We are looking for a Bank Senior Referral Clinician, who will work within the Single Point of Access team at HCRG Care Group, working as part of a skill-mixed team, ensuring a high standard of service delivery.

As a Senior Referral Clinicianyou will be responsible for the digital screening of referrals to ensure effective prioritisation, safety, and accuracy. This involves making clinical decisions regarding the eligibility for access to services, applying a no wrong door approach to ensure inclusivity and seamless care pathways.

The role focuses on managing complex referrals, including those involving safeguarding concerns and intricate health needs, ensuring a robust and safe response to the needs of children and families.

The successful candidate will bring clinical expertise and substantial experience in childrens health and development.

As a Senior Referral Clinicianyou will clinically supervise and inspire the team to pursue excellence in care delivery, while providing clinical guidance and day-to-day leadership to uphold the quality and safety of services.

This role will be based at Wonersh House in Guildford. Hours will 9am - 5pm. There is a four week training plan which will be delivered in person, once completed, there will be flexibilty for hybrid working.

• Use Clinical expertise to triage referrals to the appropriate services Be able to liaise with services regarding referrals.

• Support the team in service development and transformation.
• Be aware of the continuous service changes and be able to apply these daily.
• Discuss appropriate care pathways with referrers and families to understand their current needs, expectations of the service and to ensure utilisation of the most appropriate service.
• Support clinical and day-to day leadership of the team including induction and development of new colleagues

The Ideal Candidate

Essential

• A qualified Nurse or Allied Health Professional e.g. RCN, SCPHN, SLT, OT, Physio or equivalent, and clinical registration with a recognised professional body (e.g. NMC, HCPC)
• Postgraduate/post-qualification experience working with Children and Young People for minimum of 3 years
• Excellent communication skills, able to communicate with families, young people and other professionals verbally and in writing.
• Exceptional clinical risk assessment and management skills. A sound understanding of current evidence and best practice in child health and care.

Desirable

• 1 year minimum experience of providing clinical supervision to others
• Experience of working as a senior member of an MDT
• A positive, motivated and passionatabout providing a high quality holistic approach to referral management.

Main duties of the job

You'll collaborate with School Nurses, parents, and partners to meet the health needs of children, managing your workload under supervision. Working under the delegation of a School Nurse, you'll deliver components of the Healthy Child Programme 519 (up to 25 for SEND) across mainstream schools, homes and our Family Hubs, providing targeted interventions and groups for children and young people at risk of poor outcomes or with complex needs.

You'll join a skill-mixed Healthy Family Team of Health Visitors, School Nurses, Support Practitioners and Engagement Workers, in a service rated "Good" by the CQC and focused on outcomes, not just numbers.

Main Responsibilities

As a Children and Young People Practitioner, you'll:

• Work in collaboration with School Nurses to meet the health needs of children and young people, helping them develop self-care, autonomy and decision making, including how to access health services.
• Deliver aspects of the Healthy Child Programme 519 pathway as delegated by the School Nurse.
• Provide brief intervention and solution-focused support on issues including emotional wellbeing, exploitation, sexual health, relationships, substance and alcohol misuse, risk taking and healthy lifestyles, in schools, homes or Family Hubs.
• Deliver targeted health promotion campaigns through school drop-in clinics, assemblies and community outreach.
• Work with parents, carers, and statutory and voluntary partners to identify and respond to young people's and families' needs.
• Manage your own workload under the supervision of the School Nurse and contribute to Key Performance Indicators and outcome measures.
