We are excited to be recruiting a Tuberculosis (TB) Specialist Nurse for North Kent. We are seeking someonewith excellent communication skills, who enjoys the challenge of working with socially and medically complexpatients in a community setting and who is passionate about advocating for our patients. We are lookingforward to welcoming the right candidate to our 'Outstanding' Trust and our friendly and well-establishedteam.
We care for adults, children and babies, through case management, screening clinics, BCG vaccination and health promotion. The post will involve visiting people in their own homes, clinics and some outreach work aswell as phone and virtual appointments.
The role will primarily involve caring for patients in the North Kent, but may involve some wider travel in Kentfor training and meetings. The post holder will need to be able to work autonomously without directsupervision.

Main duties of the job
Specialist TB Nurses play a vital public health role in reducing the onwards transmission of TB throughcontact tracing, identifying close contacts of index cases and offering screening in line with TB guidelines.Their role includes co-ordinating and delivering TB screening clinics, attending incident management meetingsand responding to outbreaks.Specialist TB Nurses provide expert knowledge and skills acting as a resource to advise, educate and supportthe TB multidisciplinary team and raise awareness of TB to the public and other health care professionals.

We are seeking a Band 6 Wound Clinic Team Leader to join our Tissue Viability and Wound Therapy Service, providing high-quality, evidence-based wound care to patients attending Medway Community Healthcare wound clinics.
The successful candidate will work as part of a specialised multidisciplinary team, assessing patient needs, developing and evaluating personalised care plans, and delivering appropriate wound treatment, prescribing, advice and support to promote healing and improve outcomes.
You will support the effective day-to-day management of wound clinics, assist the Senior Wound Therapist, and lead workload allocation during short periods of absence. The role includes providing leadership, supervision and development support to Healthcare Assistants, students and visitors, ensuring safe, compassionate and high-quality care aligned with current evidence and best practice.

- Assess, treat and monitor patients attending
- wound clinics, delivering evidence-based wound care and evaluating treatment outcomes.

- Provide education, advice and support to patients, families and carers to promote wound healing, self-management and health improvement.
- Communicate effectively with GPs, community services, care homes, hospitals and other healthcare professionals to ensure coordinated patient care.

Main duties of the job
Join our dedicated multidisciplinary team (MDT) and play a key role in helping patients recover from stroke, regain independence, and return home safely. Working within our specialist stroke rehabilitation service, you will provide high-quality, person-centred care while supporting patients, families and carers throughout their recovery journey.
This is a varied and rewarding role where no two days are the same. You will assist with personal care, mobility, nutrition and hydration, while encouraging patients to achieve their rehabilitation goals. As part of the MDT, you will undertake a range of clinical duties including observations, blood glucose monitoring, catheter and stoma care, venepuncture, ECG recording, bladder scanning, simple wound management, specimen collection and maintaining accurate patient records.
You will also contribute to risk assessments, infection prevention activities, and support patients' physical, emotional and social wellbeing. Working closely with nurses, therapists and other healthcare professionals, you will help deliver safe, effective care and promote the best possible outcomes for patients.

- You will have either: A Qualifications and Credit Framework (QCF) Level 3. This could be an NVQ Level 3 apprenticeship, BTEC or equivalent international qualification in healthcare. Or you will be able to demonstrate the equivalent level of experience

Job summary
We are seeking a dynamic and enthusiastic nurse to join our friendly team in East Kent. The Children's Therapiesservice provides Speech and Language Therapy, Occupational Therapy, Physiotherapy and Care Co-ordination services toCYP aged 0-19.
Care co-ordination is a service for babies and children (up to the age of 12) who present with an identified developing condition or disability which is impacting on, or has the potential to significantly impact upon their long-term developmentand/or function.
The post is suited to a nurse with experience of working with children and families in the community and who works well in a multidisciplinary team. We currently offer a hybrid work pattern for staff with a combination of face to face andvirtual working. Please see below for the main duties of the job.

Main duties of the job
o Provide assessment, planning, implementation and evaluation of holistic care linked to packages of prescribed care oridentified projects.oSupport in the supervision and development of staff.oActively work as a member of the children's therapies team to provide high quality care to service users.oAct as lead and support for care co-ordinators, managing the care co-ordination pathwaysWork with key colleagues (ie education and health) in order to support multi agency models of service delivery.The successful applicant will join our friendly therapies team. We pride ourselves on the support and induction offered tonew staff including; regular meetings with a line manager, access to supervision; in house and external training. We offerregular clinical governance days where all localities come together to share knowledge, skills and CPD experiences.
