Main duties of the job
As a Lead Nurse, your role encompasses a wide range of responsibilities and leadership such as:

We have an exciting opportunity for Band 6 Mental Health Nurses to join the Older Adults community team based in South Warwickshire. We are a developing team committed to providing excellent patient care, proactively embedding and utilising multidisciplinary roles to provide recovery and outcome focussed interventions for individuals within the service.
We are seeking candidates who have experience of working in mental health settings who have a keen interest in working with older adults, are innovative, dynamic and looking to develop their clinical skills. Our Frailty Service is a multidisciplinary one for those over 65 years of age who demonstrate two or more of the five frailty indicators (including falls, immobility, incontinence, delirium, medication side effects susceptibility). These patients are comprehensively assessed in their own home (or care home) following which a bespoke patient centred care plan will be created. The intention is to either improve or maintain their level of health and wellbeing and also to enable them to remain as independent as possible in their own residence. The Frailty Team does not carry a caseload; each referral is classed as an intervention. Following review from the Frailty Team the patient care is handed back to their GP practice.
Each patient is assessed for falls prevalence and has their medications reviewed. An independent prescriber would be an advantage for this role but it is not essential as other members of the team can issue medication
You will attend weekly MDT meetings with a consultant geriatrician hosted by our frailty team to discuss the most complex patients who have regular home visits. You will also closely liaise with the wider PCN team as needed in order to deliver a truly holistic healthcare service. The daily Acute Home Visiting list offers a change of pace and you will encounter such problems as swellings (legs, feet etc), UTIs, chest infections, oedema, infected wounds, skin complaints such as rashes and migraines etc. The provision of this service is vital to our member practices and frees up practice GP time while we fulfil these visits for them

Main duties of the job

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

As a Registered Nurse in the emergency department you will be working across all areas of the specialty including Triage, Minor Injuries and Illness (Ambulatory Pathway), Majors (Clinical Assessment), trauma and Resus. To work cohesively with the admin and clinical team within Wheelchair Services, to undertake the following:
- dealing with patients on the telephone and face to face, fielding all enquiries and communicating timescales, using empathy and compassion at all times
- booking clinical appointments for a variety of different wheelchair clinics, including on-site and domiciliary visits and MDT work
- ordering equipment on procurement system and tracking equipment to alert clinician when ready for handover. This regularly involves communicating and negotiating with suppliers
- liaising with approved repairer (based in Carlisle) to ensure stock levels are adequate and all appropriate equipment received has been had a pre-delivery inspection undertaken and registered as ready for handover
- clinic planning alongside clinical lead ensuring patients are seen in a timely manner, allocating of patients to the appropriate clinic type and ensuring identified and tracked equipment is delivered to the correct location
- taking delivery, unpacking, labelling up and storing wheelchair accessories in on-site wheelchair store room

You will work closely with the Inpatient Matron and the existing Band 7 midwives, to ensure that the Maternity Unit is operationally safe, effective and well led. Main duties of the job
As a Band 7 Critical Care Outreach Nurse Practitioner, you will undertake clinical, teaching and management responsibilities. You will be required to demonstrate significant experience in caring for critically ill patients, have a specialist course in a critical care or Emergency area and have a minimum of 2 years in a Band 6 role or above, with shift leading experience. This post requires motivation, flexibility and initiative, the ability to prioritise workload in challenging situations and excellent communication skills. The applicant should be able to demonstrate the use of evidence-based practice and be proactive in managing change.
The main duties of the role include:

Job summary
At St George's Hospital, no two nights are ever quite the same. As an Advanced Clinical Practitioner within our ENT, Maxillofacial and Plastics team, you'll play a key role in assessing and managing complex surgical cases in a supportive and collaborative environment.
Your shift might begin with a patient arriving via air ambulance with facial trauma, or a potential airway emergency presenting in ED.
As one of our ACPs, you'll take the lead on assessments, request urgent imaging, and carry out minor surgical procedures. You'll make independent clinical decisions and prescribe treatment with the support of a wider multidisciplinary team.
Working alongside you are two other experienced ACPs, each covering their own specialty. You'll share knowledge, provide mutual support, and hand over as a team at the end of your shift.
This role offers variety, challenge and the chance to work at the top of your scope. From minor procedures to major trauma, you'll develop your clinical expertise in a team that values innovation, professional growth, and high-quality patient care.
Main duties of the job
This role places you at the heart of advanced clinical practice within ENT, Maxfax and Plastics. You'll be part of a team of six ACPs providing overnight cover and playing a key role in assessing and managing patients arriving in the emergency department.
You'll need to have:
- Completed your MSc in Advanced Clinical Practice
- Hold a non-medical prescribing qualification
- Be confident in requesting and interpreting diagnostics as well as performing clinical procedures within your scope
Please note: To be considered for this role, you must have experience working in an ED or as a Nurse Practitioner, or paramedic with exposure to acute surgical patients. While this is a night-shift position, your first six months will be spent working alongside the doctors during daytime hours as part of your induction and training period.

- The Care Act was the most significant change in social care legislation in 70 years. The Act introduced a number of new duties for local authorities, including the wellbeing principles and statutory responsibilities to carers and safeguarding adults at risk of harm or neglect. The Act also introduced a new national eligibility criterion for adult social care. - The new Adult Social Care Assurance Framework will measure the quality and effectiveness of adult social care services within the Local Authority. Main duties of the job

Oversee the patient's treatment and investigation pathway, ensuring it is seamless and well communicated to all parties, including the patient.
Provide practical and emotional support to the patient and family.
Coordinate the provision of information and ensure that it is timely, and meets the current needs of the individual patient and family/carers. Ensure that the information has been understood and that where a patient information prescription is available that this is used.
Ensure a holistic needs assessment of the patient is undertaken and that a written care plan is provided / updated.
Liaise with health and social care agencies and professionals in the community, including the primary care team.
Support the acquisition of new skills by the patient as needed, for example, care and management of Deep Brain Stimulators, infusion devices and pumps.
Liaise with the nursing and allied health professionals in clinical departments as they move between care settings along the patient pathway.
To lead on Patient Information Provision for the MDT. Ensuring in-house patient information is up to date.

Job summary
This is a fixed-term Junior Clinical Fellow post in Psychiatry, based at Bethlem Royal Hospital within South London and Maudsley NHS Foundation Trust. Main duties of the job
Our Neighbourhood Teams offer seamless proactive support and care in the community. The teams are made up of multi-professionals working closely with our partners.
Our service runs from 8am to 8pm every day of the year. We are looking for a nurse who is dynamic, innovative, passionate and committed with experience of case management and multidisciplinary work.
We are looking for leaders who possess excellent communication skills and the ability to motivate others. We want individuals who are resilient and able to support others through change to maintain the delivery of high quality, compassionate, proactive and safe care.
Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
We currently have 2 secondment positions available for 9 months each initially with the opportunity for them to be extended.

The General Practice Clinical Lead will provide clinical leadership within a GP practice

The Upper GI Cancer Clinical Nurse Specialist provides expert nursing advice, leadership, and support to patients, relatives, carers, and the wider multidisciplinary team to ensure the delivery of high-quality, person-centred care. The post holder works collaboratively with healthcare professionals across disciplines to optimise patient outcomes, provide holistic support, and facilitate informed decision-making.
As a clinical leader and professional role model, the Clinical Nurse Specialist provides both direct and indirect patient care, contributes to service development, supports teaching and training activities, and participates in research and quality improvement initiatives. Main duties of the job
To assess, plan, implement and evaluate programmes of care, providing specialist advice and actively seeking evidence based practice which will improve the quality of care and meet the needs of a group of patients.
