Job summary
An exciting opportunity has arisen for a passionate and motivated Band 6 Chronic Kidney Disease (CKD) Specialist Nurse to join our friendly, supportive and forward-thinking renal team.
We are looking for an experienced renal nurse with excellent communication skills, a compassionate approach and a genuine interest in long-term condition management, patient education and service development. You will work within a multidisciplinary team supporting patients with advanced kidney disease, helping them understand their condition, manage symptoms and make informed decisions about their treatment and future care.
You will manage your own caseload and develop specialist expertise in kidney replacement therapy education, anaemia management, hepatitis B vaccination clinics and nurse-led patient support. This is a role that requires autonomy, clinical judgement and the ability to build meaningful therapeutic relationships with patients and families.
Our small specialist team is collaborative, welcoming and committed to delivering outstanding patient-centred kidney care. We support each other, value new ideas and are expanding our conservative kidney management service, giving you the opportunity to contribute to service development and innovation.

As a Band 6 CKD Specialist Nurse, you will play a key role in delivering and developing our specialist CKD service.
You will manage a caseload of patients, providing holistic assessment, care planning and ongoing monitoring. The role requires a high degree of autonomy, using clinical judgement to assess risk, prioritise care and make timely decisions within agreed protocols.
You will run nurse-led clinics, provide kidney replacement therapy education, support anaemia management and participate in hepatitis B vaccination clinics. You will review pathology results, coordinate investigations, monitor treatment effectiveness and act as a key point of contact for patients and their families.

We are seeking to recruit a Clinical Nurse Specialist within our Child and Adolescent Crisis Service (CAIST) in Norfolk and Waveney. An ideal role for someone that wants to combine elements of management and leadership while also maintaining specialist direct clinical work.
We have bases in East, Central & West Norfolk; we allocate a base closest to where you live. The role is peripatetic so you will be required to travel to different sites, therefore being able to travel independently is essential.
The CAIST (Crisis, Assessment & Intensive Support Team) service is a vibrant and dynamic service that has 3 teams based across Norfolk & Waveney service, 7 days a week. The service is commissioned for 08:00-20:00 7 days per week.
The team offers community based intensive support to children, young people & their families. CAIST aim to establish trusting therapeutic relationships, to reduce risks when children & young people may be experiencing high levels of distress.

Main duties of the job
Whilst experience in working with children and young people would make you perfectly suited to this role, we are really open to welcoming people from clinical backgrounds who have experience in other areas of mental health. We understand that you will be able to bring with you a wealth of skills and experience to the team and would welcome new ways of thinking.
You will offer direct clinical work (crisis assessment and individual interventions via intensive support) with children under 18 years that have presented in crisis, alongside their families and carers where appropriate.
Working as part of a multidisciplinary team, you will model high standards of clinical practice, undertake complex mental state and risk assessments, and regularly lead on clinical decision-making. You will provide specialist clinical advice, consultation, and formulation to colleagues across the CYP pathway, (including CAMHS and Youth teams), to promote safe and effective crisis management.
With oversight from the Senior Nurse, you will take a key role in service development and quality improvement, supporting governance, supervision, training, and implementation of key clinical frameworks. You will act as a role model for compassionate and recovery-focused care, promoting least-restrictive practice and instilling a culture of continuous learning, safety, and responsiveness.

Main duties of the job
As a Registered Nurse Associate, your responsibilities will vary. You will
- Provide outstanding care to our patients, listening to and acting on their feedback so that care is personalised and informed by what matters to them

This is an exciting opportunity to join the Central Norfolk Early Intervention in Psychosis team on a full-time, permanent basis as a Clinical Nurse Specialist. This Clinical Nurse Specialist role will include Non-Medical Prescribing to the Central Norfolk caseload as appropriate and under supervision of our service consultant. As such, NMP qualification and experience is an essential criterion for this post.
Main duties of the job
The job role with consist of the following, but is not limited to;
- Accepting and triaging all new referrals received by the team.
- To communicate /liaise effectively with relevant referring partners and other agencies.
- Arranging and carrying out initial assessments.
- Offering time to support the wider team with visits where specialist knowledge could be required
- Being part of the wider Senior Leadership Team and encouraging evidence based changes and processes across the service.
- To ensure that any identified immediate risks are appropriately responded to and a proportionate risk management strategy is actioned, with input from the multidisciplinary team members.
- Keeping up to date with relevant perinatal research and news to share with the wider team
- Offering caseload supervision to case managers

Our Mental Health Liaison team based at the James Paget University Hospital (JPUH) in Gorleston has an exciting opportunity for a Clinical Nurse Specialist - Older Adults to join their fantastic team on a permanent full time basis.
This role is there to provide expertise, support and advice to clinical practitioners regarding all aspects of clinical care support those attending the JPUH with new or ongoing mental health difficulties and illness; to identify their needs, ensuring these are met effectively whilst in hospital and to contribute towards timely/supportive discharge planning.

JPUH is located in the coastal town of Gorleston, just a stones throw from beautiful beaches and with great connections to Norwich, Great Yarmouth and Lowestoft. Job summary
This year CATAT have been given the opportunity to expand the team with several different posts, some of which have already been filled. But we do have a post that has now become vacant for a CNS - this CNS needs to have their qualification of Non-Medical Prescriber as well as a Mental Health Nursing qualification.
CATAT who after our first year of being live (2023) was highly recommended for the Working Together for Better Mental Health at the staff excellence awards. Are looking for professionals that have a keen interest of working with CYP and their families, wanting to help them achieve better outcomes for their future and strive to keep them in the community.
The aim of our team is to prevent CYP from having inpatient admissions wherever possible, and to support them and their family in the community in a proactive way. The team will serve as a gateway in and out of Tier 4 (T4) provision. CATAT work alongside the core community teams as well as partner agencies to offer CYP increased/additional support.
CATAT have 2 main pathways:
Admission Avoidance - Least restrictive option, CYP who are already open to a core team but need a more intensive intervention to support their mental health needs. To prevent things from worsening and/or needing a hospital admission.
In-reach/Outreach - support to young people leaving inpatient settings (T4) to help them adjust back into life in the community.
Main duties of the job

- Therapeutic interventions and approaches in relation to the expressed mental health needs of the CYP. Such as emotional regulation, DBT skills, AMBIT technics, risk management.
- Systemic approach to working with CYP, families, carers, and wider professional networks.
- Training and education for other services
- Promote joined up working and liaison with other involved services and teams across mental and physical health and children's services.

Main duties of the job
About the role
As an ANP, you will assess and manage patients presenting to the practice, including those with acute, undifferentiated and long-term conditions.
You will be able to work autonomously within your scope of practice, using your advanced clinical skills to assess, diagnose, treat and refer patients appropriately.
Your role will include:
- Assessing and triaging patients, including same-day presentations.
- Taking comprehensive histories and undertaking appropriate clinical examinations.
- Assessing and managing undifferentiated and undiagnosed presentations.
- Diagnosing and treating a broad range of acute and chronic conditions.
- Prescribing safely within your scope of practice and in line with relevant national and local guidance.

- As this post is being recruited as an Advanced Practitioner Nurse through the Primary Care Network Additional Roles Reimbursement Scheme (ARRS), applicants will need to meet the applicable NHS England/PCN requirements for the role.

- Current registration with the NMC.
- Independent prescribing qualification (V300 or equivalent).
- A masters degree level qualification in a relevant area of expertise.
- Evidence of working at masters level or equivalent across the four pillars of advanced practice: clinical practice, leadership and management, education and research.
- Evidence of recognised preparation and capability in advanced practice in accordance with the current NHS England/Centre for Advancing Practice requirements.
- The clinical knowledge, skills and experience required to undertake autonomous advanced clinical practice within general practice.
