An exciting nursing opportunity has arisen for the right candidate to work in the Integrated Community Neurology Team as a Neurology Nurse, at East Coast Community Healthcare in the Great Yarmouth and Waveney area. We are looking for an enthusiastic forward-thinking clinician with a strong interest in Neurology and experience in managing patients in the community.
Following a comprehensive training programme, we require you to work autonomously, be confident in managing patients independently, and possess good integrated working skills with the wider MDT.
The Integrated Community Neurology Team aims to provide excellent clinical management and community rehabilitation to people with Parkinson's Disease, Motor Neurone Disease and Multiple Sclerosis. We work to deliver multidisciplinary, evidence-based practice that improves outcomes for people with long-term neurological conditions.
The Community Neurology Nurse will provide specialist skills and knowledge to patients, carers, professional and non-professional staff in Great Yarmouth and Waveney ; to support the already existing Neurology service in its development and management to work towards a seamless integrated neurology service.
Interviews are currently expected to take place on 19th November 2026, although this date may be subject to change.
Main duties of the job
The post holder will work autonomously and have responsibility for the assessment, development, implementation and evaluation of planned programs of care in conjunction with the multi-disciplinary team for patients with the neurological conditions of Parkinson's Disease/ Multiple Sclerosis/ Motor Neurone Disease, within the community setting; in accordance with evidence based practice and local policies/ protocols/ guidelines.
Supporting the Neurology Nurse Practitioner for Neurology Services in the implementation and day to day management of clinics, home visits, and acting accordingly to urgent reviews when required; working alongside Neurologists / General Practitioners on medication / therapy outcomes.
Providing education and monitoring of injectable / oral disease modifying therapies in specialised clinics offering regular review and support. Liaising with the Neurology Nurse Practitioner on the ongoing development of the Disease Modifying Therapy pathways in conjunction with secondary care. Supporting MS patients through their Disease Modifying Therapy journey.
Provide advice, information, support and clinical practice to newly diagnosed and existing neurological patients and their carers in primary, secondary and tertiary care.

To use and have direct involvement in existing audit, benchmarking and evaluation tools within the neurology service.

Keep up-to-date with general specialist training, reading relevant journals and attendance at study days to further develop knowledge and competencies relevant to role.

Play an active part in the development of local policies and guidelines by contributing to multi -- disciplinary working groups.

Job summary

Do you want to progress your nursing career with a supportive and encouraging team, delivering person centered care in a clinic and occasionally a home setting; utilising health coaching to support patient involvement in shared decision making.
Do you like the idea of forming longer term therapeutic relationships with patients and their families?
The PCH is a multidisciplinary team delivering health care to people in Great Yarmouth. The combined skills of the team mean they are able to respond to the changing needs of patients and support those who may have previously been admitted to hospital to remain at home. The leg ulcer clinic is a valuable asset within the PCH delivering specialised leg ulcer care to patients who are not housebound.
The clinic operates Monday to Friday 08:30- 16:30 excluding bank holidays.
This post is offered up to 37.5hours per week. We are able to support part time and flexible working arrangements.
Main duties of the job
As a Clinic Nurse you will be providing an excellent standard of person centered care to adults with a lower leg wound in a clinic setting and occasionally their own home.
You will participate in the planning and efficient running of the leg ulcer clinic at Joseph Kittle centre Martham, Northgate hospital and Shrublands surgery.
This role includes triage of incoming referrals, determining clinical priority, completing holistic patient assessment, specific wound assessments including recording dopplers, diagnosis and treatment plan, for patients referred to the service and supporting patients to self care using health coaching.
You will be supported to complete the leg ulcer management course delivered in partnership with Accelerate CIC, and a health coaching qualification. You will be encouraged to continue your own professional development and contribute to service improvement. You will be part of the PCH support for students; those on an apprenticeship route and newly qualified nurses. Once experienced you will be a knowledgeable resource on leg ulceration to the wider PCH; other ECCH clinicians; primary care and other partners.

Job summary
An exciting opportunity has arisen for a Sister/ Charge Nurse/ District Nurse to join our Primary Care Home team delivering high quality nursing care to the population of Lowestoft. If you are motivated, adaptable, forward thinking and you see yourself leading within community healthcare, then this could be the role for you.
The role contains variety every day with a mixture of face to face clinical practice, leadership and management. You will be responsible for the line management of a number of clinicians, have the opportunity to participate in HR processes including recruitment, appraisals and supporting junior members of the team. We welcome applicants who have a desire to improve clinical practice.
This role sits in a multidisciplinary team providing community services 7 days per week 07:00- 20:30 hours
The core nursing shift is 08:30- 16:30 other shifts included in the rota 07:00- 15:00 and 12:00 -20:00. Weekend and bank holiday working on a rotational basis.
The post is offered up to 37.5 hours per week.

Main duties of the job
- To provide clinical leadership, operational management and professional support to community nursing within Primary Care home
- To work with other members of the multidisciplinary team to provide planned and unplanned care
- To provide expert clinical case management for patients living in the community who require nursing care
- To work autonomously at a level appropriate to clinical competence and within scope of professional practice
- To carry out rapid assessments in patients own home with the aim of preventing admission to the acute hospital. These assessments will take a holistic ethos including physical, physiological and social aspects of need.

Lowestoft Primary Care Home is a multidisciplinary clinical team which includes other Nurses, Occupational Therapists, Physiotherapist, Assistant Practitioners, Registered Nurse Associates, Rehab Support Workers, Healthcare Assistants and Phlebotomists. The team work in partnership with Primary Care, Social Services, social care providers, other specialist community health teams and voluntary sector providers to ensure patients are able to remain in their own homes whenever possible.
