Urgent Primary Care Doctor Bank (Home Visiting)
Bring urgent primary care to the patients who need it most - in their own homes.
Urgent care is General Practice at its most immediate.
Patients don't always know what they need when they reach us. They may have been referred through NHS 111 and, following clinical assessment, need to be seen in their own home.
What they need is an experienced doctor who can listen, assess, make confident clinical decisions and get them to the right care.
At IC24, we're looking for Urgent Primary Care Doctors to work within our Home Visiting Service across Sussex, providing urgent face-to-face care to patients in their own homes. You will have access to IC24s fleet of vehicles and any equipment required to carry out this role, we only need you to bring your clinical expertise.
You'll work alongside our drivers, dispatch teams and wider urgent care colleagues, providing safe, timely and effective care while helping patients access the most appropriate treatment for their needs.
If you enjoy the variety of General Practice, the autonomy of home visiting and the clinical challenge of assessing and treating patients in their own environment, this could be the role for you.
What you'll focus on
Assessing and treating patients who need urgent care
You'll provide face-to-face assessment, diagnosis and treatment for patients who have been clinically triaged as requiring a home visit.
Patients may be housebound, approaching the end of their life or have an urgent clinical need that means being assessed in their own home is the most appropriate option.
You'll assess what's happening, decide what needs to happen next and ensure patients receive appropriate treatment, referral and safety-netting.
Making confident clinical decisions
Home visiting requires sound clinical judgement.
You'll often be working with patients you haven't met before, sometimes with limited background information and sometimes when they're anxious, unwell or experiencing difficult circumstances.
You'll need to be able to assess risk, manage uncertainty and make safe decisions independently, recognising when a patient can be treated and supported at home and when they need escalation or an alternative care setting.
Delivering care in patients' homes
This role takes urgent primary care beyond the traditional consulting room.
You'll travel to patients across Sussex, with visits allocated through our IC24 dispatch systems.
You'll assess, diagnose, treat and prescribe appropriately, managing a broad range of urgent primary care presentations and providing compassionate care to patients, relatives and carers.
This can include supporting patients who are frail, housebound or receiving palliative and end-of-life care.
Working as one multidisciplinary team
Good urgent care depends on people working well together.
You'll work closely with IC24 drivers and dispatch teams, as well as colleagues across NHS 111, community nursing, pharmacy, paramedic services and primary care.
You'll communicate clearly, share clinical information and work collaboratively to make sure patients receive joined-up care.
Providing the right care in the right place
Not every patient who needs urgent care needs to be in an Emergency Department.
Where clinically appropriate, you'll help patients remain safely in their own homes by putting the right treatment, advice, referrals and safety-netting in place.
You'll liaise with community and primary care services to support continuity of care and recognise when a patient needs escalation to another service or hospital.
Whether you're treating a patient at home, referring them to another service or escalating their care, the patient and their safety will be at the centre of your decision-making.
Maintaining excellent clinical standards
You'll work within IC24's clinical governance framework and prescribing formulary, maintaining accurate and timely clinical records and working in accordance with GMC guidance and relevant national and local clinical standards.
You'll maintain your CPD and essential learning and have opportunities to contribute to audit, learning events, education and quality improvement.
This is urgent care and variety comes with it
No two sessions will necessarily look the same.
You'll see patients across ages and backgrounds, presenting with everything from relatively straightforward acute illness to complex physical, mental health or social circumstances.
Some sessions will be busy. Priorities can change quickly. You'll sometimes need to make important clinical decisions with limited information, working independently in a patient's home while remaining connected to the wider IC24 team.
You'll probably thrive here if you:
- Enjoy the variety and unpredictability of urgent care
- Are confident assessing undifferentiated patients
- Can make sound clinical decisions independently
- Are comfortable managing risk and uncertainty
- Have strong face-to-face consultation and home visiting skills
- Communicate clearly and compassionately with patients, relatives and carers
- Are comfortable supporting frail, palliative and end-of-life patients
- Can adapt when service demands change unexpectedly
- Are comfortable working independently while knowing when to seek support
If you prefer a predictable caseload and a fixed clinical environment, home visiting may not be the right fit.
But if you enjoy thinking on your feet, value clinical autonomy and get satisfaction from helping patients receive the care they need in their own homes, we'd like to hear from you.
What you'll bring
You'll:
- Hold current GMC registration
- Be an MRCGP Graduate
- Have relevant clinical experience within areas such as Primary Care, Emergency Medicine, A&E or previous Out of Hours experience
- Have experience of face-to-face clinical consultation
- Have experience of primary care home visiting
- Hold a full clean UK Driving License
- Be comfortable driving both automatic and manual vehicles
The environment you'll work in
Urgent care operates around the needs of our patients, so flexibility is an important part of this role.
You'll work across our Home Visiting Service, travelling to patients in their own homes across Sussex.
You'll work alongside IC24 drivers and dispatch teams, with visits allocated through our dispatch systems using CLEO on Toughbook devices and laptops.

Urgent Primary Care Doctor Bank (Urgent Treatment Centres)
Be the clinician patients need when their care can't wait.
Urgent care is General Practice at its most immediate.
Patients don't always know what they need when they reach us. They may have been referred through NHS 111, redirected from an Emergency Department or walked into an Urgent Treatment Centre looking for help.
What they need is an experienced doctor who can listen, assess, make confident clinical decisions and get them to the right care.
At IC24, we're looking for Urgent Primary Care Doctors to work across our Urgent Treatment Centres in East Kent, including William Harvey Hospital in Ashford, Kent and Canterbury Hospital in Canterbury and Queen Elizabeth The Queen Mother Hospital in Margate.
You'll work as part of a multidisciplinary urgent care team, providing safe, timely and effective care while helping patients access the most appropriate treatment for their needs.
If you enjoy the variety of General Practice, the pace of urgent care and the clinical challenge of never quite knowing what is coming through the door next, this could be the role for you.
What you'll focus on
Assessing and treating patients who need urgent care
You'll provide face-to-face assessment, diagnosis and treatment for patients presenting with a broad range of acute illness, minor illness and injury.
Patients may come to you through NHS 111, as walk-ins or following redirection from an Emergency Department.
You'll assess what's happening, decide what needs to happen next and ensure patients receive appropriate treatment, referral and safety-netting.
Making confident clinical decisions
Urgent Treatment Centres require sound clinical judgement.
You'll often be working with patients you haven't met before, sometimes with limited background information and sometimes when they're anxious, unwell or experiencing difficult circumstances.
You'll need to be able to assess risk, manage uncertainty and make safe decisions under pressure, recognising when a patient can be treated and discharged and when they need escalation or an alternative care setting.
Working at the heart of urgent care
You'll work in a fast-moving clinical environment where patient needs can vary considerably from one consultation to the next.
You'll see a broad mix of acute illness, minor illness and injury, using your General Practice and urgent care experience to determine the most appropriate course of action.
Some patients will need assessment, treatment and discharge. Others may need further investigation, onward referral or escalation into another part of the healthcare system.
Being able to recognise that difference and make the right decision safely is a fundamental part of the role.
Working as one multidisciplinary team
Good urgent care depends on people working well together.
You'll work alongside nurses, advanced practitioners, reception colleagues, other doctors and allied healthcare professionals, as well as colleagues across Emergency Departments, NHS 111, community services and primary care.
Within our East Kent Urgent Treatment Centres, you'll work as part of the wider East Kent UTC Alliance, alongside colleagues from EKHUFT, Invicta Health CIC, Thanet Health CIC and IC24.
You'll communicate clearly, share clinical expertise and work collaboratively to make sure patients move smoothly through the system.
Providing the right care in the right place
Not every patient who arrives at hospital needs Emergency Department care.
You'll help assess, prioritise and redirect patients where clinically appropriate, supporting people to access the right level of care while helping reduce unnecessary pressure elsewhere in the system.
Whether you're treating and discharging a patient, referring them to another service or escalating their care, the patient and their safety will be at the centre of your decision-making.
This is urgent care - and variety comes with it
No two sessions will necessarily look the same.
You'll see patients across ages and backgrounds, presenting with everything from relatively straightforward minor illness to more complex physical, mental health or social circumstances.
Some sessions will be busy. Priorities can change quickly. You'll sometimes need to make important clinical decisions with limited information and balance the needs of several patients at once.
You'll probably thrive here if you:
- Enjoy the variety and unpredictability of urgent care
- Are confident assessing undifferentiated patients
- Can make sound clinical decisions under pressure
- Are comfortable managing risk and uncertainty
- Have strong face-to-face consultation and triage skills
- Communicate clearly and compassionately with patients, relatives and carers
- Enjoy working within a busy multidisciplinary clinical environment
- Can adapt when service demands change unexpectedly
- Are comfortable working independently while knowing when to seek support
If you prefer a predictable caseload and a slower-paced clinical environment, an Urgent Treatment Centre may not be the right fit.
But if you enjoy thinking on your feet, value clinical variety and get satisfaction from helping patients at the point they need care most, we'd like to hear from you.
What you'll bring
You'll:
- Hold current GMC registration
- Be an MRCGP Graduate
- Have relevant clinical experience within areas such as Primary Care, Emergency Medicine, A&E or previous Out of Hours experience
- Have experience of face-to-face clinical consultation
- Be confident assessing and managing undifferentiated patients
- Be comfortable making autonomous clinical decisions and managing risk
- Have strong communication and clinical record-keeping skills
- Be able to work effectively as part of a multidisciplinary team
The environment you'll work in
Urgent care operates around the needs of our patients, so flexibility is an important part of this role.
Hours include evenings, weekends, overnights and Bank Holidays.
You'll work within our East Kent Urgent Treatment Centres, alongside colleagues from across the wider urgent and emergency care system.
It's a responsive clinical environment where demand and priorities can change quickly. You'll need to be comfortable adapting while maintaining safe, effective and patient-centred care.

Urgent Primary Care Doctor Bank (Home Visiting)
Bring urgent primary care to the patients who need it most - in their own homes.
Urgent care is General Practice at its most immediate.
Not every patient who needs urgent care can come to us. Some are housebound, frail, approaching the end of life or have been clinically assessed as needing a face-to-face review in their own home.
What they need is an experienced doctor who can listen, assess, make confident clinical decisions and get them to the right care.
At IC24, we're looking for Urgent Primary Care Doctors to work within our Home Visiting Service across Kent.
You'll work alongside our drivers, dispatch teams and wider urgent care colleagues, providing safe, timely and effective care to patients in their own homes.
If you enjoy the variety of General Practice, the autonomy of home visiting and the clinical challenge of assessing and treating patients in their own environment, this could be the role for you.
What you'll focus on
Assessing and treating patients in their own homes
You'll provide face-to-face assessment, diagnosis and treatment for patients who have been clinically triaged as requiring a home visit.
You'll see patients with a broad range of urgent primary care needs, considering not just the immediate clinical problem but their wider circumstances, environment and support needs.
Treating the whole patient
Seeing someone in their own home can give you a very different perspective on their health.
You'll care for patients across ages and backgrounds, including older and frail patients, people with complex health needs and those approaching the end of their lives.
You'll consider the patient's clinical presentation alongside their environment, support network and wider circumstances to determine what they need to remain safe.
Keeping patients safely in their community
Not every patient who needs urgent care needs to be in an Emergency Department.
Where clinically appropriate, you'll help patients remain safely at home by putting the right treatment, advice, referrals and safety-netting in place.
You'll work with community nursing, pharmacy, paramedic, primary care and wider healthcare teams to support continuity of care.
And when hospital or another service is the right place for a patient, you'll recognise that and escalate appropriately.
Working independently, but never in isolation
Home visiting is autonomous by nature.
You'll be making important clinical decisions in patients' homes without the immediate infrastructure of a traditional clinical setting around you.
But you'll remain part of the wider IC24 team.
You'll work closely with our drivers and dispatch colleagues and communicate with other health and care professionals to make sure patients receive timely, joined-up care.
This is urgent care beyond the consulting room
No two home visits will necessarily look the same.
You'll see patients with everything from relatively straightforward acute illness to complex physical, mental health and social circumstances.
Sometimes the answer will be clear.
Sometimes you'll need to piece together limited information, understand what's happening around the patient and make an important clinical decision without having every investigation immediately available.
You'll probably thrive here if you:
- Enjoy autonomous clinical practice
- Are confident assessing undifferentiated patients
- Have strong face-to-face consultation and home visiting skills
- Can make sound clinical decisions independently
- Are comfortable managing risk and uncertainty
- Can make safe decisions without having every investigation immediately available
- Are confident caring for frail, complex and end-of-life patients
- Communicate clearly and compassionately with patients, relatives and carers
If you prefer working within a predictable clinic environment with immediate access to a wider clinical team, home visiting may not be the right fit.
But if you value clinical autonomy, variety and the opportunity to deliver genuinely patient-centred care in people's own homes, we'd like to hear from you.
What you'll bring
You'll:
- Hold current GMC registration
- Be an MRCGP Graduate
- Have relevant clinical experience within areas such as Primary Care, Emergency Medicine, A&E or previous Out of Hours experience
- Have experience of face-to-face clinical consultation
The environment you'll work in
Urgent care operates around the needs of our patients, so flexibility is an important part of this role.
Hours include evenings, weekends, overnights and Bank Holidays.
You'll work from our Kent Home Visiting dispatch points, travelling out into the community to assess and treat patients in their own homes.
Home visiting is responsive by nature, so priorities can change during a shift. You'll need to be comfortable adapting while maintaining safe, thoughtful and compassionate care.

You'll provide face-to-face assessment, diagnosis and treatment for patients who have been clinically triaged as requiring a home visit.
You'll see patients with a broad range of urgent primary care needs, considering not just the immediate clinical problem but their wider circumstances, environment and support needs.

Lead Nurse / Advanced Practitioner
Build the nursing and clinical teams that will change how primary care is delivered in your Brighton community.
This isn't a role for someone looking to simply step into an established team and maintain the status quo.
We're looking for a Lead Nurse / Advanced Practitioner with a builder's mindset - someone who enjoys creating, improving and leading through change, and who is excited by the opportunity to help shape what a great primary care team looks like from the ground up.
Over the last two years, we've been on a significant transformation journey across our Brighton practices. We've made substantial changes to how we work, strengthened our clinical and operational approach and are now bringing our Sussex practices much closer together, enabling teams to share expertise, resources and support.
We're now taking the next step.
Allied Medical Practice and Ship Street Surgery are coming together as one group, with plans to move into purpose-built practices next year. This will give us the capacity and infrastructure to significantly grow our patient population and build a modern primary care service for the future.
That's where you come in.
We're looking for someone who wants to help build the team, the culture and the clinical service that will take us into that next chapter.
This is a genuine opportunity to make your mark.
The role
As Lead Nurse / Advanced Practitioner, you'll combine advanced clinical practice with visible, hands-on leadership.
You'll be responsible for developing our nursing and healthcare assistant workforce, establishing high clinical standards and creating a culture where people are supported to learn, improve and deliver excellent patient care.
You'll work closely with Practice Managers, Clinical Leads and the wider multidisciplinary team to turn our ambitions into reality.
There is already a significant amount of good work happening - but we're not finished.
We're ambitious about where these practices can go, and we're looking for someone who is equally ambitious about what they can build.
You won't be joining simply to inherit a finished model. You'll have the opportunity to help shape it.
What you'll focus on
Building and developing the clinical team
You'll provide leadership, coaching and mentorship to Practice Nurses and Healthcare Assistants, helping to establish a strong, capable and confident clinical workforce.
You'll have a particularly important role in building team capability and culture, creating clear expectations and supporting colleagues to develop.
Setting the clinical standard
You'll provide advanced clinical assessment and care while acting as a visible role model for clinical excellence.
You'll help establish consistent standards of practice, ensuring patients receive safe, effective and evidence-based care.
Driving quality and governance
You'll play a key role in strengthening clinical governance, audit, quality improvement and compliance.
You'll be comfortable identifying where things need to improve - and, importantly, you'll enjoy working out how to make those improvements happen.
Leading change
Change is a significant part of this role.
You'll help implement new ways of working, develop clinical pathways and bring colleagues with you as we continue to evolve the service.
We're looking for someone who can stay positive and focused when things are changing, adapt quickly and keep the bigger picture in mind.
Developing people
You'll create opportunities for colleagues to learn, grow and take ownership of their professional development.
Through coaching, mentoring, one-to-ones and development plans, you'll help build a team that is confident, accountable and proud of the care it provides.
Improving the service
You'll work with clinical and operational colleagues to identify opportunities to improve patient outcomes, access, preventative care and the overall patient experience.
You'll be encouraged to challenge existing ways of working and bring forward brave ideas about how we can do things better.
Working as one team
Our Brighton practices are increasingly connected to our wider IC24 primary care network across Sussex.
You'll have access to colleagues, expertise and support across our other practices, giving you the opportunity to learn from what works elsewhere and share your own ideas and successes.
This is a role for a builder
We're deliberately looking for someone who is comfortable with transformation rather than maintenance.
You may be at your best when:
- You're given a challenge and asked to work out how to solve it.
- You enjoy building teams and developing people.
- You're comfortable when not everything is perfect yet.
- You can adapt quickly when priorities or circumstances change.
- You have the confidence to challenge established ways of working constructively.
- You don't need everything mapped out before you get started.
- You can bring people with you through change while maintaining clear standards.
- You are resilient, pragmatic and solutions-focused.
- You see problems as opportunities to improve rather than reasons why something can't be done.
- You want to leave a service significantly better than you found it.
If you need a completely established team, fixed processes and a service that simply needs maintaining, this probably isn't the role for you.
But if the idea of building something from the ground up, developing a team and seeing your impact translate directly into better patient care excites you, we'd love to hear from you.
What we're looking for
You'll need:
- First Level Registered Nurse with current NMC registration
- Non-Medical Prescribing qualification
- Experience working within General Practice, Primary Care or Urgent Care
- Strong clinical leadership and decision-making skills
- Experience supporting, mentoring or managing clinical teams
- Understanding of clinical governance, quality improvement and audit
- A genuine interest in developing people and building high-performing teams
- Strong communication and relationship-building skills
- The resilience, adaptability and confidence to lead through change
- A proactive, organised and continuous-improvement mindset
We're particularly interested in people who have built or transformed teams, services or ways of working - whether that's within primary care, urgent care or another complex healthcare environment.
Most importantly, you'll care deeply about patients, have high clinical standards and genuinely enjoy helping people and services become better.
Why now?
We're at an exciting point in the journey.
Over the next few years we're bringing our Sussex practices closer together, creating stronger links between teams and sharing expertise and resources across our services.
At Brighton, Allied Medical Practice and Ship Street Surgery are being brought together as one group, with plans to move into purpose-built practices next year.
That investment will give us the opportunity to significantly increase our capacity and grow our patient population, and deliver exceptional care to the residents of Brighton.
We're not looking for someone to simply run the nursing team we have today. We're looking for someone who wants to help build the nursing team we'll need tomorrow.
Who are we?
Integrated Care 24 (IC24) is a leading not-for-profit social enterprise providing NHS 111, GP Out of Hours, primary care and urgent care services to more than six million patients across the UK.
Every day, our people help patients access the care they need, when they need it most.
As a social enterprise, we're driven by purpose. We reinvest in our services and our people, and we're committed to finding better ways to deliver care for the communities we serve.
We celebrate brave ideas and brave people.
Location
Allied Medical Practice, Brighton
Travel to other IC24 sites may occasionally be required to support service needs.
What's in it for you?
- £54,140.87 per annum
- 6.6 weeks' annual leave
- Access to the NHS Pension Scheme
- Learning and development opportunities to support your career journey
- Employee Assistance Programme including free 24/7 counselling support
- Access to health and wellbeing initiatives
- Blue Light Card discounts
- Colleague recognition and reward schemes
- The opportunity to play a key leadership role in a growing and transforming service
- The chance to build a team and shape a service rather than simply inherit one
- The opportunity to make a genuine and lasting impact within a growing Social Enterprise
We celebrate brave ideas and brave people.
