Job summary
The Perinatal CMHT are looking for an enthusiastic Band 6 CPN to join our team in South Derbyshire, based at the Radbourne Unit, Derby. Experience of the Perinatal service is not required but an keen interest in the area is essential. The Post is for a specialist assessor role who will have previous experience of completing in-depth assessments and will be responsible for assessing a range of mental health difficulties suffered by pregnant women and those with infants up to two years old. this will include identifying risk factors, evaluating the parent-infant relationship, and determining the most appropriate treatment pathway. This may include signposting and referring on to psychological therapies, primary care, third sector organisations and also referrals within the perinatal mental health for specific pathways
This post is 37.50 per hours a week, and is a Fixed term 12 Month contract
The perinatal mental health service cares for women who are suffering with moderate to severe mental health problems, from the end of the first trimester in pregnancy up until the baby is two years old.
The service works with women and their families from Derby City and North and South Derbyshire .
We operate a flexible hybrid working model, with staff working from home and the office on a rota basis. Through investment in digital technology, we offer both video and face-to-face appointments, providing patients with greater choice in how they access our service.Main duties of the job
You will work as a member of the multi-disciplinary community team offering a comprehensive perinatal mental health service to women experiencing mental health difficulties in the perinatal period.
You will be responsible for facilitating assessments of women with a range of mental health difficulties and where appropriate signposting to other services. you may support in the triaging of referrals into the team on a rota basis. We also operate a dedicated advice line where you will be able to support and advise other professionals who are thinking of referring into our service.
You will need to work collaboratively and communicate effectively across primary care, secondary care and children's social care agencies.
We are committed to the training and development of future nurses who we have on regular placement and you will be expected to contribute to this through mentorship and facilitating learning opportunities.

Our service has been accredited for many years by the Royal College of Psychiatrists Care Quality Network for Perinatal Mental Health, as we have met and adhere to the required standards for delivering perinatal care in the community, ensuring best practise is adhered to.
We have a weekly MDT where you will be able to discuss referrals and the recent assessments you have carried out along with any support you may need to outcome these.
Local induction, along with the Trusts corporate induction, will be provided and there are opportunities available to access training to support the development of your knowledge and skills in this area. Induction will also include shadowing experienced members of the team and regular clinical, managerial and team supervision.

Derbyshire Healthcare is transforming its Mental Health Rehabilitation offer and developing its Community Rehabilitation Services. We are looking for practitioners who want to be part of the new service where occupational and psychological approaches arecentral to successfully working with people who have complex mental health needs.
You will have a passion for co-production and working collaboratively with the voluntary, community and social enterprises in Derby and Derbyshire.
We want people to join our team whose clinical ability is underpinned by values-based practice and structured with evidence-based interventions. These exciting and innovative roles are an opportunity to work with service users using meaningful activities and to help people build their skills and well-being in a person-centred, recovery focused way.
You will work across the MDT and be involved with assessment, service development and providing clinical interventions specific to your professional speciality and past experience.
Main duties of the job
To work across the adult rehabilitation services, offering senior clinical leadership and having clinical responsibility across the service.
Be responsible for the assessment, planning, implementation, and evaluation of evidence-based programmes of care to the group of service users supported by the community rehabilitation service.
Use advance clinical skills and expert knowledge to assess mental health, social and occupational needs of the defined client group.
Work within a multidisciplinary team (MDT) structure that ensures effective care planning and treatment. This will include care coordination and carrying out treatment activity with other professionals and external agencies.
Being responsible for the planning, allocation and delegation of duties and care priorities to the MDT.
Provide support, advice, education and information to services users and their relatives/ carers as needed. This will include involving service users and carers in the ongoing planning and delivery of care.
Work collaboratively across the MDT, reporting, and supporting the team caseload as required.
Develop and maintain effective partnerships with stakeholders, professionals, and agencies across the ICB footprint and beyond to ensure effective and safe care and discharge.

Based in Bakewell, our service promotes a person-centred and non-pharmacological approach to support older adults with functional health , who may be experiencing or approaching a crisis in their mental health.
We cover the High Peak and North Dales area of Derbyshire.
The aim of the service is to assist in maintaining the independence and dignity of those living with dementia, with the ultimate goal of preventing hospital admission where possible.
In addition the IRHTT can support early discharge from hospital, working closely with carers and relatives to facilitate this.
The service operates a service 365 days a year,9am-5pm seven days a week and Bank Holidays.
Main duties of the job
You will:
- Employ a wide range of skills, building on and developing their expertise in managing complex cases as part of a wider multi-disciplinary team, which includes Occupational Therapy, Physiotherapy, Nurses, Specialist Support Workers, Psychology and a Consultant Psychiatrist.
- Be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.
- Be expected to travel independently across the catchment area of the service and occasionally outside of this if required.
- Work a shift pattern facilitating the service, which operates 7 days per week.
- Undertake the Duty role; triaging referrals, allocating visits, preparing and delivering handover, ensuring colleagues are "safe and well".
- Work as part of a MDT; liaising with a multitude of different professionals and agencies.
- Complete initial assessments of patients.
- Apply knowledge of the Mental Capacity Act and Mental Health Act when required.
- Assess risk and formulating safety plans accordingly.
- Formulate person-centred care plans focusing on non-pharmacological interventions.
- In-reach to inpatient wards, attending 117 meetings.
- Visit patients in their own home and those in 24 care.

You will work under the lead of the Specialist Community Public Health Nurse (SCPHN), working within the public health framework (0-19), to meet the prioritised holistic needs of family's children & young people.
For this post you will need to be proactive and enthusiastic as you will be key to assessing the health needs of children and young people and implementing and evaluating programmes of intervention for a defined school health caseload.

Main duties of the job
Assessing the health needs of school age children and young people.
Implementing and evaluating programmes of intervention for a defined school age caseload.
Work within the 0-19 framework ensuring a seamless service to the family.
To work collaboratively and in partnership with other agencies and partners, ensuring a seamless delivery of services.
To hold child protection responsibilities within a caseload with support/supervision from the named SCPHN in line with service delivery and local policies.
To support, mentor and supervise appropriate members of the team.
