Main duties of the job
Are you an experienced Band 5 Trauma Orthopaedic nurse ready for the next steps in your nursing career, or are you an experienced Band 6 nurse seeking a new and exciting opportunity?
We are seeking to appoint a highly motivated, dynamic and enthusiastic Band 6 to join our existing team and help develop further our service.
The post will be based on the Northwick Park Hospital site and the roles' key focus will be:
- To act as deputy to the Clinical Nurse Manager as required.
- To lead a team of nurses and healthcare assistants, supporting the Clinical Nurse Manager in providing a unified approach to high standards of patient care.
- To create an environment that promotes excellence in nursing practice and empowers staff to develop high levels of clinical skills and autonomy.
- Work collaboratively and co-operatively with others to meet the needs of patients and their families.
- Being a pro active member of the nursing team attending consultant ward rounds, multidisciplinary team meetings and ward / divisional meetings influencing decisions and the direction of nursing being an advocate for patients and staff.

Make a Difference to the Health and Wellbeing of Care Home Residents
We are looking for an enthusiastic and experienced Advanced Nurse Practitioner to become part of our established and forward-thinking team, delivering dedicated support to care home residents and housebound patients across Bridlington.
At Drs Reddy & Nunn, we provide proactive, responsive healthcare to a population of around 42,000 patients, alongside specialist support for 24 care homes in the local area. Our highly regarded service is built on strong partnerships with care home providers, GPs, community services, and wider system colleagues - working together to deliver truly person-centred care for residents and patients with complex and often vulnerable health needs.
Full-time hours are preferred; however, we would be happy to consider part-time hours for the right applicant, with flexibility to support a positive work-life balance.

As part of the Enhanced Health in Care Homes model and wider community service provision, you will help improve quality of life, prevent avoidable hospital admissions, and support individuals to live well, with dignity and independence.
Main duties of the job
As an Advanced Nurse Practitioner, you will work autonomously and as part of a multidisciplinary team, providing advanced clinical assessment, diagnosis, treatment and care planning for care home residents and housebound patients with complex physical, mental health and social care needs.
The successful post holder will primarily support care homes residents and housebound patients, whilst also providing clinical support to the wider practice team through clinics and service delivery as required to meet patient demand and organisational priorities
You will act as a key clinical link between primary care, care homes, community services and wider health and social care partners, supporting proactive management of frailty, long-term conditions and urgent healthcare needs.
In addition to your care home and housebound patient responsibilities, you will provide clinical support to the practice through face-to-face, telephone and digital consultations, long-term condition management, urgent care assessment and other clinical clinics as required, helping to ensure safe, effective and responsive patient care across the practice population.

Main duties of the job

The PCH is a multidisciplinary team delivering health care to people in Great Yarmouth. The combined skills of the team mean they are able to respond to the changing needs of patients and support those who may have previously been admitted to hospital to remain at home. The leg ulcer clinic is a valuable asset within the PCH delivering specialised leg ulcer care to patients who are not housebound.
The clinic operates Monday to Friday 08:30- 16:30 excluding bank holidays.
This post is offered up to 37.5hours per week. We are able to support part time and flexible working arrangements.
Main duties of the job
As a Clinic Nurse you will be providing an excellent standard of person centered care to adults with a lower leg wound in a clinic setting and occasionally their own home.
You will participate in the planning and efficient running of the leg ulcer clinic at Joseph Kittle centre Martham, Northgate hospital and Shrublands surgery.
This role includes triage of incoming referrals, determining clinical priority, completing holistic patient assessment, specific wound assessments including recording dopplers, diagnosis and treatment plan, for patients referred to the service and supporting patients to self care using health coaching.
You will be supported to complete the leg ulcer management course delivered in partnership with Accelerate CIC, and a health coaching qualification. You will be encouraged to continue your own professional development and contribute to service improvement. You will be part of the PCH support for students; those on an apprenticeship route and newly qualified nurses. Once experienced you will be a knowledgeable resource on leg ulceration to the wider PCH; other ECCH clinicians; primary care and other partners.

Activity Co-ordinator - Basildon Mental Health Unit
Under the guidance of the Ward Manager, you will be expected to assist in the delivery of recreational activities within the service. Activities will be delivered one to one and through group work and may include creative, social and physical activities, the majority of which will be on the ward environment. We are looking for someone who can work flexibly; as well as working week days, you are required to work some evenings and weekends.
Main duties of the job
- To liaise with ward staff in the planning of recreational activities.
- To lead on recreational activities, working with groups of patients and patients on their own.
- Prepare craft and any other appropriate equipment before patients arrive.
- Maintain stocks of appropriate supplies for use in the activities provided; informing the ward manager of any shortages of stock or equipment needed.
- To develop rapport with patients to encourage their interests and skills, and actively involve them in deciding and planning their participation in activities.
- To be aware of individual patients' needs through communication with nursing staff.
- To liaise with the wider multi-disciplinary team in order to encourage collaboration with the therapeutic timetable.
- To observe general behaviour, ability and response of patients to activities.
- To have an awareness of the situations that may make patients vulnerable or at risk.
- To report any concerns immediately to qualified staff.
- To respect patients choices and confidentiality at all times.

Main duties of the job
Screen & identify patients eligible for clinical trials and to maintain detailed knowledge of the protocols for the clinical trials associated with the designated site-specific groups.

Job Context
- The Care Act was the most significant change in social care legislation in 70 years. The Act introduced a number of new duties for local authorities, including the wellbeing principles and statutory responsibilities to carers and safeguarding adults at risk of harm or neglect. The Act also introduced a new national eligibility criterion for adult social care. - The new Adult Social Care Assurance Framework will measure the quality and effectiveness of adult social care services within the Local Authority. Job Context
- The Care Act was the most significant change in social care legislation in 70 years. The Act introduced a number of new duties for local authorities, including the wellbeing principles and statutory responsibilities to carers and safeguarding adults at risk of harm or neglect. The Act also introduced a new national eligibility criterion for adult social care. The Act represents a shift in social care practice to encompass all professions within social care and emphasises personalisation in social work practice, where assessment is considered an intervention and where social work practice should take account of the individual strengths of the person, their family and friends and wider community.
- The new Adult Social Care Assurance Framework will measure the quality and effectiveness of adult social care services within the Local Authority. There will be a significant focus on developing staff awareness of performance, quality, the experience and outcomes for people using social care services and continuous assessment of local authorities against nationally agreed Key Lines of Enquiry.
- You will work as part of the team under the Practice Lead, Portfolio Lead or Countywide Group Manager in a specific locality or team. The focus of the Advanced Practitioner in the team is to implement the requirements of the Care Act with a focus on quality, performance, practice and outcomes for people. There is an exciting opportunity within the Mental Health Access Team for a full time permanent Band 6 Mental Health Nurse to join our dedicated Team.
There aretwo vacant posts. One post is based at Vale house in Winsford covering the Central locality and one post is at Cherrybank Resource Centre in Ellesmere port covering West.
We are looking for an enthusiastic and motivated Mental Health Nurse with excellent communication skills, who has the ability and confidence to work autonomously as part of a multi disciplinary team.
The service covers the hours of 9am-5pm Monday to Friday, however there is opportunity for flexible working hours, dependent upon service needs.
Main duties of the job
The post holder will carry responsibility for the triage, assessment and review of patients who require interventions within Primary Care or onward referral to appropriate services.
As a single point of access service, the workload is rich and diverse, including assessment of mild/ moderate mental health difficulties, linking in with our Medical Provision, referring onto Specialist Services, including working closely with Talking Therapies colleagues as well as assessment of complex and severe mental health problems and referring on to appropriate services, depending on needs and complexities. We also have Medical Provision within the service consisting of Psychiatrists and an Advanced Clinical Practitioner.
You will work as part of an experienced MDT, including Mental Health Nurses, Psychiatrists, Psychologists, Social Prescriber and Liaising with other teams.

Royal Free London NHS Foundation Trust Ophthalmology service provides a number of services across different settings in North London. We are recruiting into a full time 37.5 Band 5 position in our ophthalmology department at Chase farm hospital.
The post includes working in Ophthalmology outpatients . The post is primarily at Chase farm hospital but also requires working at Barnet Hospital on a weekly bases when needed and cross site cover as required at Edgware community hospital
This post is only available to Nurses who already have full NMC registration or student nurses awaiting their pin. Not suitable for nurses requiring OSCE.

Working in a friendly environment you will be responsible for the assessment and planning of care needs for Ophthalmic patients in a out patient setting .
Previous experience in Ophthalmology would be an advantage but not essential as full training will be given.
In addition to the Ophthalmology department at Chase farm hospital we also have ophthalmic clinics in other hospitals within the Royal free London foundation trust and you will be expected to work there if needed.
Working in a team you will be responsible for assessing patients and preparing them for doctors assessment in clinics as well as working in nurse led clinics e.g. pre-operative assessment and fundus Fluorescein angiography clinics
The hours ours will be worked across 5 days Monday to Friday depending on activity demand.

An exciting opportunity has become available for a qualified Band 6 Mental Health Nurse to join Cambridgeshire Liaison and Diversion Service, who support people with vulnerabilities, of all ages who find themselves in the criminal justice system.
Practitioners are based in courts and police custody units across the county. Their role is to screen and assess service users of all ages and where appropriate divert them out of the criminal justice system where there are no public protection issues.
Main duties of the job
The successful candidate will be working in police custodies and the Magistrates courts across the county, and will either be based in Cambridge or Peterborough. This may also include visiting rural areas to ensure that there is an equitable service for all service users to access.
The role will include working in host environments, working at times autonomously from the main team but with the support from police, court and probation staff.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
