An exciting opportunity has arisen for an experienced and enthusiastic renal nurse to join the Home Therapies Renal Team at Liverpool University Hospitals working across the Royal Liverpool Hospital & Aintree sites.We are looking for a Home Therapies Renal Nurse Specialist to provide a quality service to patients who choose to have dialysis at home on either HD or PD.
We are seeking to recruit an individual with excellent communication skills, along with extensive post registration experience in renal replacement therapies, along with evidence of professional and personal development to degree level, or equivalent specialist study/experience. We are an enthusiastic and dedicated team and the role will not only impact on patient care but will be challenging and rewarding on a personal level.
Main duties of the job
The Home Dialysis Team is working to provide training and support for patients choosing to have dialysis at home. This is a growing service that aims to provide the highest standard of care for patients through on going education and support, both in hospital and at home.
Community work is required for this role therefore full driving license and access to a vehicle is essential for applicants.

We are looking for a senior RMN with experience of working in mental health services to join our integrated Mental Health & Substance Use Service at HMP Dovegate, a Category B facility for remand an sentenced adult males.
You would be joining a professional and compassionate multi disciplinary team committed to Recovery focussed and trauma informed approaches to care. The Service is responsible for assessment, planning and the delivery of a range of evidence based therapeutic interventions to support mental health and substance use needs for the men under our care.
This role is specifically designed to support early screening assessment and support for men in the early days in custody
Main duties of the job
Working effectively within the team you will provide evidence based interventions. You will manage a caseload and will be required to undertake assessments and reviews. Using the Care Programme Approach, you will be a care coordinator, communicating effectively with those involved in the care of people in custody up to release. The role will require you to be a reflective practitioner, eager to work collaboratively with individuals, carers and other professionals to contribute to the delivery of an effective, dynamic, integrated mental health and substance misuse service. You will work with individuals from initial assessment through to discharge, which can include transferring care to other providers such as hospitals or CMHTs whilst working with a variety of agencies in the Offender Health Pathway. You will help to rehabilitate and support service users to live hopeful and fulfilling lives as members of the community. The Clinical Nurse Specialist (CNS) in Respiratory Care is an advanced practice nurse who provides expert clinical leadership, specialist assessment, and evidence-based interventions to patients with acute and chronic respiratory conditions. This is an exciting opportunity to take on a part time role to improve access to heart failure services for Epsom and St Helier. We ideally would like somebody with heart failure experience that is keen to improve the access to specialised care in hospital and assisting in the transition to the community.
Main duties of the job
We are an expanding cardiology team and we are looking for an experienced heart failure specialist or cardiology nurse who wants to help shape the care we deliver.
Working closely with the lead Heart Failure specialist nurse, the Cardiology Matron and the lead Heart Failure Consultant, you will run and develop thediagnostic and treatment pathways for heart failure patients promoting high quality care across the care continuum from tertiary to primarycare, engaging with healthcare professionals across the network.
The post holder will also be required to support clinical staff in heart failure management plans; make appropriate onward referrals for follow up care; be involved in optimisation of medical therapy; provide education and support to patients, relatives and carers in both inpatient and outpatient settings with the aim being to reduce symptoms, prevent unnecessary admissions/readmission and maximise patients' quality of life. The role involves working closely with the community heart failure teams and the collection of data for local and national audits. You will have a keen interest in education, research, training and development.

Job summary
The appointee will join the existing multi-disciplinary respiratory team, and work in conjunction with their team members in coordinating and developing the service. The post holder will work collaboratively with healthcare professionals and stakeholders involved in the provision of integrated care, to ensure joined up care for patients across acute, community and primary and secondary care settings.
This post would suit a Nurse with a sound respiratory knowledge base who is interested in contributing to the delivery of Integrated Respiratory Care in those with chronic respiratory conditions. The service has been running since October 2016 and continues to grow and develop. The Hillingdon Integrated Respiratory Service (HIRS) provides consultant and nurse led community clinics, early supported discharge services and admission avoidance services across the borough of Hillingdon. Pulmonary rehabilitation and home oxygen services are provided within Hillingdon by Harefield Hospital and there are good links between the two organisations.
The HIRS participates in the National COPD Audit and has proudly achieved the best practice tariff for the last financial year and have been highlighted as a positive outlier nationally.

IPS (Individual Placement and Support) is an evidence-based approach to helping people with mental health conditions find and keep a paid job.

At present, only around 8% of people receiving support from secondary mental health services are in paid employment, while studies suggest almost 90% of people with serious and enduring mental health conditions want to work.
Following further investment and expansion of the IPS service an exciting opportunity has arisen for a number of permanent IPS Employment Specialists in the following area:
York, Northallerton, Harrogate and Ripon, Scarborough, Whitby and Ryedale.
The IPS worker will be working with people who have a mental illness and support them into employment and then enabling them to retain that employment. As an IPS worker you will be looking for paid employment opportunities for clients who are currently involved with secondary care adult mental health services. This will involve developing opportunities with local employers and supporting them as well as the client, to ensure the right person is in the right job.
You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.
Main duties of the job
The successful postholders will support clients with job searching, completing CV's and application forms and interview preparation. Working with our clients, you will liaise and signpost to a range of employment and training programmes within the local area, to include Job Centre Plus.
Within your role, you will develop your knowledge and networks of employment related support services and inclusive employers within the local area is essential to this role. Employment opportunities would include those within TEWV itself.
This role is part of a wider employment support service, working closely with people with moderate to severe common mental health problems and their clinicians. The postholders will support service users to gain or return to competitive (paid) employment following the Individual Placement and Support process, aiming for fidelity to this delivery model. The role will involve working directly with Jobcentre Plus, employers, trade unions and employment agencies to secure paid employment opportunities, including supporting them and their employers to retain these opportunities for up to one year once gained.

Following on from a successful proof-of-concept, Southern Hospice Group (SHG) has been awarded the contract to establish a 24/7 consultant led childrens palliative and end of life care service for Sussex.
The neonatal CNS is a vital member of our CNS Team, ensuring that babies with life-limiting or life-threatening conditions receive compassionate, specialist, family centred care from the earliest stages of life. This role is essential in delivering equitable, high quality palliative care from diagnosis onwards, ensuring that every baby and family has access to expert support wherever care is delivered.
Working with our Nurse Consultant and Lead Consultant, this is a fantastic opportunity to be part of the clinical team that shapes and delivers a fully integrated 24/7 paediatric palliative care service across Sussex, working across home, hospice, community and hospital settings to improve continuity of care.
You will use your specialist clinical skills, experience, knowledge and expertise to provide excellent care to babies, children and young people and their families/carers across Sussex. You will provide high quality symptom and palliative care, working closely with Chestnut Tree House (Childrens Hospice) and community teams to provide a seamless comprehensive service.

Helping families to navigate complex decisions and co-ordinate care across hospital and
community settings to access timely palliative care that focuses on quality of life, symptom management and family well-being.- Working collaboratively with neonatal intensive care units, maternity services, paediatric teams, hospices and primary care.

LMHT is a community-based service which operates Monday-Friday 9am-5pm.
Main duties of the job
The LMHTs provide mental health services for adults across Nottinghamshire.
We help patients and their families cope with periods of severe mental illness and assess, plan and implement a personalised path of recovery. Some of our patients may require short term intervention, and sometimes longer-term care plans will be put in place.
We are seeking an enthusiastic team player, working within a dynamic multi-disciplinary team, with excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies. In return the successful applicant will receive training and support to develop specific therapeutic skills to enable them to provide a high-quality service to our patients and their families.
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

Applicants will need to have a genuine interest in Perinatal Mental Health and the development of the service, excellent communication skills, the ability to provide supervision to junior staff and mentorship to students.
The Specialist Perinatal Mental Health Service also offers pre-conception advice for woman with a known mental illness wishing to conceive and provide training and education to professionals alongside experts by experience.
Experience of working in the Perinatal Mental Health field is desirable as is knowledge of Perinatal Mental Health Conditions, prevalence rates, treatment interventions and Safeguarding Policies and Procedures. A keen interest and passion for working with women and their families is a must, as is the ability to work closely with a range of professionals from health visiting, Maternity, Children's Services and the Third Sector.
