Job summary
We are looking for a Registered General Nurse (RGN) who is able to work flexibly and is passionate about person-centred end-of-life care. If that describes you we would love to hear from you. The post holder will have a key role in providing support to patients and families in their own homes who have palliative and end-of-life care needs. They will also contribute to planning, assessing, implementing and evaluating patient care in partnership with Primary Health Care Teams and Social Services. In return, we offer a competitive salary, a generous sick pay scheme, paid for DBS checks, funded training and development to experienced, motivated nurses who are committed to delivering high-quality care to patients who are nearing the end of their lives.
Salary: Basic salary Band 5 plus enhanced payments for weekend, Bank Holiday and night work.

Main duties of the job
Assessing, planning, implementing and evaluating care provision within your scope of practice. Monitor and evaluate end of life care plans to measure progress and ensure the effectiveness of any interventions. Maintaining concise and accurate records of care delivery within care plans and Caring for the Dying Patient documentation. - Demonstrate an awareness and understanding of the legal status and implications of the Advance Care Planning process in accordance with the provisions of the Mental Capacity Act
- Show an understanding of Informed Consent and demonstrate the ability to give sufficient information in an appropriate manner
- Work sensitively with families and carers to support them as the patient decides upon their preferences and wishes during the Advance Care Planning process
Leadership, Supervision and Appraisal
- Work without direct supervision to deliver care to patients, seeking advice from specialist nurse or members of the Primary Health Care Team when appropriate
- Work in partnership with Community and district nursing teams, Specialist Palliative Care Nurses, GPs and other health professionals who refer into our services
- Take responsibility for personal learning and continuing professional development and contributing to the learning of others
- Promote evidence-based practice through continued professional development and the sharing of best practice within the team and with others
- Appraise and supervise interventions performed by other members of the nursing team in accordance with Hospice policies
- Attend Supervision sessions and appraisal as required by the Hospice
- Promote health and safety, undertaking risk assessments as necessary
Clinical Governance and Quality Standards
- Assist in the development of clinical governance and quality frameworks through patient focused benchmarking, clinical audit and critical incident reviews demonstrating learning from multiple sources

Job summary
We are seeking an experienced, motivated and enthusiastic registered nurse to join the Colorectal Clinical Nurse Specialist team. The post holder will support the delivery of a robust, efficient and timely pathway for patients referred via the Two Week Wait (2WW) colorectal and anal cancer pathway.
The role will involve coordinating and facilitating straight-to-test diagnostic investigations, supporting the delivery of the 28-day Faster Diagnosis Standard for patients with suspected colorectal and anal cancer. The successful candidate will demonstrate a good understanding of colorectal and anal cancer pathways and recognise the importance of personalised care and support throughout the patient journey.
This role requires a high level of autonomous practice, clinical decision-making and professional judgement. Excellent communication, organisational and clinical skills are essential, together with the ability to work collaboratively with the multidisciplinary team across both primary and secondary care settings to ensure seamless patient care and pathway management.
Main duties of the job
- Provide expert clinical support, nursing care and professional leadership to patients referred through the suspected Lower Gastrointestinal (LGI) cancer pathway.
- Assess and manage new referrals within nurse-led clinics, making informed clinical decisions regarding investigations, treatment pathways and onward care.
- Coordinate and facilitate a safe, effective and timely diagnostic pathway for patients referred with suspected colorectal cancer, ensuring a high standard of patient-centred care.
- Work collaboratively with multidisciplinary teams across primary and secondary care to support seamless patient journeys and optimise pathway performance.
- Communicate outcomes and next steps effectively to patients, carers, GPs and other healthcare professionals, including the appropriate discharge of patients from the cancer pathway.
- Support compliance with national Cancer Waiting Time standards, including the 28-day Faster Diagnosis Standard and 62-day treatment pathway targets.
- Proactively identify actual or potential risks to pathway compliance, implementing appropriate actions and escalating concerns where necessary.
- Contribute to service improvement initiatives that enhance patient experience, pathway efficiency and clinical outcomes.

Job summary
East Midlands Cancer Alliance has provided 12 months' funding for Band 7 Trainee Advanced Nurse Practitioner (ANP) posts within Breast Services at University Hospitals of Northamptonshire (UHN). These roles will support the development of a sustainable Breast ANP workforce, enhance the Northamptonshire Breast Pain Pathway, increase clinic capacity across both breast units, improve patient flow through the Two-Week Wait (2WW) pathway, and contribute to achieving Faster Diagnosis Standards.
The post holder will be an NMC-registered nurse who holds, or is willing to work towards, a Master's-level qualification. Working under the supervision of experienced Advanced Nurse Practitioners and Breast Care Nurses, they will develop advanced clinical knowledge, assessment skills, and clinical decision-making capabilities.
The role will involve supporting patients across 2WW, follow-up, post-operative and mastalgia clinics at Kettering General Hospital, Northampton General Hospital, Nene Park Outpatients Department, and community settings. The post holder will work closely with the multidisciplinary team, consultant colleagues, and the Lead ANP to develop clinical competencies and contribute to the ongoing development of breast services.
Through supervised practice, reflective learning, and portfolio development, the Trainee ANP will progress towards autonomous practice and advanced clinical expertise, supporting high-quality, patient-centred care across Northamptonshire Breast Services.

- Undertake a structured clinical and academic programme towards Advanced Clinical Practitioner status within Breast Surgery.
- Provide patient care under the supervision of the Breast multidisciplinary team.
- Perform holistic assessments and support the planning, delivery and evaluation of care across the breast pathway.
- Develop advanced clinical assessment, diagnostic and decision-making skills through supervised practice.
- Support timely investigation, diagnosis and treatment in line with protocols.
- Complete competency-based training, including imaging and radiology competencies.
- Participate in consultant-led, nurse-led and follow-up clinics across the patient pathway.
- Communicate effectively with patients, carers and colleagues, including in sensitive situations.

Main duties of the job
Deliver resuscitation training for clinical and non-clinical staff, supporting the development and maintenance of skills in basic, immediate, advanced and paediatric life support, emergency response, human factors, simulation training, and recognition of the deteriorating patient.
Use a range of educational methods, including e-learning, simulation, and competency-based assessment, to support learning and compliance with Trust and national standards. Job summary
Join our well established Liaison Psychiatry team, who are diligent in creating effective care plans and comprehensive clinical strategies focused on delivering exceptional care for people with acute mental health needs.
We welcome applications from highly motivated and experienced senior psychiatry practitioners who are capable of building and supporting teams of practitioners with effective decision making and communication skills. The ability to facilitate timely interventions and provide specialist assessment that will enhance the best outcome and recovery for adults (18years+) is vital in this role.
The ideal candidate will be an experienced /registered mental health Nurse or Occupational Therapist with post registration experience of working in mental health. You must have completed practice supervision, be educated to degree level and have or be working towards a level 7 Masters qualification.

Main duties of the job
- Accept referrals of patients aged eighteen years and over for assessment, treatment and coordination with other relevant services.
- Work in collaboration with all members of the Liaison Psychiatry Service and involved agencies.
- Maintain a clinical caseload of patients with co morbid physical and mental illness.
- Provide specialist clinical input, liaison, consultancy and education to colleagues.
- Develop and provide evidence-based treatment plans for identified patients, in collaboration with the patient and referrer.
- Formulate, implement and evaluate treatment programmes.
- Promote the well-being and mental health of people, by providing relevant information and advice to them, their families and supporting agencies.
- Provide advice and support on clinical management and assessment of risk.
- Provide advice on the management of patients subject to the MHA and MCA.

Job summary
An opportunity has arisen for a registered nurse or midwife to join a small team of maternity vaccinators. You will work as part of the vaccinators to ensure eligible birthing people are offered vaccinations in pregnancy.
We currently work closely with the inpatient and outpatient maternity services ensuring that services are accessible to service users. As such you will be expected to hold a full driving licence and have access to a vehicle for work purposes.
The post will involve weekend working.

Main duties of the job
o Maintain accurate patient records in line with professional standards and local guidelines
o Report all complaints/incidents immediately to a senior member of staff and document accordingly. Signpost patients to the Patient, Advice and Liaison Service
o Participate in the Trust appraisal process each year, with the first appraisal taking place within the first 6 months from appointment
o Ensure communication channels are used appropriately and effectively to facilitate the flow of information throughout the Division
o Respond to queries or escalate to the clinical supervisor
o Responsible for providing specialist advice and technical services in relation to the care of patients considering and receiving vaccinations

Acute Frailty Unit - We are an 18 bedded unit. Our frail patients receive rapid assessment from our specialist geriatrician consultants and Supportive Discharge Service.

Byland Ward and Jervaulx Ward are both 30 bedded acute frailty wards. We specialise in nursing frail and elderly patients with complex medical and care needs. We have an expert multidisciplinary team which includes nurses, therapists and doctors working alongside a wide support team who provide patients with the care they need.

This is an exciting opportunity to influence and transform the future of Frailty, Palliative and End of Life Services across the Birmingham, Black Country and Solihull Integrated Care System, co-designing services fit for an ageing population.
Working within a complex and rapidly evolving Integrated Care System the post holder will focus upon improving clinical outcomes, safety and experience and reducing inequalities across the Frailty, Palliative and End of Life Care Pathways.
This role supports the delivery of strategic commissioning and transformation across both unplanned and planned care, ensuring services provide high-quality outcomes, value for money, and improved population health. Working with NHS providers, primary care, local authorities and system partners, the post holder will develop and implement neighbourhood health commissioning frameworks, drive service improvement, reduce health inequalities, and identify opportunities for joint commissioning. Main duties of the job
Screen & identify patients eligible for clinical trials and to maintain detailed knowledge of the protocols for the clinical trials associated with the designated site-specific groups.
Provide verbal and written information and to offer support to patients and carers for the duration of their involvement in a clinical trial. To assist in the informed consent process, receiving informed consent where appropriate. To be a point of contact for patients and carers for the duration of their involvement in a clinical trial.
With appropriate training, to take and process clinical samples (e.g. blood samples) for studies, co- coordinating tissue sample collection and dispatch to relevant department or trial centre as appropriate.
Assist in the provision of background information about potential clinical trials to obtain approvals from Research Governance and Local Ethics Committees.
Working in conjunction with research clinicians, nurses, pharmacists and other support staff within the East and North Hertfordshire NHS Trust
To work at all times according regulations as described in ICH Good Clinical Practice and EU Directives, and to the most current guidance relating to Research Governance and Research Ethics in each Trust.
Participate in the informed consent process, CRF completion, safety reporting, protocol compliance, monitoring and auditing of clinical trials.

Successful candidates will join our friendly, supportive teams and be part of a team reflective of the acuity of the patient group. Regular staff engagement, feedback session and supportive ethos is actively practiced.
We are looking to recruit staff who are enthusiastic and committed to providing high quality evidence based care. We have 7 Intensive care cots, 7 high dependency cots and 12 special care cots.
We provide care to all new born babies within SSBC Neonatal Network from 23/40 weeks gestation. We are a cooling center for the west midland Neonatal Network. The post holder must be highly motivated and want to develop the service as well as their own personal skills and knowledge.
All applicants should be flexible providing 24 hour, 7 days a week service, working a shift pattern of days and nights to meet the needs of the service. We can consider flexible working and part time hours.
We have a Practice Education team, Quality and Experience Nurse team and SIM team to support your transition, ongoing education and support.
We advocate staff support and wellbeing with Professional Advocate Nurses and Clinical Psychologist.

Main duties of the job
Responsible for supporting the Ward Manager in the delivery of consistently high standards of care to meet the service needs of the patients and the Trust objectives. The post holder will be a role model of the Barts Health values and key behaviors to all members of the team and the wider MDT. As a senior member of the Ward team, the band 6 will be a competent confident practitioner who can care for, and manage the deteriorating and acutely ill patient, escalating concerns to the appropriate medical and outreach teams. The post holder must have competent management and facilitation skills in order to coordinate and supervise the activity of ward staff, promote multi-disciplinary working and foster a supportive, learning environment. In the absence of the Ward Manager the post holder will assume responsibility for the effective operation of the ward, including audit and budgetary roles, that the ward environment is maintained in a clean safe way, and all staff follow the Trusts infection control policy, dress code and all other Trust policies and protocols. As a Registered Nurse the post holder is expected to consistently act in accordance with the NMC Code of Professional Conduct.

Work collaboratively as part of a multidisciplinary team, maintaining strong communication with colleagues, service users, carers, and partner agencies.

Act as a mentor and role model to junior staff and students, offering supervision and guidance.

Maintain accurate and timely clinical records in line with trust policies and professional standards.

Support service users in their recovery journey, promoting independence, social inclusion, and overall well-being.

Apply up-to-date knowledge of relevant NMC standards and ensure high standards of professional practice.
