Main duties of the job
As a Senior Primary Care Nurse your responsibilities will vary. You will
- You will lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- You will undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
- Seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference, as defined throughout our bespoke Health in Justice competency framework.
- Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of.
- Evidence based care and continuous quality improvement, embedding a culture of shared learning.
- Building strong multi-disciplinary teams and supporting each other to do a great job.

Job summary
This is an exciting and unique opportunity to deliver diagnostic spirometry to the clients of Change Grow Live (CGL) Coventry, the commissioned drug and alcohol treatment provider. The successful post holder will conduct clinics in CGL Coventry to diagnose respiratory conditions and advise accordingly on their management. The innovative project is paving the way for tackling health inequalities and bringing care to the most vulnerable within society. This trailblazing project, funded by the ICB (Integrated Care Board), enables us to deliver care to those who typically do not engage with primary or secondary care, aiming to increase early diagnosis and reduce drug related deaths. The post holder will integrate across primary and secondary care. Support to be successful in this role will be provided by clinicians at CGL and the Community COPD Service and Respiratory Physiology department at University Hospitals Coventry and Warwickshire.
The post-holder will need to provide evidence of a valid Full UK driving license and must have access to their own vehicle for work purposes.

Job summary
The INT Data and Development Coordinator will provide data, administrative, and project support to the Integrated Neighbourhood Team (INT). The post holder will assist in the collection, analysis, and reporting of performance and activity data to support service improvement and transformation initiatives across the locality.

Specific to Charing Cross we have 4 procedure room, and we are recruiting to meet the increase demand of Endoscopy and Bowel Screening Services. You will have the opportunity to work closely with our screening colleague's ant West London Bowel Cancer Screening programme as we deliver the Faecal Occult blood Test Services to our catchment area.
You will receive full training and mentorship to enhance your confidence and competence within our JAG accredited Endoscopy Unit.

Main duties of the job
- Deliver safe, effective, and patient-centred nursing care to patients undergoing diagnostic and therapeutic endoscopic procedures, in line with Trust policies and national guidance.
- Undertake comprehensive pre-, intra-, and post-procedure assessments, planning, implementation, and evaluation of individualised care.
- Support the Endoscopy Unit Manager / Sister in the day-to-day running of the unit by maintaining safe standards of care and escalating concerns appropriately.
- Take responsibility for the organisation and coordination of patient care for a designated group of patients during a shift.
- Assess the needs of patients, relatives and carers and provide them with information as requested.
- Assist in the teaching and development of nursing assistants, pre-registration students helping them to achieve their learning outcomes.

We are looking for a motivated, enthusiastic and organised Registered Nurse to join our Gastroenterology Service as an IBD Support Nurse.
Working as part of our established multidisciplinary IBD team, you will play a key role in supporting patients living with Crohn's disease and Ulcerative Colitis. Your primary responsibilities will include assisting with the management the IBD telephone advice line, monitoring and responding to patient queries submitted through the Trust's digital IBD platform/app, and providing high-quality patient support, education and coordination of care.
