This is an exciting time to join our service as we create a new North East Oxfordshire team in order to better meet the growing population of the Bicester, Kidlington, Islip, Woodstock and Yarnton locality.
We are seeking experienced mental health nurses ready to embrace innovative ways of working in line with the Community Mental Health Framework. If you are motivated by positive change and want to be part of the setting up and creation of the team from the outset, alongside existing committed staff who currently cover that area out of Oxford City, we would love to hear from you.
Together you will form a innovative and caring service build upon the values of Oxford Health. Working alongside a range of clinical professionals and closely with third sector partners to provide high-quality, holistic, person-centred care.

This is a full-time role (37.5 hours, Monday-Friday, 09:00-17:00).
Due to the nature of the role, the ability to travel independently across Trust sites and to conduct home visits is essential and not all areas are accessible via public transport.

Main duties of the job
As a Band 6 Community Mental Health Nurse and key worker, the post holder is responsible for managing a caseload of adults with complex mental health needs, undertaking comprehensive mental health and risk assessments, developing and reviewing personalised care plans, and delivering evidence-based interventions to support recovery and relapse prevention. The role involves working autonomously within a multidisciplinary team, coordinating care with families, carers and partner agencies, monitoring both mental and physical health needs, including cardiometabolic assessments, and providing specialist advice on medication management. The post holder acts as an advocate for service users, contributes to duty and urgent care functions, supports students and junior staff through supervision and mentorship, participates in service improvement and quality initiatives, and promotes recovery-focused, rights-based and co-produced approaches to care.

Oxford Health NHS Foundation Trust is delighted to invite applications for a Community Mental Health Nurse role within our newly established Assertive Outreach Team (AOT).
This innovative service will provide high-quality, person-centred care to individuals with complex mental health needs who may struggle to engage with traditional services. Operating across Oxford, the team will deliver proactive, flexible support, responding to need in a timely and coordinated manner.
Assertive Outreach is founded on active engagement rather than office-based care. Staff will work flexibly across the city, supporting individuals in community settings, their homes or Trust premises. Interventions will be tailored, relationship-focused and recovery-oriented, promoting continuity, trust and consistent engagement. Shared caseload responsibility, alongside a key worker model and regular MDT reviews, will enable rapid responses to relapse, risk and disengagement.

You will be confident in managing complexity and dynamic risk, while working within a recovery-focused, trauma-informed framework. Main duties of the job
The Band 6 Nurse for the Oxfordshire Assertive Outreach Team (AOT), working across Oxford City, is responsible for delivering high-quality, recovery-focused and evidence-based nursing interventions to adults with severe and enduring mental health difficulties who experience significant challenges in engaging with services.
The Team will be a fully integrated multidisciplinary service comprising a Consultant Psychiatrist, Clinical Team Manager, Psychologists, Occupational Therapists, Nurses, Support Workers and Administrators.
You will:
- Work as an autonomous practitioner an active participant of the multi-disciplinary team undertaking specialist nursing assessments in collaboration with the patients and their families/carers and presenting back to the wider MDT.
- Work in partnership with patients and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.
- Deliver evidence-based interventions following NICE guidelines to promote recovery and relapse prevention in a variety of settings.
- Regularly review care and outcomes, supporting service users to complete clinical outcome measures and monitoring progress and adapting treatment options as required.

We are seeking a community mental health nurse to work with our Aylesbury based team. You may possess specialist knowledge of looking after Older Adults, alternatively, you may not have worked with an Older Adult patient group before but will always put the patient at the centre of what you do to provide individualised, person-centred care.
Our hub is based at Whiteleaf Centre in Aylesbury, which is the established mental health hub in Buckinghamshire. We provide community-based treatments and are fortunate to work alongside our own dedicated Social Care Team. Our team provide focused interventions for the older adults of Buckinghamshire.
This role also offers an exceptional support and training opportunities within the team to grow and thrive in your nursing career.
Main duties of the job
- Independently assess patients' mental health needs and develop personalised care plans ensuring that interventions are tailored to individual requirements.
- Deliver home-based treatment and support for patients in the community experiencing mental health crises, helping to stabilise their condition and support them further.
- Manage a defined caseload of patients, acting as their key worker. Including continuous assessment, monitoring, and adjustment of care plans to ensure they meet the evolving needs of the patients.
- Work in partnership with patients, their carers, and other healthcare professionals to set and achieve treatment goals, ensuring care is holistic and patient-centred.
- Provide various therapeutic interventions in line with professional standards and job specifications, and manage the discharge process, ensuring safe and timely transitions from care.
- Engage in clinical and managerial supervision, contribute to the training and education of team members, and participate in service improvement initiatives to enhance the quality of care provided.

As part of our services across Oxfordshire, we are implementing Keystone Mental Health & Wellbeing Hubs to support GP practices with Mental Health assessment and support for patients in their communities.
We have created this exciting post within the Keystone Abingdon MH & Wellbeing Hub (KMHT).
We are looking for a part-time MH integrated hub clinician who is experienced in delivering evidence-based interventions in a community setting, with a passion for partnership approaches to care for adults living with serious mental illness.
This role needs someone who is innovative and who wants to help achieve our vision for cross-sector integrated pathways, removing existing barriers, to deliver better access to dedicated mental health support in local, non-stigmatised community settings.
Main duties of the job
- Supporting the Clinical Lead in the development and implementation of the service and your role within it.
- Supervision and leadership for appropriate staff identified
- Screen, triage, assess and coordinate referrals
- Provide brief support and time limited evidence-based interventions,
- Support patients ready to transition to primary care and develop care plans and prepare people for step down.
- Map community resources and assets that are available for those with SMI within the specific PCN geography.
- To support collaborative working between the full range of physical health and mental health services

- Be responsible for the assessment, treatment, and discharge of patients whose problems are managed by psychologically based standard care plans.
- Evaluate and make decisions about treatment options considering both theoretical and therapeutic models and complex factors concerning the patients mental health difficulties.
- Help co-facilitate MBT groups within our well-established countywide Mentalisation-Based Treatment (MBT) programme. Delivered in person from our Headington base in Oxford city.

Job summary
The Patient Advice and Liaison Service Officer is required to provide easily accessible customer services to the local community working closely with the Complaints & PALS Manager and PALS Service Lead.
One of the great things about this role is that you'll be liaising directly with service users and their families; this is a great opportunity if you really enjoy working with people. We are seeking to recruit an enthusiastic and hardworking individual to work within the Complaints & Patient Advice and Liaison Service (PALS) team. This role is also suitable for someone who has experience of using secondary mental health services as a patient or a carer.
Main duties of the job
The successful postholder will be responsible for providing an accessible customer service for patients using the Trust's services and their carers and families. You will be required to run a series of regular PALS surgeries, speaking directly to patients and their families about their experiences.
The postholder will facilitate a speedy and informal resolution of problems and concerns raised by patients, their families, and carers through liaison with Trust staff and other external agencies. They will be responsible for providing information about local services, including voluntary and statutory agencies.
Part of the role will require the postholder to provide high quality guidance and support to Directorates on resolving and responding to concerns and complaints. They will be required to identify themes and tends arising from concerns and patient feedback to ensure actions are taken. You will be responsible for working with Directorates to ensure that they utilise all feedback received to continuously improve services.
Excellent written and verbal communication skills, attention to detail, and a sensitive and caring manner are key to this role. The successful candidate will be required to understand the need for confidentiality and display a mature and professional attitude. The post-holder will be required to travel to the Trust's various hospital sites.

We're establishing a newIntensive Case Management (ICM) and Rehabilitation Pathway, bringing together specialist professionals to deliver flexible, proactive support for adults with some of the most complex mental health needs.
We're looking for aMental Health Nurses who are committed to building therapeutic relationships, working collaboratively across services and delivering recovery-focused care in the community. As part of this newly established pathway, you'll play an important role in developing a service designed around continuity, assertive outreach and person-centred support.
This is a community-based role where you'll meet people where they are, whether that's at home, in community settings or Trust premises. Rather than relying on traditional clinic appointments, you'll work proactively to engage people who may struggle to access or remain connected with services, helping to reduce relapse, prevent crisis and improve long-term outcomes.
Main duties of the job
As a Community Mental Health Nurse, you'll manage a defined caseload of adults with complex mental health needs, providing specialist nursing assessments, evidence-based interventions and personalised care planning.
Working autonomously and as part of the multidisciplinary team, you'll build strong therapeutic relationships with service users and their families, contribute specialist nursing expertise to clinical decision-making, and support people to achieve their recovery goals.
You'll also contribute to service development, quality improvement and the supervision of students and junior colleagues, helping to embed a culture of learning and continuous improvement within the pathway.
