Job summary
The Complaints & PALS Team are looking for a well-motivated, friendly and hardworking individual to join the team in the role of the Complaints & PALS Officer. We are looking for someone who pays great attention to detail, has an excellent telephone manner and has previous experience of working in an office environment. The focus of the job is to ensure that all formal complaints & PALS contacts are handled in line with Trust policy
This key position will require excellent communication and organisation skills with knowledge of basic IT. Previous complaints experience is desirable. You must have a methodical approach to work and be able to work independently as well as in a team whilst remaining calm under pressure.
Main duties of the job
Duties will include logging, acknowledging and closing complaints & PALS contacts, arranging meetings, taking minutes, answering telephone calls, maintaining databases and spreadsheets and liaising with staff across the organisation. You will also attend meetings with complainants and their relatives and will be someone who is confident in interacting with a variety of different people.
This is an excellent opportunity to join a friendly and supportive team whilst working in a fast-paced and interesting department within the Trust.

Based in Bakewell, our service promotes a person-centred and non-pharmacological approach to support older adults with functional health , who may be experiencing or approaching a crisis in their mental health.
We cover the High Peak and North Dales area of Derbyshire.
The aim of the service is to assist in maintaining the independence and dignity of those living with dementia, with the ultimate goal of preventing hospital admission where possible.
In addition the IRHTT can support early discharge from hospital, working closely with carers and relatives to facilitate this.
The service operates a service 365 days a year,9am-5pm seven days a week and Bank Holidays.
Main duties of the job
You will:
- Employ a wide range of skills, building on and developing their expertise in managing complex cases as part of a wider multi-disciplinary team, which includes Occupational Therapy, Physiotherapy, Nurses, Specialist Support Workers, Psychology and a Consultant Psychiatrist.
- Be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.
- Be expected to travel independently across the catchment area of the service and occasionally outside of this if required.
- Work a shift pattern facilitating the service, which operates 7 days per week.
- Undertake the Duty role; triaging referrals, allocating visits, preparing and delivering handover, ensuring colleagues are "safe and well".
- Work as part of a MDT; liaising with a multitude of different professionals and agencies.
- Complete initial assessments of patients.
- Apply knowledge of the Mental Capacity Act and Mental Health Act when required.
- Assess risk and formulating safety plans accordingly.
- Formulate person-centred care plans focusing on non-pharmacological interventions.
- In-reach to inpatient wards, attending 117 meetings.
- Visit patients in their own home and those in 24 care.
- Utilise the EPR to ensure excellent documentation of patient assessment and treatment.

Whether you're treating and discharging a patient, referring them to another service or escalating their care, the patient and their safety will be at the centre of your decision-making.
Maintaining excellent clinical standards
You'll work within IC24's clinical governance framework and prescribing formulary, maintaining accurate and timely clinical records and working in accordance with GMC guidance and relevant national and local clinical standards.

Urgent Primary Care Doctor Bank (Home Visiting)
Bring urgent primary care to the patients who need it most - in their own homes.
Urgent care is General Practice at its most immediate.
Not every patient who needs urgent care can come to us. Some are housebound, frail, approaching the end of life or have been clinically assessed as needing a face-to-face review in their own home.
What they need is an experienced doctor who can listen, assess, make confident clinical decisions and get them to the right care.
At IC24, we're looking for Urgent Primary Care Doctors to work within our Home Visiting Service across Kent.
You'll work alongside our drivers, dispatch teams and wider urgent care colleagues, providing safe, timely and effective care to patients in their own homes.
If you enjoy the variety of General Practice, the autonomy of home visiting and the clinical challenge of assessing and treating patients in their own environment, this could be the role for you.
What you'll focus on
Assessing and treating patients in their own homes
You'll provide face-to-face assessment, diagnosis and treatment for patients who have been clinically triaged as requiring a home visit.
You'll see patients with a broad range of urgent primary care needs, considering not just the immediate clinical problem but their wider circumstances, environment and support needs.
You'll assess what's happening, decide what needs to happen next and ensure patients receive appropriate treatment, referral and safety-netting.
Making confident clinical decisions
Home visiting requires sound clinical judgement.
You'll often be working with patients you haven't met before, sometimes with limited background information and sometimes when they're anxious, unwell or experiencing difficult circumstances.
You'll need to be able to assess risk, manage uncertainty and make safe decisions independently, recognising when a patient can be treated and supported at home and when they need escalation or an alternative care setting.
Treating the whole patient
Seeing someone in their own home can give you a very different perspective on their health.
You'll care for patients across ages and backgrounds, including older and frail patients, people with complex health needs and those approaching the end of their lives.
You'll consider the patient's clinical presentation alongside their environment, support network and wider circumstances to determine what they need to remain safe.
Keeping patients safely in their community
Not every patient who needs urgent care needs to be in an Emergency Department.
Where clinically appropriate, you'll help patients remain safely at home by putting the right treatment, advice, referrals and safety-netting in place.
You'll work with community nursing, pharmacy, paramedic, primary care and wider healthcare teams to support continuity of care.
And when hospital or another service is the right place for a patient, you'll recognise that and escalate appropriately.
Working independently, but never in isolation
Home visiting is autonomous by nature.
You'll be making important clinical decisions in patients' homes without the immediate infrastructure of a traditional clinical setting around you.
But you'll remain part of the wider IC24 team.
You'll work closely with our drivers and dispatch colleagues and communicate with other health and care professionals to make sure patients receive timely, joined-up care.
Maintaining excellent clinical standards
You'll work within IC24's clinical governance framework and prescribing formulary, maintaining accurate and timely clinical records and working in accordance with GMC guidance and relevant national and local clinical standards.
You'll maintain your CPD and essential learning and have opportunities to contribute to audit, learning events, education and quality improvement.
This is urgent care beyond the consulting room
No two home visits will necessarily look the same.
You'll see patients with everything from relatively straightforward acute illness to complex physical, mental health and social circumstances.
Sometimes the answer will be clear.
Sometimes you'll need to piece together limited information, understand what's happening around the patient and make an important clinical decision without having every investigation immediately available.
You'll probably thrive here if you:

Main duties of the job
To work effectively and efficiently within the framework of team nursing and individualised patient care. In collaboration with the department's integrated multidisciplinary team to develop, implement and evaluate programmes of care for patients.
To contribute towards developing and maintaining a suitable and stimulating learning environment, and to supervise junior registered and non-registered staff.

With an increasing demand for urgent care, we are seeking the right person/s to join our workforce within our Urgent Treatment Centre . We would welcome applications from Emergency Nurse Practitioners and Paramedic Practitioners, all based within our Hospital in Ashford, Surrey.
The Urgent Treatment Centre provides access to urgent care, 7 days a week, 365 days a year. As a clinician, you will work as an independent, autonomous practitioner providing high quality, evidence based, patient centred care. Responsible for utilising expert diagnostic and clinical skills to provide unscheduled medical care and management of a diverse range of patients with undifferentiated and undiagnosed minor illnesses and injuries. This will include children over the age of 2 and adults.

Clinical:

Main duties of the job
The post holder is responsible in assisting in the delivery of healthcare services to the prison population. Supported by registered nursing staff within healthcare, they will deliver care within the boundaries of their role, focussing on supporting patients to be healthy; monitoring of long term conditions, health promotion and screening and responding to emergencies. They will work collaboratively with the Healthcare Team to meet the needs of patients, supporting the delivery of Local and national Policies and Procedures.
