primary Care Nursing Team. The post holder will also need to collaborate closely with colleagues at nearby Kent prisons, including HMP Emley and HMP Standford Hill, and may occasionally provide cross-cover between sites.

Main duties of the job
In this varied role, you will complete specialist assessments, develop and review care and risk-management plans, and deliver therapeutic interventions to individuals and groups. Working closely with multidisciplinary teams and partner agencies, you will help patients manage their health, promote wellbeing and ensure they receive coordinated, evidence-based care. You will also provide visible clinical leadership, supporting and supervising junior colleagues, contributing to safeguarding, audit, training and service improvement. This post is based across both HMP Ashfield and HMP Leyhill. You will be required to work 3 shifts per week at HMP Ashfield (7.30am-5.30pm) and 1 shift per week at HMP Leyhill (7.30am - 5pm).
