Working Pattern: Part-time, Monday to Friday. Hybrid working will be considered. Days are flexible except Tuesdays as this is the team meeting day.
Two part-time roles available:
- 1 x 22.2 hrs per week (working specifically into REACH project)
- 1 x 29.6 hrs per week (working into Children's Home Collaborative)
Location: Based at Glebelands, Shoreham-by-Sea but will be required to provide countywide coverage.

As Specialist Senior Psychological Practitioners for our service, you will:
- Deliver evidence-informed, integrative, and systemic interventions.

- Champion a compassionate, trauma-aware, relational approach at all levels of the system.
- Help shape the future of our Psychological Services Hub, contributing to its growth and impact.
About us
The Attach Service delivers psychological support into our Children We Care For, Care Leavers, Unaccompanied Asylum-Seeking and Special Guardianship teams and includes support to our Residential and Fostering services and into our two specialist projects.
The Attach team delivers a range of evidence-informed support to children in care and the professionals and carers around them, helping them understand and respond to the impact of developmental trauma, disrupted attachment, and hidden emotional needs. Within West Sussex Fostering, there is a drive and vision to deliver therapeutic parenting within foster care and kinship, underpinned by DDP principles and an attitude of PACE. Please see the West Sussex Fostering Vision Statement document for further details of the PACE culture ambition in the Information Pack.
The Attach Projects Team -These Senior Clinical Psychologist roles will specifically be linked to the newly created Projects team within the Attach Service. An experienced Principal Clinical Psychologist dedicated to offering clinical oversight of REACH and the Childrens Collaborative Project, has already been appointed. They will offer clinical supervision to both Senior Clinical Psychologist roles in the Projects team (1x 29.6 hours, and 1x 22.2 hours).

Attach is a psychology-led service supporting children in care that has recently launched within West Sussex County Council. We offer direct therapy, support for foster and residential carers and reflective supervision and training for social work colleagues with the aim of improving the lives of young people in care. The Projects team is a specialised branch of the Attach service, working into projects that are a priority for Childrens Services. The main projects we work into at the moment are:
- REACH this is a wraparound service supporting children to step down from residential homes into foster care. REACH also support foster families where there is a risk of placement breakdown and residential care is likely. REACH is a small team of social workers and child and family workers. It is anticipated that the psychologist working into REACH would offer consultation to professional networks, therapeutic parenting support to foster carers and holding a small caseload of direct therapy cases.
- The Childrens Collaborative this is an innovative project aimed at reducing the number of children that are placed in expensive out of area placements with independent providers. The Childrens Collaborative works with commissioners and independent providers to increase the number of residential placements available locally and offers training and therapeutic support through West Sussex County Council. The ATTACH Projects team will be working with the Childrens Collaborative to provide a therapeutic offer for the residential homes within this project. It is anticipated that this will include therapeutic parenting training/ support for residential carers, reflective practice, consultation and team formulation, as well as holding a small caseload of direct therapy cases.
As the Attach Projects team and the projects that we are working into are relatively new, there are real opportunities for innovation here, including the possibility of shaping a service to fit the needs of the children and services we are supporting. We are looking for clinicians who are passionate about improving outcomes for children in care and who have energy and enthusiasm to help grow and shape a new service.
As a senior psychological practitioner in the team the role will involve clinically supporting other staff and taking on some supervision and delegated leadership responsibilities. There will also be opportunities to be involved in service development and training delivery.
The post holders will receive a comprehensive induction which will include opportunities to shadow and to co-work.
The post holders will receive regular clinical supervision from the Principal Clinical Psychologists in the Attach Projects team, in addition to ongoing opportunities for co-working and taking part in regular team case discussions. The team has links to the University of Surrey Clinical Psychology Doctoral programme and the opportunity to provide placements to trainees.

- 1. You will have post qualification experience in undertaking assessments and therapeutic interventions with children and their families. Ideally, you will also have good experience of working with children who are placed in the care of the local authority, and/or those placed with special guardians. This will include a detailed understanding of the needs of children who present with issues arising from developmental trauma and the support needs of those who care for them in foster, special guardianship, and residential placements.
- 2. Specific expertise in delivering Dyadic Developmental Informed
- Psychotherapy, PACE-based interventions and delivering PACE-based
- training, would be an advantage. Opportunities to receive further training in this area will be provided.
- 3. Detailed understanding of the intersections of developmental trauma with neurodiversity and other psychological/emotional needs will be essential.
- 4. Candidates will also have a passion for working with care experienced

Job summary
The Critical Care Outreach team (CCOT) covers 2 hospital sites 24/7 at Eastbourne District General Hospital and Conquest Hospital.
We are looking for an enthusiastic, highly motivated nurse with extensive critical care experience to join the Outreach team. This is an ideal opportunity for a critical care who wishes to expand their clinical skills, to work autonomously within a supportive and progressive team. The post holder will be expected to work flexibly across both hospital sites (12 hour shifts with internal rotation of both nights and weekends).
Main duties of the job
At EDGH and CQ, CCOT is led by a Senior Matron for Critical Care. The service provides expert assistance and support to acutely and critically ill patients outside the critical care unit. This includes patients recovering from critical illness and those at risk of deterioration (in ward and emergency care environments). You will work in collaboration with ward based teams and take part in education programmes for all groups of staff.
The post holder will complete an orientation period in accordance with the Trust's induction programme. You will also be supported completing the National CCO Professional Framework.

Mayfield Surgery is looking for one or two Salaried GPs to join our friendly and supportive GP Training Practice in Roehampton, South West London.
We have flexibility to offer 37 sessions per week across one or two posts and welcome applications from GPs at different stages of their careers. No particular special interest is required.

We are seeking a motivated, compassionate and values-driven practitioner to join Leeds Community Healthcare NHS Trust's new Proactive Support Service.

Main duties of the job
This role would particularly suit a Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD). However, we also welcome applications from Registered Social Workers and other suitably qualified and registered practitioners who can demonstrate relevant experience and skills in supporting children and young people with complex needs, trauma, behavioural challenges, and placement instability.
We are looking for a reflective and innovative practitioner who is passionate about trauma-informed and relational approaches to supporting children and young people.

This year CATAT have been given the opportunity to expand the team with several different posts, some of which have already been filled. But we do have a post that has now become vacant for a CNS - this CNS needs to have their qualification of Non-Medical Prescriber as well as a Mental Health Nursing qualification.
CATAT who after our first year of being live (2023) was highly recommended for the Working Together for Better Mental Health at the staff excellence awards. Are looking for professionals that have a keen interest of working with CYP and their families, wanting to help them achieve better outcomes for their future and strive to keep them in the community.
The aim of our team is to prevent CYP from having inpatient admissions wherever possible, and to support them and their family in the community in a proactive way. The team will serve as a gateway in and out of Tier 4 (T4) provision. CATAT work alongside the core community teams as well as partner agencies to offer CYP increased/additional support.
CATAT have 2 main pathways:
Admission Avoidance - Least restrictive option, CYP who are already open to a core team but need a more intensive intervention to support their mental health needs. To prevent things from worsening and/or needing a hospital admission.
In-reach/Outreach - support to young people leaving inpatient settings (T4) to help them adjust back into life in the community.

An exciting opportunity has arisen for a dynamic staff nurse to join First Community Health and Care working with the Responsive Services Team.
Responsive Services consists of the Urgent Community Response Team, Intermediate Care Team, Virtual Ward and Referral Hub.
This role with work in in collaboration with Primary Care and Secondary Care Colleagues, Urgent Community Response, Intermediate Care (Responsive Services), Community Nursing and other Specialist Health Care Teams across First Community, statutory and voluntary agencies to deliver excellent standards of patient care for patients who are acutely unwell requiring support to avoid admission to the acute hospital. Providing practical care, education and training for patients and carers admitted to responsive services.
Main duties of the job
You will complete assessments in the patients home and undertake treatments including IV medications, wound assessment, and management. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own home.
The candidate will be able to adapt to change and work in a busy environment. Excellent communication skills are essential to enable liaison with all our health and social care partners in the management and assessment in the community.
Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.

We are seeking to recruit a Community Nurse Practitioner to provide and support the provision of high-quality patient care. This is an exciting opportunity for an experienced, dynamic, enthusiastic, and motivated practitioner to develop new ways of working. They will be integral in delivering community services in different ways across the localities. Main duties of the job
A key part of the role will include making rapid clinical assessments of patients with undifferentiated conditions, making evidenced based treatment plans whilst aiming to ensure patients are cared for in the most appropriate place. As part of the Community Urgent Care ResponseTeam, the practitioner will be expected to respond to patients who are experiencing crisis within a 2 hour national target. You will be required to ensure delivery of high quality patient centered care measured by clinical effectiveness, patient safety and patient experience as well as meeting the key performance measures as required by the Trust.
This is an exciting role for the right candidate. You will be joining a developing and progressive service, giving you the opportunity to participate in the ongoing development of the team and the Clinical Hub. The successful candidates must have experience of service development and clinical leadership to ensure patients and their families receive timely, effective and supportive care. You will be managed and supported daily by the Advanced Clinical Practitioners, Hub Lead and Medical Lead as well as giving support to junior staff within the Dudley Clinical Hub.

The Service Desk Officer will provide a proficient NHS GM Digital & IT support service across the customer base. Provide in-depth technical remote support for all end-user equipment, systems and software supported by the IT Service. We are looking for an enthusiastic and dedicated nurse to join our integrated, multidisciplinary team to work collaboratively, deliver care meeting best practice in heart failure. It is envisaged this post holder will work with the cross-site team at Chelsea & Westminster site & at West Middlesex University Hospital site. The post holder will be expected to contribute to the service as a whole under the direction of the lead specialist nurse for heart failure. The Heart Failure Specialist Nurse provides expertise in all aspects of cardiac care provision with a specific interest & experience in Heart Failure. The post holder will be an experienced practitioner with extensive experience of cardiology & the ability to work autonomously and as an autonomous practitioner will be required to manage their own caseload including nurse-led clinics, virtual clinics, and telephone consultations. Will also provide support & education to cardiac inpatients' within the hospitals. The post holder will provide a leadership role and forge links with those involved in the development of the cardiology services both internally and externally. They will analyse service performance by undertaking regular audits, quality improvement activities and participate in the strategic development of the service. Main duties of the job
As an autonomous practitioner, manage a caseload of patient's both acute and chronic heart failure across the Trust providing specialist assessment, planning, implementation and evaluation of care. Undertake nurse-led heart failure clinics to monitor, review and optimise patient management. Work cross-site under the direction of the Trust's Lead Heart Failure Nurse and collaborate with the multidisciplinary cardiology team to provide specialist inpatient heart failure reviews, management plans and patient education. Ensure patients understand their diagnosis and evidenced based heart failure medications that improve symptoms and prognosis whilst supporting effective discharge planning, arrange follow-up care. Maintain the heart failure telephone advice service for patients and healthcare professionals. Incorporate information and communication technologies (ICTs) into patient pathways and support the development of digital personal health records and digital care pathways. Work within agreed policies, procedures and protocols using professional judgement. Utilise multidisciplinary and multi-agency resources to ensure patients receive appropriate services and referrals. Main duties of the job

Job summary
The Royal Wolverhampton Hospitals NHS Trust is one of the largest acute Trusts in the West Midlands. The Heart and Lung Centre, which opened in October 2004, has established itself in the West Midlands. The Centre delivers approximately 1000 adult cardiac surgical cases including minimally invasive aortic and mitral valve surgery, major aortic surgery. The unit's activity also includes 400 thoracic surgical cases, 1,000 PCIs, 1,000 diagnostic angiograms, a Primary PCI and TAVI program.
For this replacement post we seek applications from knowledgeable, innovative, and enthusiastic Senior Clinical Perfusionists who wish to join a well-established and like-minded clinical perfusion team. The success of this team is as a result of full involvement of all team members and an inclusive team approach.
The successful applicants should have relevant experience and be conversant with a broad spectrum of cardiopulmonary support techniques, although training will be given where necessary. You will be an Accredited Clinical Perfusionist with registration as such with the College of Clinical Perfusionists of Great Britain and Ireland.
You will be expected to participate fully in Clinical Perfusion's 24hr on-call commitment. The working week is 37.5 hours, spread over a 3/4 day week.

An exciting opportunity has arisen for a motivated and compassionate experienced registered nurse to join the Specialist Palliative Care Service at UHDB as a Specialist Palliative Care Development Nurse.
This developmental Band 6 role offers a unique 18-month rotational fixed term contract or secondment across our specialist palliative care services, including the Nightingale Macmillan Inpatient Unit, Ward 2 (EOLC ward) at FNCH, Hospital Palliative Care Team at RDH + QHB and Community Palliative Care Team across Derbyshire. As a supernumerary member of the team, you will receive structured support and development to build the knowledge, skills and confidence required to become a specialist practitioner in palliative and end-of-life care.
Working alongside experienced nursing and medical colleagues, you will contribute to the assessment, planning, implementation and evaluation of holistic, person-centred care for patients with complex palliative and supportive care needs. You will also provide advice, education and support to patients, families and multidisciplinary teams across a variety of healthcare settings.
The successful candidate will demonstrate excellent communication skills, resilience, professionalism and a commitment to ongoing learning and development.

- Assessing, planning, implementing and evaluating holistic, person-centred care for patients with specialist palliative and end-of-life care needs.

You will be joining a friendly and supportive Rheumatology Department which provides comprehensive Rheumatology services for Derby and Burton together with South Derbyshire/East Staffordshire and some parts of adjoining areas of Nottinghamshire and Leicestershire.
The Rheumatology Department is looking to appoint an enthusiastic and motivated Band 6 Specialist Nurse to work within the Rheumatology team.
We are progressive forward thinking department with positive approach at meeting and delivering the current challenges of the NHS. The Rheumatology Nursing Team work alongside the consultant rheumatologist, specialist pharmacist and specialist therapist.
Main duties of the job
Under direction of the Lead Specialist Nurse
- Deliver care to defined case load of patients in an out patients settings
- Conduct nurse-led clinics
- Act as the patients advocate - maintaining patient privacy and dignity
- Provide advice, support and education to patients/significant others
- Provide advice and support to junior staff and learners
- Coordinate care with MDT, including consultants, allied health professionals and primary care services

If the answer to the above is yes then this is an exciting secondment opportunity and be a part of our team in the Combined Triage Assessment Unit (CTAU)
There will be a requirement to work weekends and Bank Holidays on a rota basis.
Main duties of the job
We are ideally looking for an experienced Registered Nurse with demonstrable post registration experience with acute and/or Oncology/Haematology experience, who will thrive in our bespoke assessment unit .
We are in search of driven, compassionate and forward thinking nurses to join our team to support the cancer patient journey when they become unwell as a result of their treatment.
You must be adaptable, able to thrive under pressure, a great team player, be able to empathise and to remain professional and positive during challenging situations.
The role entails telephone triaging, assessing, planning and implementing care to acutely unwell patients. We have a 24hr triage line where patients undergoing treatments for their cancer diagnosis can call the line for advice and assessment. For the most, our patients will attend CTAU for a period of a few hours then are discharged home with a plan, some patients will be admitted directly to one of our cancer wards.
CTAU is open 7 days a week currently 7:30 am- 8 pm week days and 8am - 6pm weekends and BH (weekend and BH hours may change to make the week days in the future).
Winter hours are staggered and extended from 08.30 am - 9pm due to higher activity.
