Job summary
Supporting High-Quality Diabetes Care Across the Community
This is an exciting opportunity for an experienced nurse with a passion for diabetes care to join our community-based specialist team. The role focuses on delivering expert assessment, treatment, education, and support to patients with diabetes, their families, and carers.
The successful candidate will work within a multidisciplinary team while exercising a high degree of professional autonomy in the management of patients with diabetes. They will contribute to the delivery of safe, effective, and patient-centred care, support service improvement activities, and work collaboratively with colleagues across Wiltshire to ensure consistently high standards of practice in line with local and national guidance.

As Lead Nurse, you will provide expert clinical leadership for children and young people with complex respiratory needs, including those requiring long-term ventilation, tracheostomy care and highly specialised respiratory support. Main duties of the job
As a Band 6 Sexual Health Nurse, you will deliver a comprehensive contraception and sexual health service as part of a Level 3 integrated service. Your role will include competencies in implants, IUDs, or sexual health advising. You will manage your own clinics ensuring safe, high-quality, patient-centred care, provide STI testing, diagnosis, and treatment including HIV care support. You will deliver and manage contraceptive methods, including implants and IUDs with training and support patients in making informed choices and managing complex care pathways. Main duties of the job
The Community Staff Nurse in the School Nurse Team is a registered general nurse working as part of the public health nursing team in providing preventative and supportive health care to children, young people and their families.
