Main Responsibilities:
- Work across B&NES to contribute to the identification of population health needs in line with the organisations policies and priorities.
- Carry out public health work delegated by the Specialist Community Public Health Nurses, (School Nurse) and Clinical Team Leaders, as part of a skill mix team, led by the Professional Lead.
- Work in partnership with children, families, parents and carers to identify health needs in order to negotiate appropriate intervention from the B&NES Child and Family Health Service or other relevant agencies.
- Deliver drop-ins, health education sessions, needs-led interventions, information provision and signposting/introduction to other agencies to support and empower children, young people and families to take ownership of and manage their health care.

Main Responsibilities

Join our small, dedicated Jessie May Care Team in the Bath, Swindon and Wiltshire (BSW) area, where you will provide specialist care to children and young people in the community.
Our respite sessions last 3.5 hours long, this is to allow parents and carers to take a break from caring duties. We are looking for two part-time candidates that can predominantly work our teatime sessions, these sessions are usually 3:30/4pm 7:00/730pm, with travel time included around your shift timings. We can be flexible of working days and are happy to discuss your personal preferences whilst ensuring that they meet the service needs.
We support families who live in B&NES, Swindon, and Wiltshire, and whilst we will aim to roster you to support a family who lives nearest your work base location there is an expectation with this role that your will have to travel long distances across BSW.
You will provide skilled holistic nursing care to children and young people who have complex medical needs within the family home or other environments, this may include visiting a child in hospital.You will also provideemotional support and advice to the children, and parent/carers on the caseload, and will be responsible for the planning, implementation and the evaluation of individualised care during each respite visit in the absence of the parents or carers.
You will be skilled in communicating, establishing and maintaining good relationships with the children, young people and their families on the caseload and work effectively within the Jessie May Care Team.
This is an extremely rewarding role and we have 2 part-time positions available working 12.75 hours per week.
We have base locations across BaNES, Swindon and Wiltshire so you will be allocated a base closest to your home however, as this is a community based role travel is required and therefore willingness to travel and a valid UK driving licence is essential for this role.

Main Responsibilities

- Support the palliative care needs of children and families
- Maintain clinical skills and competency on a range of complex health needs, to include (but not exclusive to): tracheostomy management, enteral feeding, seizure management, long term ventilation, oral suction, nursing observation and assessment.
- Follow any emergency procedures/guidelines for the child or young person receiving care.
- Administer medication in line with medicines administration charts and service policy.
- Assist with dressing, undressing, personal hygiene, toileting in a sensitive manner, encouraging the child or young person to be as independent as possible within their capability.
- Ensure the child and their family are at the centre of everything that we do. Provide care that is consistent with their wishes and ensure the voice of the child is heard and underpins everything that you do.
- Actively listen to the child and family, and offer emotional support using empathetic communication.
- Keep records of all care given and observations made in accordance with HCRG Care Group policy.
- Identify the need for any additional emotionalsupport for the child and the parent/carer and communicate this in an appropriate manner to the care team and Jessie May team around the family.
