Job summary
Suffolk GP Federation are excited to announce an excellent opportunity for a Lead Practice Nurse to join our dynamic team. In this role, you will play a pivotal part in delivering high-quality patient care whilst leading and supporting our dedicated primary care nursing team.This is a full time role, 37.5 hours per week, Monday Friday based across the Unity sites: Haverhill, Long Melford, Lavenham and Glemsford. Travel between sites will be necessary.
We're looking for someone with a passion for healthcare, strong general practice experience, and the ability to inspire and guide the team to excellence.

Main duties of the job
- You will lead, manage and deliver a range of practice nursing services and be accountable for the nursing team on a day-to-day basis
- You will foster a learning environment, supporting the training and development of the nursing team
- You will lead on ensuring the nursing services are compliant with CQC and other regulatory authoritys requirements
- You will champion and lead quality improvement initiatives within the nursing service

Our dedicated team offers a wide range of services, including long-term condition management, annual health reviews, wound care, and same-day appointments for urgent health concerns. Main duties of the job
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.

Glenpark & Teams Medical Services has an exciting opportunity for an enthusiastic, experienced Practice Nurse to join our friendly and committed nursing team. The position is for up to 37.5 hpw - full time or part time considered - working across both of our sites in Dunston and Teams.
You will be joining a great team lead by an experienced ANP, working alongside a Nurse Practitioner, 2 Practice Nurses, 2 Nurse Associates, a Healthcare Assistant and 2 Phlebotomists. Regular mentoring sessions will be provided, along with support for both career and personal development within the role/team.
The Practice Nurse will work delivering person-centred long term condition management to our patients, and depending on experience and skills could also see patients with acute respiratory/minor illness following triage by the on-call team. Glenpark Medical Practice and Teams Medical Practice are both well established, high-achieving practices and are part of Gateshead Inner West PCN and consistently maintain a high QOF achievement.
The practice comprises 6 GP Partners and 7 salaried GPs, supported by the Practice Managers/Assistant Practice Manager. We have an established nursing team, and a highly skilled and responsive reception and administrative team.
As a practice, we support the nursing team throughout their professional development as well as also being a training practice for Doctors in Training and medical students. The practice holds a strong belief in the importance of continuous professional development as well as personal wellbeing.
The practice also specialises in substance misuse, provides a minor surgery service and LARC service.
We work closely with our PCN team comprising Pharmacists, Pharmacy Technicians, Physiotherapists, Mental Health Nurse and Social Prescribing Link Workers.
We have close working relationships with our community colleagues and have an active Patient Participation Group.

Job summary
We are seeking motivated and experienced healthcare professionals to join our team as Trainee Emergency Care Practitioners, within the Urgent and Emergency Care service.
Working alongside Doctors, Advanced Clinical Practitioners, and fellow ECP's/ENP's you will play a key role in delivering high quality timely care to patient presenting with minor illness and injury. The post holder will be required to work across all 3 sites (Minor Injuries Unit, Rugby Urgent Treatment centre and Coventry City Urgent Treatment Centre where required.

Main duties of the job
- The post holder will be working as a trainee ECP within a busy Minor Injury and Illness Unit and Urgent Treatment Centre under a scope of practice; seeing, treating and independently discharging patients as required.
- The post holder will work with Senior EP colleagues to deliver a high quality, effective and sustainable service.
- The post holder will undertake a training post and is supported to complete a Minor Injury and Minor Illness University course with a EP portfolio to work towards to becoming a qualified EP.

This is a part-time position based at York Hospital with an expectation to occasionally travel to work and provide training in other locations across the Trust.

Main duties of the job
The Band 6 Resuscitation Officer supports the delivery of high-quality resuscitation training and clinical support across the Trust. The post-holder provides Basic and Immediate Life Support education, assists in maintaining resuscitation equipment, and contributes to audits, data collection, and service improvement, reporting outcomes to Resuscitation Team Lead. Working under the guidance of the Resuscitation Team Lead, the Band 6 officer helps ensure that staff have the skills and confidence to respond safely and effectively to patient deterioration and cardiac arrest.

Ward 1 has recently maintained their PLATINUM standard in LQA assessment for the 2nd time.

Emerald Place Clinic is a general adolescent inpatient unit for the care and treatment of young people aged 12 up to the age of 18. Young people who are admitted to the unit will have a primary diagnosis of mental illness across a wide range of disorders and complex needs.
These could include Mental Health conditions such as Mood Disorders and acute Psychosis. Additionally young people may have co-existing additional needs such as Neurodevelopmental Disorders, including mild Learning Disabilities or Autism. A young person could also be admitted if they have disordered eating.
What you will be doing:
- Facilitating physical healthcare plans/ treatments to patient’s, in order to maintain their optimum level of physical well-being.
- To facilitate/support staff to ensure that routine and ad hoc tests e.g. blood screening for Clozapine/Lithium are carried out as prescribed and the results recorded appropriately within Care notes.
- To facilitate/support that routine testing for long term conditions e.g. asthma, diabetes, are carried out as prescribed and that the results are recorded appropriately in Care notes and in line with latest national guidance
- To lead the development and review of all care plans for physical health, to ensure that patient’s physical health needs have clear interventions, which are being met
- You will be supporting the clinical team on the ward and covering staff when required .
- To ensure that all patients have their ECG’s as prescribed by the Consultant and evidence is documented on Care notes.

12 bed CAMHS service to support young people aged 12-18 with eating disorders. With an onsite school providing educational services while the young people are in our care and full multidisciplinary team, it’s a great opportunity to be part of something new and help shape its future.

Middlewood Clinic provide care and treatment for men and women who have an eating disorder including Anorexia Nervosa, Bulimia Nervosa and atypical eating disorders. Our approach will be to restore a healthy body weight and address psychological problems during treatment.
The Multidisciplinary Team (MDT) at Middlewood Clinic will be made up of the Registered Manager, Consultant Psychiatrist, Nurses, Healthcare Workers, Psychologist, Dietician, Occupational Therapist and Social Worker.
The hospital will have 12 comfortable ensuite bedrooms and a variety of communal and recreational spaces including lounge areas, a light and airy dining room, activities room, visitors room, therapy rooms, therapy kitchen area and a lovely cottage style garden.

What you will be doing:
As a Registered Mental Health Nurse (RMN) or Learning Disabilities Nurse (RNLD) you will provide high-quality evidence-based care to young people aged 12-18 with eating disorders, while supporting your colleagues and promoting good teamwork.

Middlewood Clinic provide care and treatment for men and women who have an eating disorder including Anorexia Nervosa, Bulimia Nervosa and atypical eating disorders. Our approach will be to restore a healthy body weight and address psychological problems during treatment.
The Multidisciplinary Team (MDT) at Middlewood Clinic will be made up of the Registered Manager, Consultant Psychiatrist, Nurses, Healthcare Workers, Psychologist, Dietician, Occupational Therapist and Social Worker.
The hospital will have 12 comfortable ensuite bedrooms and a variety of communal and recreational spaces including lounge areas, a light and airy dining room, activities room, visitors room, therapy rooms, therapy kitchen area and a lovely cottage style garden.
