We are currently recruiting for an Out of Hours experience Band 5 Community Nurse to join our busy yet rewarding team.
We are seeking to recruit someone with previous experience of working in a Community Nursing team who can provide leadership and support to develop staff to reach their full potential.
The Out of Hours District Nursing team works overnight from 19:00 pm to 7:00am. This Post is for 8 x 4 hrs 19:00-23:00 Hours aweek.This will be different nights and can be discussed at interview.
We believe in care that treats the whole person, not just the condition. We enable patients to stay at home longer and leave hospitals faster. Our community nurses work closely with members of the wider multi-disciplinary teams, including our GP's, local hospices, complex care nurses, health and social care coordinators, specialist nurses and therapist colleagues.

Main duties of the job
- To work in conjunction with the Clinical/Team Leader, Community Nursing Band 6 and all staff within the team,to holistically assess the needs of patients andcarers, and to work in partnership with the patient and family to plan, implement and evaluate programmes of nursing care.
- To be able to undertake first visits and take any immediate action, reporting back to the Clinical/Team Leader,Community Nursing Band 6 and Community Matron for further discussion and/or assessment.
- To establish working relationships with other professionals and voluntary agencies to achieve health aims.
- To undertake practice assessor and supervisor training support students and new staff to the service.

We are looking for a qualified Level 3 part-time Nursery Nurse to join our team.
Kiddies Cabin Day Nursery, located within Berkshire Healthcare NHS Foundation Trust, is looking for aLevel 3 qualified part-time Nursery Nurse to join our friendly and supportive team.
We are open Monday to Friday, from 7:30 to 18:00, closed on bank holidays and during the Christmas and New Year period. This role will be 15 hours per week across two days at 7.5 hours per day.
We pride ourselves on providing a positive, nurturing environment where children can learn, develop and thrive.
Youwill be responsible for a group of children, providing daily care, activities, and completing developmental reviews. You will support children from 6 months to reception age, ensuring high standards of care and learning in a safe, stimulating environment, in line with the Early Years Foundation Stage
You will support and inspire children to become independent, creative, and confident individuals. By planning engaging and stimulating activities, you will help children achieve their full potential. Also building strong relationships with parents is essential.
You will also maintain a positive, caring approach, contribute creative ideas, and work collaboratively within the team and bring experience in working with children with special needs, working closely with the nursery team and the area SENCO to provide the best support.
Main duties of the job
- Encouraging all areas of the children's development within Early Years Foundation Stage (EYFS) with theappropriate activities, play opportunities and actions through imaginative play and innovative programming.
- Compiling andmaintainingdevelopment records on children and report to the Team Leader at agreed intervals.
- Providing a stimulating, friendly and caring environmentat all times.
- Demonstrating good practice with regards to special needs and inclusion, working with the nursery and area SENCO asappropriate.
- Being a key carer carrying out all related responsibilities in building relationships with a small group of children and their families.
- Working with Early Years and other professional teams to continuously improve the quality of childcare and education and to learn about current developments in childcare practice and policy.

Main duties of the job
- Provide compassionate clinical and operational leadership for Mental Health Inpatient Wards including line management of Ward Managers
- Work with the Head of Service, Senior Leadership Team and multidisciplinary teams to deliver safe, high-quality, person-centred care
- Ensure inpatient services work effectively as part of Berkshire's wider Mental Health Services developing pathways and strong partnership working with community and system partners

Berkshire Healthcare is developing an innovative Assertive Outreach service to provide intensive, flexible and compassionate support to people living with severe and enduring mental health conditions who find it difficult to engage with traditional services. Main duties of the job
- To undertake health assessments for children and young people in care aged 0 -17 years living within Berkshire and/or within the 20-mile radius of Berkshire.
- To assess and provide recommendations on plans of care for children and young people in care.
- To provide health promotion advice to children and young people in care and their carers on issues such as sexual health, contraception, smoking, diet, exercise and other relevant issues.
- To be responsible for a caseload as delegated by the Specialist Nurse for children and young people in care.
- To participate in the wider children and young people in care team training function regarding the health needs of children and young people in care in conjunction with the lead specialist nurse.
- In conjunction with the wider children and young people in care team, undertake audit, interpret data and use findings to enhance clinical practice.
- Contribute to Inter agency assessments and where appropriate, analysis of risk.
- When required, attend relevant meetings e.g. statutory review, professionals' meetings.
- Engage children on young people in care in feedback and engagement activities to inform service development.
- In conjunction with social care ensure that health recommendations following assessments are implemented.
- To contribute to and promote the achievement of key performance indicators.

Are you an experienced Children's Nurse looking for the next step in your leadership career? We are seeking an enthusiastic and compassionate Team Lead for our Community Children's Nursing and Specialist School Nursing Team in East Berkshire.
Based at Upton Hospital, Slough, you will provide operational and clinical leadership to a dedicated team supporting children and young people with complex health needs across homes, schools, special schools and respite settings. - Lead the assessment, planning, delivery and evaluation of evidence-based nursing care in collaboration with the Community Children's Nursing (CCN) Team.
- Deliver training, mentoring and care planning for staff nurses, nursery nurses, support workers and school staff using competency-based training packages.
- Develop and maintain individualised care plans for pupils.
- Work closely with safeguarding and children-in-care teams, contributing to reports and attending relevant meetings.

The post will involve working both day and night duties. This could predominantly be night duty depending on service cover. We are a dynamic and progressive community hospital setting and as a Band 5 Registered Nurse you work to support the Ward Sisters in the assessment, planning, implementation and evaluation of patient care. Initiatives including RIO care records, Electronic Observations and Quality Improvement programmes are currently in progress within the Unit and your input with these will be encouraged within a supportive team.
Main duties of the job
- The post holder will be responsible for the assessment of care needs, development and implementation of evidence-based programmes of care.
- The unit consists of two wards where staff members work together across both.
- Predominantly night duties, although expectation to work a combination of days and nights as service dictates

Job summary
We are seeking an experienced and motivated Community Nursing Team Lead to play a key role in the success of our Community Nursing Teams. This leadership post is for an NMC-registered nurse with a Specialist Practitioner Qualification (SPQ) who demonstrates the Trust's values and behaviours.
You will lead, develop and operationally manage a designated Community Nursing Team, ensuring the delivery of high-quality, evidence-based, person-centred care across a seven-day service. This includes managing capacity, prioritising visits and responding effectively to service pressures.
The role involves supervising, supporting and developing staff, promoting interdisciplinary working, professional development and lifelong learning. Working closely with the Clinical Lead and stakeholders, you will support service integration, maintain CQC standards, manage delegated budgets, oversee recruitment and retention, and ensure compliance with policies, performance targets and contractual requirements.
You will demonstrate strong communication skills, sound clinical judgement and the ability to manage risk, investigate incidents and complaints, and embed learning across the service.
You will maintain your own clinical competencies, NMC registration and professional portfolio while providing visible clinical leadership to teams managing complex caseloads.

Main duties of the job
- Provide clinical and operational leadership for a Community Nursing Team, ensuring high standards of safe, person-centred care.

- Lead, supervise and develop staff through appraisal, performance management, clinical supervision, education and training.
- Recruit, retain and support staff wellbeing, attendance, and professional development.

- Manage delegated budgets, resources and staffing efficiently.
