Job summary
An exciting opportunity has arisen for a year-long secondment into a clinical lead role across Hampshire MHSTs (as part of the wider HIOWH MHST provision). This is a fantastic opportunity to support the development of MHST as we expand across increasing school numbers over the coming year.
You will have the opportunity to work as part of the MHST leadership group and share your voice in regard to local geographical needs. We are looking for someone who is passionate about early intervention, working with school communities and advocating on behalf of our MHST teams, partnership agencies, young people, schools and families. Knowledge of MHSTs is essential as supervision and governance support will be key areas of the role.
In partnership with operational colleagues, this role also brings an opportunity to oversee the implementation of the Whole School Approach (WSA) offer in Hampshire. This is well embedded with clear governance systems in place. Alongside other clinical leads there will be opportunity to support the development of the workforce including psychological practitioners and practitioner psychologists (where relevant).
The post is advertised as full-time but we will consider part-time and flexible working requests. The position will be based in the Hampshire locality for administrative and day-to-day purposes, although work on the WSA and wider leadership responsibilities require flexibility to travel county-wide, including to Southampton and Portsmouth.
Main duties of the job
HIOWH NHS Foundation Trust currently has over 20 MHST teams working with children, young people, schools and multi-agency partners across our geography including on the Isle of Wight, and in Southampton, Portsmouth and Hampshire. This service is set to expand significantly over the coming three years.
The development of these services is in line with commitments set out in the NHS 10 Year Health Plan for England.
MHSTs work with school and college age children up to 18 years of age, and are a school and college-based service. Therefore, the majority of work and interventions will be completed within an education setting, face to face with young people.
Trust sites are also available to use, particularly during school holidays.
MHSTs work in close alignment with Child and Adolescent Mental Health Services (CAMHS) and with other partners including school nursing, public health, educational psychology, and inclusion and learning support teams to name a few.
The post holder, under supervision and with support, would offer sound knowledge and practice skills in delivering evidence-based interventions for children and young people in education settings with mild to moderate mental health problems. The post holder would also be supporting and facilitating staff in education settings to identify and where appropriate manage issues related to mental health and wellbeing through consultation and advice.

Job summary
We are looking to recruit enthusiastic and motivated Nurses to join the Hampshire and Isle of Wight Healthcare NHS Foundation Trust School Age Immunisation Service.
We have two positions available:
- 22.5 hours a week term time only
- 22.5 hours a week all year round
You will participate in the organisation and delivery of the childhood immunisation programme for school aged children across Hampshire, under the supervision of the immunisation lead.
Hampshire & Isle of Wight Healthcare is committed to improving the quality of child health services across Hampshire and Isle of Wight. Your role will include working in partnership with other services such as the School Nursing Service, Education service (including Elective Home Educated). You will demonstrate clinical expertise and governance in all aspects of the service, including safeguarding children, whilst being supported by robust organisational supervision, guidelines, and policies.
You must be either RN qualified and have current NMC registration or have relevant professional registration, e.g. Midwife.
Hours of work are Monday to Friday 8:00am to 4:00pm. Happy to discuss flexible working as to which days of the week that you work but due to the nature of the role the hours each day will need to be as stated.

Main duties of the job
To work as part of a team to deliver the commissioned school aged immunisation programme to children aged 4-18 years living and attending school within Hampshire and Isle of Wight area.
Demonstrate partnership working with the 0-19 Public Health Nursing Service, Child Health Information Service, Primary Care, Public Health England and Education, including the Elective Home Educated community to increase the uptake of immunisations.
Undertake delivery of immunisation sessions in schools and community clinics and complete the associated data collection
Understand the relevant chapters in the Immunisation Green Book (DOH) and understand storage and carriage instructions for immunisations including maintaining the Cold Chain at all times.
Understand the pre-requisites of giving an immunisation, understanding the contra-indications, pre and post care and the relevant advice to give to the child/young person.
Understand all aspects of consent and adhere to trust policies and procedures.
Participate in audit to ensure services are clinically effective, providing best care and using up to date evidence based practice.
To be aware of local and national research and development programmes relevant to immunisation to maintain an effective workforce capable of meeting its objectives.
Demonstrate computer literacy in order to enhance robust communication and data analysis
Promote and support an effective team ethos through the use of excellent communication skills

We are seeking an experienced nurse with strong leadership skills to deliver high-quality care for children and young people with complex health needs. You will lead clinical practice, manage caseloads, and support colleagues and school staff while making a real difference every day.

Our team supports nine special schools across Hampshire, Portsmouth, Southampton, and the Isle of Wight. Main duties of the job

We are recruiting for a Clinical Nurse Specialist to join our Community Mental Health Team on the Isle of Wight.

Job summary
The Safeguarding Specialist Nurse/Practitioner will be part of a service dedicated to supporting and promoting the welfare of children and adults. The role involves assisting the Named Nurse/Professional to embed a Family Approach towards safeguarding across Hampshire and Isle of Wight Healthcare (the Trust) for Specialist Services to include community CAMHS, inpatient CAMHS and Adults Forensic units across the Trust being based in a hub within Safeguarding Support Team. There will also be a requirement to provide generic safeguarding support as part of the Safeguarding Support Team. The post holder will actively contribute to family approach, safeguarding training across the Trust, preparing and delivering training sessions for staff groups across Hampshire and the Isle of Wight.
Main duties of the job
The post holder will also work in the multiagency arena supporting and working with the Named Nurse/Professional as required in the completion of safeguarding reviews, internal and external, including Child Safeguarding Practice Reviews (CSPR), Domestic Abuse Related Death Review (DARDR), Adult Safeguarding Reviews.
A key aspect of the role is to provide tenacious safeguarding leadership, working within multi-agency frameworks to improve outcomes for children, young people, adults, and families. This includes identifying and addressing risks, supporting learning, and maintaining the Trust's reputation within statutory frameworks.
The post holder will also provide formal expert supervision and support to teams and services across the Trust, working within governance frameworks and escalating safeguarding concerns to the Named Nurse/Professional for resolution.

The postholder will work as part of the Integrated Community Nursing Service on the Isle of Wight, delivering high quality physical nursing care to patients in their own homes as part of a rapidly developing community based model. If so, we invite you to join our Endoscopy team as an Endoscopy Nurse to deliver high-quality, patient-centred care. This exciting opportunity offers a healthy work-life balance with a Monday to Friday service operating between 08:00 and 18:00, with flexible working patterns available. As part of our supportive and welcoming team, you will play a key role in the admission, care, recovery and discharge of patients, including those undergoing conscious sedation. You will work closely with the multidisciplinary team to ensure safe, effective and compassionate care is delivered at every stage of the patient pathway.

Main duties of the job
Maintains safe and high standards of practice in accordance with the NMC Code of Professional Conduct.
Assessment, delivery and recording of direct & indirect care to patients undergoing diagnostic endoscopic procedures.
To setup equipment required and assist during endoscopic procedures.
Management of biopsies and specimens according to local procedures.
Delegate to registered nurse/ other team members when fit to do so.
To participate in maintaining standards as set in the Global Rating Scale for Endoscopy and JAG accreditation.
Expansion and ongoing development of their nursing skills through personal development and subsequent skills transference to other members of the team and student nurses.
Administration of drugs as prescribed according to Trust policy and NMC regulations.
Contributes to the development of the Unit focusing on the delivery of timely, efficient and high-quality services.
Demonstrate knowledge of decontamination of endoscopes, clinical and sharps waste, COSHH, Health and Safety and Infection Control guidelines
Effective verbal and written communication with patients and staff, liaising with the multidisciplinary team in the delivery of care

Main duties of the job

An exciting opportunity has arisen in the Winchester District Nursing Teams.
We are seeking to recruit a full-time Band 6 Enhanced Wound Care nurse into our teams
You will be working under the supervision of Clinical Nurse Specialists in Tissue Viability. You will receive support and supervision to complete a range of competence-based assessment by an experienced and highly visible TV team and gain experience in clinical practice. The successful candidate will act as an expert resource to community and care home staff. The applicant will develop the ability and willingness to participate in some tissue viability education programmes with other team members. The role also provides support to an established tissue viability Link Nurse cohort in their geographical area.
Your main role will be working within the community nursing teams in the locality to support registered and unregistered staff to achieve competence, skill and experience with Tissue Viability. In addition, you will be supporting the senior staff to provide management and leadership to the teams.
You will be expected to maintain clinical skills and competence, you will have the ability to work unsupervised, be able to prioritise your workload, have excellent communication skills and the interest and enthusiasm to promote best practice in tissue viability. As a team player, you'll be able to prioritise your work in order to meet deadlines and have a customer focused approach
Main duties of the job
The primary role of the Enhanced Wound Care Nurse is to work across a designated geographical area, providing support in the delivery of evidence based high quality specialist wound / skin care, advice and education to a range of staff within the Integrated Community Winchester District.
The Enhanced Wound Care Nurse in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people With complex tissue viability issues.
To undertake a comprehensive competence programme that will ensure high levels of knowledge and skill in the area of tissue viability care across the Community team.
To work in clinic settings, patients' homes and in residential care settings to support general clinical staff in providing tissue viability care
To act as a role model and expert in the prevention, treatment and care of tissue viability conditions
To provide , with others, management and leadership support to the team.

We are looking for a Pulmonary Rehabilitation Practitioner to join our multi professional team.
This is a full time post but we welcome applications from those who can work 22.5 hours or more.
The Southampton COPD team works together with University Hospital Southampton to provide an integrated approach. We provide a PR Pathway & Community COPD clinics for people with complex COPD.
The service runs at Bitterne Health Centre, Royal South Hants Hospital and Adelaide Health Centre plus community sites.
We are looking for an enthusiastic practitioner who will actively promote the COPD pathway & primarily support the delivery of pulmonary rehabilitation in the community. They will independently manage & deliver evidence based PR exercise programmes to patients in different community settings, including digital programmes. The post holder will also support community respiratory clinics across the city supporting consultants & specialist practitioners in delivering complex COPD care.
The successful candidate will work as an active member of the team in supporting & leading patients during their time on our programme. They will have in depth knowledge of exercise & its prescription, being able to lead & progress patients through their programme. With this knowledge they will also be a resource for training of other team members & the development of the service.

Main duties of the job
Complete referral triages, assessments and review of patients for Pulmonary Rehabilitation
Implement the delivery of PR programmes for groups in face to face and virtual settings.
Independently manage allocated PR venue, including caseload, logistics, equipment and staffing.
Support patients in line with national and local guidelines, monitoring progress, modify and adapt treatment programmes as required using SMART goal setting.
Work independently, within level of competency, to undertake assessments and interventions, recognising their own limits and request supervision of senior clinicians to ensure the delivery of safe care.
Be aware and take appropriate action of clinical deterioration of patients, emergencies, internal and major incidents which affect service continuity.
Provide direction and guidance for patients and carers on treatment options; physical, emotional and social.
Make person centred, evidence-based judgements in partnership with others involved in the care process to ensure compassion, dignity and high-quality care.
Respond to patient, relative and carer's concerns and take action as required and escalate appropriately.
Take a lead in working with 3rd party organisations to facilitate pathways to support long term behaviour change.

The postholder will work as part of the Integrated Community Nursing Service on the Isle of Wight, delivering high quality nursing care to patients in their own homes as part of a rapidly developing community based model. Job summary
HIOW Healthcare is looking for an inspirational nurse to deliver a high standard of quality care to patients within the community setting. Community Nurses work autonomously and as part of a team and care is delivered in the patient's own home environment. Our nurses are responsible for a manageable caseload, working across the city ensuring our patients are receiving the best quality care from the comfort of their own homes and thereby avoiding hospital admission.
HIOW Healthcare Community Nurses work with adults who need support for a multitude of health conditions. These include:
Long term conditions management
Acute wounds and complex wound care
Palliative and end of life care
Facilitating discharge from hospital, including drain management, administration of prophylactic medication
Continence care and bowel care
Administration of a variety of medications
Although a lone worker, you will be supported by a team who are always available, to allow you to develop your existing skills into becoming an autonomous practitioner in the community. You will be provided the training you require by attending appropriate training and working with our Educators in Practice and other staff within the team.
This is a part time position (27.5 hrs pw) working within a team based at Bitterne Health Centre, Southampton. Our working patterns are across 7 days. Our core shifts are 0800 to 1600 and 1400 to 2000 hrs. There is also a requirement that you will work 1 in 3 weekends as well as Bank Holidays.
Main duties of the job
A registered nurse working as part of a multi-disciplinary Locality Health Team affiliated to a cluster of GP Practices providing nursing care to the Practice population both in the patients home or clinic setting based on individual care needs.
Delivering on the organisational objectives to meet individual health care needs through high quality, safe and effective services to avoid unnecessary admissions to secondary care, support early discharge from hospital, effective case management for patients with complex long-term conditions or following an acute health episode and individualised care for the patient and carers at the end of life.
The post-holder will be required to demonstrate a high degree of autonomy and professional accountability, working within the Nursing and Midwifery guidelines to have responsibility for the management of a defined Community Nursing caseload within one or more GP practices in the absence of the Senior Community or Clinical Manager.
