Job summary
An exciting opportunity has arisen for a Community Mental Health Nurse to join our Memory Team in Redcar, based at Reed Marsh House. We are looking for a dynamic, positive and forward-thinking clinician with a passion for caring for people with cognitive impairment, their families and carers, and with a specialist interest in delivering high quality, effective and place-based Memory Services. This is an exciting time to join our team and locality - this role is an important part of a quality improvement initiative, alongside the trust Dementia pathway refresh. We deliver our memory services to broaden access and ensure delivery of specialist memory assessment in a timely manner. This is essential to enable timely diagnosis of dementia and associated needs and conditions, which supports access to early treatment and support, decision making and aids optimal management and life outcomes for those in need.

Main duties of the job
The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. The ability to work independently, effectively and collaboratively within Multidisciplinary Teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.
In the role you will be responsible for making the initial contact to support service users and their families and carers in their understanding of the memory pathway. You will undertake a detailed history using a holistic memory assessment framework, with the initial collation of all clinically relevant individual service user information (clinical history) and involving families and carers as part of our genuine commitment to co-creation. In this way you will promote positive experience and clinical outcomes from the beginning of the person's service journey.
There is a requirement to accurately record and document clinical discussions and assessment details on our clinical system and to ensure that all individual service user documentation is up to date and available to inform internal clinical meetings and aid consistent, safe and high quality decision making by members of the MDT and wider colleagues.

The post holder is required to provide an efficient and effective support service to the residents in staff residential accommodation on the Ysbyty Gwynedd Hospital site, including accommodation booking, security of property, key holding, and record keeping.

Main duties of the job
The post holder will be required to communicate with a variety of people face to face, by telephone and in writing. Whilst communication will be largely routine, there may be some barriers due to difficulties with language and culture.

Main duties of the job

The Care Coordinator will be part of the Primary Care Network (PCN) Multi-Disciplinary Team (MDT) who are responsible for managing the care of people registered with practices within a particular PCN. This will involve coordinating the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN population.
Main duties of the job
The post holder will contribute to tackling inequalities in health and social care particularly regarding individuals with long-term conditions. An ethos of promotion of independence and partnership-working is integral to this post.
A key part of the role of a care coordinator role is in the care Homes MDT: improving the continuity of care by acting as a point of contact for residents, families and professionals who visit care homes, such as MDT members and in-reach specialists.
They will support the MDT with the weekly virtual home round through identification of people in need of review, or collation of information on people requiring an MDT review in addition to providing coordination, secretarial and administrative support to the MDTs within a single or multiple PCNs.

The Home First Team provides clinical support for patients in the community to either avoid potential hospital admissions or to support people to return home through the Discharge to Assess Pathway 1 following a period of acute care in hospital.
