Job summary
An exciting opportunity has arisen for a NMC Registered Band 5 Nurse to work within the Home First Team (Discharge to Assess Pathway 1 based in South Staffordshire).
The Home First Team provides clinical support for patients in the community to either avoid potential hospital admissions or to support people to return home through the Discharge to Assess Pathway 1 following a period of acute care in hospital.
We pride ourselves on the delivery of quality care to community patients, many with complex needs and we actively promote the philosophy of self-care and the promotion of independence whilst offering the facilities of rehabilitation and assessment.
